Nick Marsh is among them—and while the episode puts his life in doubt, it resolves his immediate medical crisis.
What happens in the Season 22 finale?
“Bridge Over Troubled Water,” Season 22’s 18th episode and finale, aired May 7, 2026. The collapse of the Royer Bridge triggers a mass-casualty response at Grey Sloan Memorial. ABC’s episode listing describes the hospital treating “a surge of critically injured patients, including one of their own.” The episode runs 43:02 and is rated TV-14, according to ABC’s episode page.
Does Nick survive the bridge collapse?
Yes. Nick Marsh arrives at Grey Sloan with bleeding in his liver and spleen. Cass Beckman, played by Sophia Bush, treats those injuries and asks a colleague to keep watch over Nick’s transplanted kidney.
Meredith spots a kidney complication
Meredith wants to operate but is kept out of the operating room. When the team notices Nick is producing no urine, she connects the sign to a possible clot in a vein—a concern informed by his earlier post-transplant complication. Winston removes the clot, and Nick’s kidney responds. Shondaland’s May 8, 2026 recap says Nick is expected to recover.

What happens to Owen and the bridge victims?
Owen Hunt is thrown into the water when the bridge collapses. He escapes his vehicle and helps people trapped in the wreckage, including a family in a car. He performs an emergency tracheotomy on Sam and amputates Lainey’s trapped leg to stop severe bleeding. These are fictional events in the episode, not medical instructions. Shondaland reports that Josh and his family survive.
The episode’s scale is underscored by the report that more than 60 cars were on the bridge when it collapsed, a detail from the story as recounted by Shondaland—not a real-world bridge statistic.
