What Causes Altitude Sickness, and When Should You Descend?

Reduced oxygen availability at altitude can cause illness, especially after rapid ascent. Learn the warning signs and the rules for stopping or descending.

Altitude sickness happens when reduced air pressure at elevation lowers the oxygen available with each breath. The risk rises with faster ascent and more time spent high, especially sleeping high. If you feel ill after ascending, do not sleep at a higher altitude; descend if symptoms worsen despite rest or treatment at the same elevation. Confusion, trouble walking straight, or severe breathing problems may signal a life-threatening emergency.

Why altitude causes sickness

As altitude increases, air pressure falls. That reduces the partial pressure of oxygen in the air you breathe, placing the body under hypoxic stress. At about 3,050 m (10,000 ft), inspired oxygen pressure is 69% of its sea-level value, according to the CDC Yellow Book 2026 chapter on high-altitude travel, published April 23, 2025.

How much this affects someone depends on the altitude, how quickly they ascend, how long they remain there, and individual susceptibility. Oxygen levels can be lowest during sleep, so visiting a high point during the day and returning to a lower sleeping altitude may cause less stress than spending the night high.

The body needs time to adjust. Acclimatization begins after ascent and the acute process unfolds over the first 3–5 days. Gradual ascent reduces risk, but it cannot guarantee that symptoms will not occur. For context, the CDC reports acute mountain sickness in about 25% of visitors sleeping above 2,450 m in Colorado. Rates can approach 50% in some rapid-ascent situations to destinations above 3,400 m, and prevalence can approach 30% at higher elevations on some standard Everest Base Camp itineraries. These figures describe specific locations and ascent conditions, not the risk for every traveler.

How to reduce risk while ascending

Plan around sleeping altitude rather than the highest point reached during the day. The CDC recommends that, once above 3,000 m (9,800 ft), travelers avoid increasing sleeping altitude by more than 500 m (1,650 ft) per day and add an acclimatization night for each 1,000 m (3,300 ft) of sleeping-altitude gain. Spending 2–3 nights around 2,450–2,750 m before going higher is described by the CDC as markedly protective. These are general planning recommendations; terrain, itinerary, and personal susceptibility differ.

  • Ascend gradually, with rest or acclimatization nights built into the itinerary.
  • Take account of how high you will sleep, how quickly that sleeping altitude will increase, any prior serious altitude illness, and how readily you could descend or get help.
  • Recognize symptoms early. A pulse oximeter may help gauge acclimatization progress, but a reading does not override symptoms or make continued ascent safe.
  • If you have coronary artery disease, chronic pulmonary disease, preexisting hypoxemia, obstructive sleep apnea, sickle cell trait, or another significant health concern, discuss high-altitude travel in advance with a physician familiar with altitude issues.

Recognize the main altitude-illness syndromes

Altitude-related symptoms can overlap, and other conditions can cause similar complaints. A symptom list alone cannot establish a diagnosis. The distinction matters because neurological changes and worsening breathing need urgent action.

Syndrome Typical clues Urgency and response
Acute mountain sickness (AMS) Headache after ascent, often with appetite loss, dizziness, fatigue, nausea, or sometimes vomiting. It commonly begins 2–12 hours after arriving at altitude or going higher, often during or after the first night. Do not ascend to sleep higher while symptomatic. Descend if symptoms worsen despite rest or treatment at the same altitude.
High-altitude cerebral edema (HACE) Altered mental status, confusion, poor coordination or trouble walking straight (ataxia), or unusual drowsiness. Coma can follow. Medical emergency. Arrange urgent descent and emergency help; in remote settings, initiate descent. Oxygen and dexamethasone may be used when available under appropriate medical guidance.
High-altitude pulmonary edema (HAPE) Chest congestion, cough, shortness of breath that is disproportionate to exertion, or declining exercise ability; later signs can include breathlessness at rest, respiratory distress, or bloody sputum. Medical emergency. Descent is urgent and generally necessary; give oxygen if available and minimize exertion. HAPE can occur with or without AMS or HACE.

HAPE can become fatal more rapidly than HACE, according to the CDC. Do not wait for every listed symptom to appear before acting if breathing is deteriorating.

When to stop ascending or descend

The practical rule is clear: never continue to a higher sleeping altitude while experiencing altitude-illness symptoms, even if they seem mild. The CDC advises: “Never ascend to sleep at a higher altitude when experiencing symptoms of altitude illness, no matter how minor the symptoms seem.” If symptoms become worse despite rest or treatment at the same elevation, descend. The CDC’s guidance does not treat medication as a substitute for descent.

  • Symptoms consistent with mild AMS: Stop ascending and do not sleep higher. Monitor how you feel at the same elevation; descend if symptoms worsen despite rest or treatment.
  • Possible HACE: Confusion, altered mental status, poor coordination, trouble walking straight, or unusual drowsiness calls for urgent descent and emergency medical help. In remote settings, start descending; oxygen and dexamethasone may be used when available under appropriate medical guidance.
  • Possible HAPE: Worsening breathlessness, cough, or respiratory distress calls for urgent descent, oxygen if available, and minimal exertion. If immediate descent is impossible, oxygen or a portable hyperbaric device may be critical while help is arranged. A portable pressurization bag is specialist expedition equipment, not a general substitute for a workable descent plan.

At a medical facility, selected mild HAPE cases may be managed with oxygen and bed rest under clinical care. The CDC says descent and oxygen are much more effective than medication for HAPE.

How long symptoms may last

AMS commonly resolves within 12–48 hours when a traveler does not ascend further, according to the CDC. That timeframe is not a reason to wait when symptoms are worsening, or when signs suggest HACE or HAPE; those situations require urgent action.
