Spoilers for Fallout season 1, episode 4, “The Ghouls.” Norm and Chet find that Vault 32 had already collapsed before the raiders arrived: its residents are dead, its walls carry the messages “We know the truth” and “Death to management,” and its door was opened from outside with Norm’s mother Rose’s Pip-Boy. The episode connects Rose’s device to the vault’s breach, but does not explain the full truth behind Vault 32’s downfall. Meanwhile, Lucy escapes the Ghoul’s control and chooses to continue alone.
What happens in Fallout episode 4, “The Ghouls”?
Released April 10, 2024, and running 49 minutes, “The Ghouls” follows two parallel stories: Lucy’s struggle to survive while in the Ghoul’s custody, and Norm and Chet’s investigation of the seemingly abandoned Vault 32. The episode was written by Kieran Fitzgerald and directed by Daniel Gray Longino.
Lucy and the Ghoul reach a breaking point
As Lucy and the Ghoul cross the wasteland, they encounter Roger, an old friend of the Ghoul’s who is nearing ferality and has run out of the chems that keep him stable. Roger cannot help the Ghoul. The Ghoul kills him and eats from his body to survive, exposing both the brutal choices of wasteland life and the Ghoul’s own dependence on chems.
When the Ghoul is weakened, Lucy tries to escape. She bites off one of his fingers; he retaliates by cutting off her index finger. At the Super Duper Mart, he trades her to an organ-harvesting operation in exchange for chems, then collapses outside.

Lucy escapes the organ traders
Inside the store, the courteous Mister Handy robot Snip-Snip replaces Lucy’s missing finger, then reveals that it intends to harvest her organs. Lucy disables the robot and forces the traders to release their captives. When Martha, a ghoul close to going feral, attacks, Lucy shoots her.
Lucy later leaves chems for the Ghoul and walks away alone. He recovers and takes supplies from the store. Her choice to leave him the chems does not undo his betrayal, but it marks a shift: she is no longer simply a captive following his lead.

What do Norm and Chet discover in Vault 32?
Norm is suspicious of the raider attack and of the inaccessible records in Vault 32. He and Chet enter the neighboring vault and find long-dead residents, wreckage and signs of violence within the vault. On a wall, they find the phrases “We know the truth” and “Death to management.”
Norm also discovers that Vault 32’s door was opened from outside using his mother Rose’s Pip-Boy. That is the episode’s clearest link between Rose and the vault’s breach—and an important clue connecting the vault to the raider attack.

What is the “truth” in Vault 32?
Episode 4 establishes that Vault 32’s residents were already dead and the vault had suffered a violent collapse before Norm and Chet entered it. It also shows that someone opened the door from outside with Rose’s Pip-Boy. Those clues tell viewers that the vault’s destruction predates the raider attack and that Rose’s device was involved in opening its door.
The episode does not reveal who opened it, why the residents turned against management, or the complete cause and timeline of the vault’s collapse. The wall message is a clue and a question, not a complete explanation.

Why Lucy’s departure matters
The episode changes the balance between Lucy and the Ghoul. She resists his control, survives the organ traders, and leaves on her own terms. Giving him chems before she goes complicates the break: Lucy refuses to stay with him, but does not leave him to die.
“The Ghouls” is episode 4 of Fallout’s first season. The episode reference lists a 49-minute runtime and an April 10, 2024 release date. What to Watch reported on April 12, 2024, that all eight episodes were streaming on Prime Video at that time; that is a historical availability note, not confirmation of the current catalog in every region.
