Andrew Tanner arrives with a bullet lodged in his head and insists he is Patrick Ferguson, a Scottish soldier associated with the British army. In reality, Tanner is an American Revolutionary War reenactor. The mismatch between his present-day life and the identity he claims drives the team’s investigation.

As Watson and the fellows assess him, they find a worsening tremor in Tanner’s hand and questions about the circumstances and timing of the shooting. Tanner is also afraid that his symptoms point to Huntington’s disease. The team’s investigation overturns that fear: the recap identifies his condition as Wilson’s disease, which the episode presents as treatable. Tanner chooses to pursue treatment.
That diagnosis is a fictional plot point, not medical guidance. The episode uses Tanner’s fear of a serious inherited illness to raise the stakes, then redirects the case toward a different explanation.

Watson pushes them to reason beyond their individual specialties, and the team identifies the condition.
