Director Osgood Perkins and Cage developed the look with the makeup team to make Longlegs seem visibly damaged by his “employment with the devil.”
Why Longlegs has that distorted face
Perkins said Cage wanted to disappear beneath the prosthetics, and the two refined the design with makeup artists. The goal was an unnerving, intentionally botched-surgery effect: Perkins described wanting to capture “the embarrassment of plastic surgery, the shame that one wears with badly done plastic surgery.” He also wanted Longlegs to look as though his service to the devil had worn him down.
That explanation is about a fictional character’s design, not a claim about Cage’s appearance or medical history. The film’s transformation was collaborative: Cage and Perkins worked with the special-effects makeup, wig and costume teams.
What inspired the character’s look
Perkins said Longlegs’s pale face was in the script and drew on Bob Dylan and the Rolling Thunder Revue. The character also began with the idea of a nervous performer who might visit children’s homes for birthday parties and put on a strange show. The pale makeup and eccentric styling help suggest that unsettling performer before the character’s full menace comes into focus.

Special makeup effects designer Werner Pretorius recalled that the team explored several concepts, including botched plastic surgery and a more extreme burn-victim look. They settled on the former and refined the face and nose shapes with Cage and Perkins.
How the practical makeup was developed
Pretorius said the team considered four designs, then reduced the makeup from an initial nine prosthetic pieces to about seven. They used a fresh set each day, explored three different noses, and spent about five hours on the first day trying options to find the character. These are Pretorius’s recollections of the production process, not independently audited production figures.

The result depends on more than the prosthetics alone. Pale makeup, the wig, costume and Cage’s performance work together to make Longlegs feel like a specific, unsettling person rather than an actor simply wearing a strange mask.
Why the reveal feels so startling
Perkins kept Maika Monroe from seeing Cage in character before their shared interrogation-room scene. Monroe said she first saw the look when filming began and described being shocked. In NPR’s account of the production, Cage is likewise described as nearly unrecognizable beneath heavy prosthetics and scraggly hair.

ELLE interviewer Samuel Maude referred to Monroe’s heartbeat reaching 170 beats per minute during the moment. That number is an anecdotal figure raised in the interview, not a verified production measurement.

Cage looks weird in Longlegs because the production designed him to: custom practical prosthetics, pale makeup and a wig created an intentionally damaged, botched-surgery appearance, while the character concept and Cage’s performance gave that transformation its disturbing personality.
