The finale turns the boys’ hunt for semaglutide into a joke about insurance blocking access, then gives its anti-fat-shaming message a deliberately uneasy finish: Cartman welcomes it because he thinks it lets him keep insulting people without being mocked for his weight.
What happens at the end of South Park: The End of Obesity?
The boys’ effort to get semaglutide runs into resistance from the mothers and the sugar-mascot cartel. During the final vehicle chase, Tony the Tiger pulls Kenny from the truck and throws him into a windshield. When the group reaches the shipment they have been pursuing, the medication is not there. An insurance representative appears instead, restoring the story’s health-care obstacle just as the characters seem close to getting around it.
In the closing scenes, Randy finds Sharon about to take an injection and tells her he has been using the drug. Seeing her reaction changes his approach: he stops the injection plan and invites her to take MDMA with him, as they did in college. At school, Kyle gives a speech against fat-shaming. Cartman is pleased—not because he has embraced Kyle’s argument, but because he thinks people will no longer mock his weight while he continues insulting them.
What does the insurance truck reveal mean?
The missing medication and the insurance representative are the final punchline to the special’s central access problem. The characters have spent the story trying to get weight-loss medication; the apparent payoff is another gatekeeper rather than the drug. The reveal makes the insurance system, not just the cost or the characters’ schemes, the obstacle that remains when they reach the truck.

The official synopsis describes the special as a story in which new weight-loss drugs have a major impact on South Park, with Cartman denied access. That is promotional framing for a fictional satire, not medical advice or a factual account of how semaglutide access works. The special premiered May 24, 2024; South Park Studios credits Trey Parker as its writer. South Park Studios’ title page and Paramount+’s listing provide the official title information.
Why is Cartman happy after Kyle’s speech?
Kyle argues against fat-shaming, but Cartman immediately filters the message through self-interest. He sees a social norm that may protect him from comments about his weight, while offering no comparable reason for him to stop being cruel to others. His reaction undercuts a simple, uplifting reading of the speech: a compassionate idea can be welcomed for selfish reasons without changing someone’s behavior.

That interpretation follows the sequence of the closing scenes; no creator statement confirming a specific meaning for the ending is established here. The joke works through the contrast between Kyle’s broad appeal for kindness and Cartman’s narrow calculation about what he can get away with.
What do Randy and Sharon’s final scene add?
Randy’s conversation with Sharon shifts the ending from the chase’s public scramble over medication to a private moment between spouses. He stops her before the injection, and his invitation to revisit their college-era drug use is an abrupt, comic alternative—not a practical solution to the access problem the truck reveal leaves unresolved.

The scene also avoids giving the special a tidy answer about weight-loss drugs. It ends on character comedy and a relationship beat rather than a recommendation about whether someone should use semaglutide.
