Cincinnati firefighters do far more than put out fires: they respond to medical emergencies, conduct rescues, investigate arson, handle hazardous materials and help prevent emergencies through community work. The city’s fire department blends that broad modern role with a history that reaches back to its 1853 founding as what the City calls the first professional, fully paid fire department in the United States.
That story runs from steam-powered engines and rooftop fire signals to EMT-qualified firefighters, paramedics and formal recruit training. Here is what the Cincinnati Fire Department does, how its history shaped it and what the City’s published information says about training its recruits.
What does a firefighter do besides fight fires?
The Cincinnati Fire Department (CFD) responds to structure and brush fires, vehicle crashes, medical emergencies, false alarms and other hazards. Its listed services also include emergency medical services and advanced life support (ALS) transport, rescue, arson investigation, fire prevention, hazardous-device response and hazardous-material response.
The City’s recruiting description adds public engagement, community risk reduction, customer service and continuing education. The department’s mission, as stated by CFD, is to “protect lives and property, and to minimize the suffering of its customers during emergencies.”

What happens inside a fire station?
A station-based department must be ready for different emergencies, not just fires. CFD describes work that calls for rapid assessment, gathering information and teamwork; firefighters may also have their sleep interrupted by calls during a shift.

These are figures from the department’s published overview, not a live deployment report. The reviewed City sources do not establish current call volume, response-time performance, staffing totals or station-by-station assignments.
How do Cincinnati firefighters respond to medical emergencies?
Medical response is part of the fire department’s work. The City says all Cincinnati firefighters must be Ohio EMT-certified, and recruits can complete EMT training in recruit school. It also says at least one paramedic is present on each emergency response; paramedic certification is not required of every firefighter.

CFD’s listed medical services include emergency medical services and ALS transport. That means a fire department response may involve medical care as well as fire suppression or rescue, depending on the emergency.
How did Cincinnati get its first fire department?
The City dates Cincinnati’s fully paid professional fire department to April 1, 1853. It says interest in better firefighting technology grew after a fire at Miles Greenwood’s Eagle Ironworks. Greenwood, who co-invented a practical steam-powered fire engine with Abel Shawk and Alexander Bonner Latta, became the department’s first chief.
Cincinnati received its first named engine, “Uncle Joe Ross,” on January 1, 1853. Residents funded a second engine, “Citizen’s Gift.” The City says the practical steam engine could begin pumping from a water source in 10 minutes. By 1863, the department had replaced its hand-engines with steamers, and the City credits Cincinnati as the first to use horses to pull fire engines.
From fire tower to emergency communications
Before modern communications, Cincinnati used a Fire Tower on the Mechanics Institute roof at Sixth and Vine. The department’s history dates its beginning to 1845: watchmen illuminated one to four balls to signal a fire’s district and rang a large bell.

The department’s emergency-management page says Fire and Police Communications moved to a new facility in May 2005, with 800 MHz radio and a new telephone system; computer-aided dispatch technology was then being implemented. This is historical context, not a description of today’s dispatch workflow.
What does recruit training look like?
Recruit training combines firefighting and emergency medical preparation. As a dated example, the City reported on February 3, 2025, that Recruit Class 123 had 52 members—42 men and 10 women—and a planned 31-week course.

These numbers describe that class, not a current or universal training schedule. Fire Chief Frank McKinley said in the City’s 2025 announcement: “Recruit Class 123 will carry forward our legacy of providing the best customer service possible to the citizens of Cincinnati, while also ushering in a new generation of firefighting.”

The department’s history identifies Paula Duncan, Vickie Goodson and Pateeser Jackson as the first women hired in Cincinnati’s Fire Service, in 1984. It also identifies Herbie Baines as its first African American male member. The City says Baines was promoted to Squad 52 in 1955 and later left the division after experiencing substantial racial pressure.

What should prospective applicants know?
The City’s published applicant process includes a written exam, physical-ability test and structured oral interview, followed by background and physical examinations. Its requirements page says there is no residency requirement at application, while members at appointment must meet a specified Hamilton County, adjacent-county or 35-mile rule. Because hiring rules can change, prospective applicants should verify current requirements on the City’s page before applying.
How can residents reduce fire risk at home?
Fire prevention is part of CFD’s work. Residents looking for smoke-alarm guidance should start with the City’s own fire-department resources, which link to smoke-alarm information. This overview does not endorse a particular alarm brand.
