Ozzy Osbourne died on July 22, 2025, just 17 days after his final concert. In the years after publicly disclosing Parkinson’s disease, he continued recording, released Patient Number 9, received Grammy recognition, and worked toward one last appearance onstage. His health problems also included spinal injury, surgery, and serious mobility limitations, so Parkinson’s was an important part—but not the whole—of the story.

Ozzy Osbourne remained creatively active until the end of his life. He publicly disclosed a Parkinson’s diagnosis in January 2020, but he continued making music, released the album Patient Number 9 in 2022, considered returning to the stage, and ultimately performed one final time at Birmingham’s Back to the Beginning concert on July 5, 2025. He died 17 days later, on July 22, 2025, at age 76.

His final years were not a story of Parkinson’s disease alone. A fall, spinal problems, surgery, prolonged recovery, and severe mobility limitations all shaped what he could do. The public record shows an artist adapting his work around those health challenges—not someone who simply stopped working after his diagnosis.

What Ozzy Osbourne was doing before his death

  • Recording music: He released Ordinary Man in 2020 and Patient Number 9 on September 9, 2022.
  • Receiving major recognition: Patient Number 9 won the 2023 Grammy for Best Rock Album, while “Degradation Rules,” featuring Tony Iommi, won Best Metal Performance.
  • Trying to return to live performance: He withdrew from the 2023 Power Trip festival because his body was not ready for the demands of a full appearance.
  • Celebrating his career: He was inducted into the Rock & Roll Hall of Fame as a solo performer in 2024.
  • Preparing for a final farewell: He performed from a throne at the July 5, 2025, Back to the Beginning concert at Villa Park in Birmingham.

Those activities are the clearest answer to what Osbourne was up to before his death: he was still making records, receiving recognition for them, protecting his health when necessary, and working toward one carefully planned final performance.

When did Ozzy Osbourne reveal his Parkinson’s diagnosis?

Osbourne’s Parkinson’s diagnosis became public in January 2020, after a fall and a series of health problems. In interviews and public statements, Osbourne, Sharon Osbourne, and other family members discussed how the condition had affected his movement, daily life, and touring plans. ABC News reported on the family’s disclosure at the time.

#1 Best Overall

The public record does not establish exactly when his symptoms began, and it does not support assigning him a particular Parkinson’s subtype or disease stage. The most accurate wording is that Osbourne publicly disclosed a Parkinson’s diagnosis in January 2020 and lived with the condition during the final years of his life.

He continued recording after the diagnosis

Although touring became increasingly difficult, studio work remained possible. Osbourne released Ordinary Man in 2020, followed by Patient Number 9 on September 9, 2022. The albums are important to this story because they show that his career did not end when his diagnosis became public.

Patient Number 9 included collaborations with musicians including Jeff Beck, Eric Clapton, Pearl Jam’s Mike McCready, and Osbourne’s longtime Black Sabbath colleague Tony Iommi. The album’s title and subject matter prompted considerable discussion about Osbourne’s health and mortality, but it should not be treated as a medical document. It was a studio album created during a difficult period, not a clinical account of Parkinson’s disease.

The Recording Academy lists Patient Number 9 as the winner of the 2023 Grammy for Best Rock Album. It also lists “Degradation Rules,” featuring Iommi, as the winner of Best Metal Performance. These awards demonstrate the continuing artistic impact of Osbourne’s work; they do not indicate how advanced his illness was or whether his health was improving.

For readers interested in the music from this period, Patient Number 9 is best understood as Osbourne’s 2022 late-career studio album—not as an album about Parkinson’s disease.

Why he withdrew from the 2023 Power Trip festival

Osbourne had hoped to return to the stage at the Power Trip festival in California in October 2023. On July 10, 2023, however, his official website published a statement saying he was withdrawing. He explained that his body was not ready and that he did not want his first performance in almost five years to be below the standard audiences expected.

This decision is often simplified into “Ozzy could not tour because of Parkinson’s.” That leaves out important context. Associated Press reporting connected the touring pause to spinal surgery and a difficult recovery, while Osbourne’s own statement described broader physical limitations. Parkinson’s was a major part of his health crisis, particularly where mobility was concerned, but it was not the only reported reason he struggled to perform.

Health factor What the public record supports
Parkinson’s disease Osbourne publicly disclosed the diagnosis in 2020, and it affected his mobility and ability to tour.
Spinal injury and surgery These were separately reported contributors to his pain, recovery, and difficulty performing.
General physical decline His public statements described a body that was not ready for a demanding return to live performance.

There is no reliable basis for saying that a specific medication, treatment, or therapy enabled or prevented any particular performance. Nor should a public appearance be used to infer that Parkinson’s had gone into remission or that his overall health had recovered.

His 2025 farewell performance in Birmingham

Osbourne’s final public performance took place on July 5, 2025, at Villa Park in Birmingham, the city where Black Sabbath began. The event, Back to the Beginning, was presented as a Black Sabbath reunion and as Osbourne’s final performance.

Associated Press reported that Osbourne performed from a throne. The staging allowed him to appear at the center of the event without requiring the kind of movement and stamina expected from a conventional full-length rock show. It was a carefully managed final bow, not evidence that his mobility problems had disappeared.

The concert carried significance beyond one more date on a tour schedule. Osbourne had been a co-pioneer of heavy metal with Black Sabbath, built a major solo career, launched Ozzfest, and reached audiences through The Osbournes. In 2024, the Rock & Roll Hall of Fame inducted him as a solo performer, recognizing those contributions alongside his Black Sabbath legacy. The Birmingham farewell therefore brought together several parts of his public life: the hometown connection, Black Sabbath, his solo career, and his effort to remain involved in music despite serious illness.

What caused Ozzy Osbourne’s death?

Osbourne died on July 22, 2025, at age 76. His family’s initial announcement confirmed his death and requested privacy but did not immediately give a cause.

Later Associated Press reporting based on his death certificate said the listed causes included acute myocardial infarction, commonly called a heart attack, and coronary artery disease. Parkinson’s disease was also recorded in the certificate’s medical information.

That distinction matters. The available death-certificate reporting does not support saying that Parkinson’s disease alone caused Osbourne’s death. Parkinson’s was a serious condition affecting his final years and mobility, while the reported immediate causes involved cardiovascular disease.

Readers who want more background on Osbourne’s childhood, Black Sabbath years, solo career, addictions, family life, and recovery history may be interested in I Am Ozzy: The Autobiography. It is a firsthand memoir and can provide personal context that news reports cannot. It should not, however, be used as a clinical source for understanding Parkinson’s disease or as a definitive account of every medical detail in Osbourne’s final years.

Ozzy Osbourne’s final years in perspective

He ultimately achieved that return in a form suited to his condition: one hometown farewell performance, presented with careful staging and surrounded by the musical legacy he had spent more than five decades building. Seventeen days later, he was dead. The story is neither that Parkinson’s stopped him from doing anything nor that the final concert proved he had recovered. It is that he continued participating in his career while adapting to a complicated and progressively difficult health situation.
