No cure is ever made in the original game. But in 2025, director Neil Druckmann clarified that the creators intended the Fireflies could have made one from Ellie. That is a statement about the story’s intended possibility—not proof that their plan was medically sound or morally justified.
What happens to the cure in the game?
Ellie is immune to Cordyceps infection, and the Fireflies believe that studying her could lead to a cure or vaccine. At the hospital, their doctors plan an operation that would kill her. Joel rescues Ellie before it happens, and no cure is produced.
Later, Joel tells Ellie that the Fireflies had found other immune people, failed to make a cure, and stopped looking. That account is a lie: the official Naughty Dog podcast discussion of the game’s ending recounts Joel’s deception and the choice behind it.
Could the Fireflies have made a cure?
According to Druckmann, yes—that was the creators’ intent for the original game. In a 2025 appearance on the Sacred Symbols podcast, as reported by GameSpot, he said: “Our intent was, ‘yes, they could.’” He also acknowledged: “Our science is a little shaky and people are now questioning it.”

- Aluminum Alloy Housing: Built with a sturdy aluminum alloy shell that aids in heat dissipation and protects against daily wear and scratches. Designed to maintain a stable and secure connection.
- Compact & Travel-Friendly: The ultra-compact design allows the adapter to stay plugged into your device without blocking adjacent ports or adding bulk, reducing wear and tear on your original USB ports.
- 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.
Those statements answer two different questions. Within the story as its creators intended it, the Fireflies could have made a cure. The fictional explanation does not establish that their procedure would work by real-world medical standards, and the cure is never actually developed in the game.
Was Joel right to save Ellie?
The game leaves the moral judgment open. Joel prevents a possible cure at the cost of Ellie’s life, but the Fireflies’ plan would kill a child who had not agreed to that sacrifice. The official podcast frames the ending around whether love can justify choosing to save the people closest to us; Druckmann’s commentary does not turn the dilemma into a simple verdict.

So “Joel doomed humanity” is one interpretation, not an established outcome. The story gives players reason to question both Joel’s decision and the Fireflies’ willingness to proceed without Ellie’s consent.

The HBO series preserves the central hospital dilemma: the Fireflies believe Ellie’s immunity could help them make a cure, and the planned procedure would kill her. Its finale makes clear that Ellie has not agreed to that cost.

The show is an adaptation rather than a scene-for-scene copy. Naughty Dog describes the game and series as distinct experiences built around Joel and Ellie’s relationship in its game-and-show comparison. The television version also changes infection details, including the use of tendrils rather than the game’s spores, as discussed in TVLine’s finale coverage and ABC News Australia’s adaptation comparison. The first season retains the same basic unresolved question: a cure is possible in the characters’ beliefs, but none is made.
