Who Was the Tylenol Killer? The True Story Behind Netflix’s Cold Case: The Tylenol Murders

No one has been publicly identified as the person who poisoned Tylenol capsules in the 1982 Chicago-area murders. James Lewis became a leading suspect and was convicted of extortion, but he was never charged with the killings. Netflix’s Cold Case: The Tylenol Murders revisits the unsolved case and its competing theories; the documentary does not establish who did it.

What happened in the 1982 Tylenol murders?

In 1982, seven people in the Chicago area died after taking Tylenol capsules laced with cyanide. More than four decades later, the identity of the person who contaminated the capsules—and how the poisoning was carried out—remains unknown in the cited reporting. TIME’s 2025 account describes the case as unresolved.

The poisonings led to a major product recall. Netflix Tudum reports that Johnson & Johnson recalled five million capsules, which the retrospective describes as the largest recall in history. That characterization and figure are Tudum’s account.

Was James Lewis the Tylenol killer?

James Lewis was a leading suspect, not a person proven to have committed the murders. He sent Johnson & Johnson an extortion letter demanding $1 million in connection with stopping further killings. TIME reports that Lewis was convicted of extortion and served 12 years in prison, but was never charged with the murders. Authorities could not prove he was in the Chicago area when the deaths occurred.

Former Chicago Police superintendent Richard Brzeczek told TIME: “James Lewis was an asshole, but he wasn’t the Tylenol killer.” That is Brzeczek’s assessment, not a judicial finding. Lewis died in 2023, according to Netflix Tudum.

What is Cold Case: The Tylenol Murders about?

Netflix lists Cold Case: The Tylenol Murders as a 2025 documentary with three episodes. Its official synopsis says the series examines theories behind the unsolved killings and tracks down a key suspect. The episode descriptions move from the poisonings and the response to the suspect and later evidence raised by a victim’s daughter. Netflix’s official listing provides the episode count and descriptions.

Netflix Tudum says the documentary includes interviews with people close to the case, including Lewis. It revisits theories and testimony, but the existence of a documentary—or the claims made in it—does not prove a theory.

Did the documentary lead to a new investigation?

The Guardian quotes director Yotam Guendelman saying, “We hope by doing this documentary the FBI will look at other possible directions that they never looked at.” This states the filmmakers’ hope. It is not confirmation that the FBI reopened the case or acted on a new lead. The Guardian’s 2025 interview reports the comment.

What is known—and what remains unresolved?
