How this ranking works
The list favors distinct horror films, rather than counting the same twin pair or story twice. Classic and modern films appear together because the ranking is based on those effects, not release era. 1. The Grady sisters — The Shining (1980)

2. Niles and Holland — The Other (1972)

The quiet, psychological unease makes this one of the strongest alternatives to supernatural haunting.

3. Elliot and Beverly Mantle — Dead Ringers (1988)
David Cronenberg’s film makes doubling feel like a trap. Jeremy Irons plays identical twin gynecologists Elliot and Beverly, whose closeness and obsession pull them toward self-destruction. The unease is psychological and bodily: resemblance is not a comforting mirror, but a force that blurs boundaries and deepens their collapse.
4. Elias and Lukas — Goodnight Mommy (2014)
In the Austrian original directed by Veronika Franz and Severin Fiala, twin boys grow suspicious of their mother after she returns from facial surgery. Their isolation and uncertainty make every interaction feel unstable; the slow-building dread depends on what the boys believe as much as on what they see. Lukas and Elias are played by real-life twin brothers. This entry refers to the 2014 film, not the 2022 American remake. 5. Niles and Holland — no; Terry and Ruth — The Dark Mirror (1946)
Olivia de Havilland plays identical twins Terry and Ruth, who become suspects in a murder investigation. The psychiatrist’s effort to distinguish them gives the story its psychological and noir tension: if two people look the same, how much can appearance tell an investigator about guilt or character? Its suspense comes from uncertainty and mistaken identity rather than overt supernatural menace.
6. Danielle and Dominique — Sisters (1973)
Brian De Palma’s surreal psychological thriller centers on separated conjoined twins and a murder witnessed by a neighbor. The split between the sisters turns the body into a source of uncertainty, while the film’s disorienting atmosphere keeps the viewer questioning what is happening. Its horror lies in the fracture between apparent identity and hidden reality.

7. Seth and Jonah — Seconds Apart (2011)
Identical twins Seth and Jonah have telepathic abilities they use to manipulate other people, sometimes with fatal results. Their shared power makes the usual question of which twin is responsible less reassuring: the danger belongs to the bond itself. This is a more direct supernatural threat than the slow ambiguities of Goodnight Mommy.
8. Duane and Belial — Basket Case (1982)
In this revenge story, separated brothers reunite in a film that mixes dark humor with surreal imagery. The bodily dimension of their connection makes the twin premise especially strange, while the tonal shifts keep the horror from settling into conventional slasher territory. It is a cult-style outlier among the more restrained psychological entries.

9. Terry and Todd — Blood Rage (1987)
This slasher uses identical brothers to turn mistaken identity into a murder engine. A drive-in killing sets up later violence, and the twin resemblance keeps the threat blunt and immediate. Where some films make doubling eerie through atmosphere, Blood Rage offers a more overtly exploitative, blood-soaked contrast.
10. The twin boys — Sinister 2 (2015)
The twin boys in Sinister 2 are targets, not the story’s central antagonists: Bughuul and the ghosts of murdered children torment them. That perspective makes the twins’ shared vulnerability the point. The film’s dread comes from the threat closing in on children who are trapped in a supernatural pattern.

11. Danny Torrance and the Grady sisters — Doctor Sleep (2019)
Doctor Sleep brings the Grady sisters back through Danny Torrance’s visions. This is a legacy appearance, not a new twin pair: their power still comes from the image established in The Shining. The return makes the Overlook’s haunting part of Danny’s continuing relationship with the past.
12. Cathy and Chris — Flowers in the Attic (1987)
The Dollanganger children are locked away after their father’s death; older twins Cathy and Chris are among them, alongside their younger twin siblings Cory and Carrie. The story’s family confinement supplies the dread, making the twins’ closeness part of an oppressive situation rather than a supernatural power. Its disturbing family themes are best approached as psychological horror, not as sensational spectacle.
13. Daisy and Violet Hilton — Freaks (1932)
Tod Browning’s carnival film includes real-life conjoined twin sisters Daisy and Violet Hilton among its performers. Their presence belongs to the film’s historical world of carnival spectacle, but disability itself is not a horror device: the unsettling effect comes from the film’s events and treatment of its characters, not from the sisters’ bodies. This distinction matters when revisiting a movie shaped by an era’s dehumanizing show-business conventions.
14. The twin sisters — Twins of Evil (1971)
This Hammer horror title brings twin sisters into a gothic setting, making the contrast between likeness and individual fate part of its appeal.
