Do not rub the skin or thaw it if it could freeze again. Field rewarming is appropriate only in specific circumstances; when in doubt, protect the person and seek medical help promptly.
How do I know if I have frostbite?
Early frostbite can begin with skin redness or pain. More concerning signs include skin that has turned white or grayish, feels unusually firm or waxy, or is numb. Numbness can hide the severity of an injury, so check companions and exposed skin rather than relying on pain alone. The CDC advises: “At the first signs of skin redness or pain, get out of the cold and protect any exposed skin.”
These signs can indicate a cold injury, but they cannot establish how deeply tissue is damaged or predict its outcome. Treat a suspected injury seriously and arrange medical evaluation.
What should I do for frostbite while hiking?
- Stop further exposure. Move toward shelter or a safer location and protect the person from wind and wet conditions. Remove wet clothing when feasible, and warm the body’s core under dry layers while arranging care.
- Check for more urgent problems. Look for hypothermia or serious trauma and assess whether immediate evacuation is needed. These emergencies may take priority over treating a frozen finger or toe.
- Protect the injured part. Remove rings or other constricting items before swelling makes them difficult to remove. Protect frozen tissue from impact and friction; do not rub or massage it.
- Decide whether thawing can be kept safe. If the tissue might refreeze after thawing, do not start field rewarming. If it can remain warm and the conditions for controlled rewarming are met, use the water-bath method below.
- Protect and evacuate. After rewarming, dry the area gently by air or blotting, cover it with dry bulky dressings, and elevate it if feasible. Continue toward medical evaluation.
Can frostbite be rewarmed if it might freeze again?
No. Refreezing thawed tissue can cause further injury. The Wilderness Medical Society (WMS) says to keep frostbitten tissue frozen until a thawed state can be maintained when refreezing is a risk. Make the decision based on the party’s ability to keep the area warm after thawing and the time and route to definitive care—not simply on whether warm water is available.

How warm should the water be to rewarm frostbite?
For appropriate field rewarming, the WMS 2024 guideline recommends a monitored water bath at 37–39°C (98.6–102.2°F). Keep the water circulating and replenish warm water carefully to maintain the range. Use a thermometer if available; if not, a caregiver’s healthy hand can help judge tolerability. Keep the injured part from touching the heated container, and do not use water that is hotter than the recommended range.
Rewarm until the tissue is red or purple, soft, and pliable. This often takes about 30 minutes, but the time varies with the injury’s depth and extent; the endpoint, not a fixed duration, matters. Then dry gently, protect the area with a dry bulky dressing, and continue arranging medical care. Field rewarming does not replace clinical assessment.

Should you rub frostbite to warm it up?
No. Do not rub or massage frostbitten skin, apply snow or ice, or warm it directly against a fire, stove, radiator, heat lamp, heating pad, or electric blanket. These methods can injure tissue further, and numb skin may be burned without the person noticing. Hand warmers are not a treatment for established frostbite and should not be placed directly against injured or other skin.
Can I walk on a frostbitten foot during evacuation?
Avoid walking on frostbitten feet when possible because movement and pressure can worsen injury. Do not let this caution prevent necessary evacuation. If walking is unavoidable, protect and immobilize the foot as much as possible, padding and splinting it where feasible while minimizing movement and trauma.

What if the person may also have hypothermia?
Frostbite can occur alongside hypothermia. The CDC lists shivering, exhaustion, confusion, fumbling hands, memory loss, slurred speech, and drowsiness among adult warning signs. A body temperature below 95°F (35°C) warrants immediate medical attention. Treat suspected severe hypothermia as an emergency: protect the person from further cold, warm the core under dry layers, and arrange urgent evacuation or medical help.
What will clinicians assess?
Field observers cannot reliably judge tissue depth or final viability from appearance alone. Clinicians assess the injury and manage pain, wounds, and blisters; for selected deep injuries, hospital teams may consider time-sensitive treatments such as thrombolysis or iloprost. These decisions depend on clinical assessment and local protocols. Do not attempt medication or blister treatment on your own.

How can I reduce frostbite risk on a mountain?
- Limit cold exposure where feasible and protect skin from wind and moisture.
- Keep extremities dry; avoid excessive perspiration by adjusting layers and pace.
- Use adequate insulation, including mittens when appropriate, and minimize exposed time.
- Check yourself and companions regularly for pain, redness, or numbness. WMS advises warming digits with paresthesia as soon as possible because the time they can remain numb before frostbite develops is unknown.
- WMS names chemical and electric warmers as prevention aids. Let warmers reach near body temperature before activation, keep them from touching skin directly, and ensure they do not constrict circulation inside boots.
WMS does not consider emollients a reliable prevention method and notes they may increase risk. Clothing, insulation, and warmers can help reduce exposure, but they do not treat frostbite that has already developed.
