The twins then spent many weeks in the NICU. She later described being in “survival mode” and only fully processing what had happened months afterward.
Why did Beyoncé have an emergency C-section?
In a first-person essay for Vogue, published August 6, 2018, Beyoncé wrote that she was “swollen from toxemia” and had been on bed rest for over a month. She said her health and the babies’ health were in danger, so she underwent an emergency C-section.

“Toxemia” is a term commonly used in coverage of preeclampsia. The public account does not include her medical records, exact gestational age, lab results, a severity classification or a clinician’s detailed explanation of the timing. It establishes Beyoncé’s stated reason for the emergency delivery, but not a more specific clinical chain of events.
What happened to Rumi and Sir after the birth?
Beyoncé said the twins spent “many weeks in the NICU.” Her public account does not specify their individual diagnoses, treatments or the reason each needed neonatal intensive care, so those details cannot be established from what she has shared.

What did Beyoncé reveal later in Homecoming?
In narration featured in the 2019 documentary Homecoming, Beyoncé added that she had high blood pressure and that one baby’s heartbeat paused a few times in the womb. TIME reported on that account on April 17, 2019. These details came later and should be distinguished from the 2018 Vogue essay.
Why did she describe the experience as traumatic?
The emergency surgery, her concern for her own health and the babies’ safety, and the twins’ extended NICU stay help explain the gravity of the experience she described. Beyoncé called the C-section major surgery and said she needed time to heal and recover. She also reflected on becoming more accepting of her postpartum body; those remarks describe her personal experience, not a typical recovery timeline.

- Beyonce
- RENAISSANCE (Deluxe Edition)
She wrote that she had been in “survival mode” and did not grasp the experience until months later. That delayed processing is part of her account of how the birth affected her, rather than a claim that every parent responds to a difficult birth in the same way.

Her account provides a firsthand description of a high-risk pregnancy and emergency birth, while leaving many clinical details private. It does not establish the twins’ medical conditions, the precise decisions made by her care team, or that race caused her complications.

The U.S. Commission on Civil Rights’ 2021 report, Understanding Racial Disparities in Maternal Health, mentions Beyoncé among examples of severe pregnancy complications affecting Black women across socioeconomic levels. Its discussion of her individual case relies on secondary reporting; it is context for a broader issue, not evidence of the cause of her specific complications.
