Spoilers for the completed Oshi No Ko manga: Ai Hoshino is physically stabbed by Ryosuke Sugano, an obsessive fan. The larger mystery involves Ai’s biological father, Hikaru Kamiki: Aqua believes his father was an accomplice in her murder, but that is not the same as saying Kamiki personally stabbed her.

Who physically killed Ai?

Ryosuke Sugano is identified in a secondary plot reference as the person who stabbed Ai. That answers who carried out the attack, but it does not by itself explain whether anyone else helped bring it about or what that person intended.

What role does Ai’s father play?

The official anime introduction describes Aqua as vowing revenge on his biological father, whom he believes to be an accomplice in Ai’s murder. The wording establishes Aqua’s suspicion; it does not say that his father was the physical attacker. Hikaru Kamiki should therefore be described as the suspected collaborator, not simply as the person who stabbed Ai. The official synopsis does not settle every nuance of Kamiki’s intent.

Does the answer differ between the manga and anime?

Story scope What the available official descriptions establish
Completed manga Shueisha identifies volume 16 as the final volume and describes it as the climax of Aqua’s revenge story, with the meaning behind Ai’s “15 Year Lie” revealed. The attacker’s name, Ryosuke Sugano, comes from a secondary plot reference.
Anime, as described for Season 3 KADOKAWA’s Season 3 synopsis says Ruby is still pursuing the truth about Ai and Gorou’s deaths. It does not present the manga’s completed ending as already adapted.

So an anime-only viewer may not yet have seen the full manga answer. The anime’s official descriptions frame the investigation as ongoing, while the manga’s final volume closes its revenge arc.
