Billy Joel says he is okay, but his balance is still poor and he has lost some hearing. In his latest reviewed update, reported by PEOPLE on October 2, 2026, he said he was recovering after shunt surgery and following doctors’ advice to pause performing. His earlier “I feel good” remark came in July 2025, before he publicly disclosed the surgery.
What is Billy Joel’s diagnosis?
Joel’s official website announced on May 23, 2025, that he had been diagnosed with normal pressure hydrocephalus (NPH). The statement connected the condition with problems affecting his hearing, vision and balance, and said he was receiving specific physical therapy. It also said his doctor had advised him to refrain from performing during recovery. Joel’s official announcement did not give a date for his return to the stage.
Joel has recounted that doctors told him his concert performances had exacerbated the condition. That is his account of what he was told; it does not establish that loud music caused NPH.
Is Billy Joel okay now?
In comments made September 29, 2026, and reported by PEOPLE on October 2, Joel said, “I know people are worried about me because they read that I have a brain disorder, which sounds a lot worse than it actually is,” adding, “I’m okay.” He also said his balance was still not good and that he had lost “a little bit” of his hearing. He suggested age and loud music may have contributed to the hearing loss. These are Joel’s personal descriptions, not an independent medical assessment. PEOPLE’s October 2, 2026 report also says he was following doctors’ instructions and pausing performances while he focused on recovery.

What did Billy Joel mean when he said, “I feel good”?
In July 2025, Joel told Bill Maher on the Club Random podcast, “I feel good.” In the same period, he made clear that he was still dealing with symptoms: “My balance sucks. It’s like being on a boat,” he said, adding that the condition was still being worked on. The positive remark was an update on how he felt at that time, not an announcement that NPH had resolved. The July 2025 Club Random conversation records his comments.
In a separate interview reported in July 2025, Joel said, “It was scary, but I’m okay.” The People interview provides that earlier update.

Did Billy Joel have brain surgery?
Yes. In September 2026, Joel disclosed on The Billy Joel Channel that he had undergone surgery to have a shunt implanted in his brain. He said, “I had a surgery and they implanted a shunt in my brain — and it actually improved things.” He described improvement in memory, balance and his ability to function, and said he was continuing physical therapy and following doctors’ instructions. This reports Joel’s experience; it is not a claim that the procedure cured his condition or that the same outcome applies to others. SiriusXM’s September 2026 account has his disclosure.
