What to do when your child is on a long waitlist for autism services

If your child is waiting months for an autism evaluation or services, do not wait passively for one appointment. Keep the specialist waitlist active, ask about earlier openings, and pursue the U.S. public evaluation pathway that matches your child’s age. Children younger than 3 can be referred directly to their state or territory’s early-intervention program; children 3 and older can be referred to the local public school system under Child Find. The CDC says a doctor referral or formal autism diagnosis is not required to request either evaluation.

You can also ask about help for specific developmental needs now. Speech therapy for a language delay, for example, often does not require waiting for an autism diagnosis, although each program sets its own eligibility rules and provider capacity. The steps below help you move several processes forward without treating ordinary activities or a public evaluation as a substitute for individualized care.

Take these steps today

  1. Tell your child’s doctor exactly what you are seeing. Describe changes or concerns in communication, play, behavior, learning, sensory responses, sleep, eating, or daily routines. Ask for developmental screening, referrals, and recommendations for services that address the specific concern. A referral to a specialist can remain active while you pursue public evaluations.
  2. Open the age-appropriate public pathway in parallel. For a child under 3, contact the state or territory early-intervention program and request an evaluation. For a child age 3 or older, contact the local public elementary school or school district and ask for the evaluation or Child Find contact, even if your child is not enrolled there.
  3. Call the clinic holding your appointment. Ask how to be considered for an earlier opening, whether the clinic keeps a cancellation list, which records it wants, whether it needs updates if your circumstances change, and what to bring. Not every clinic uses a cancellation list, so ask rather than assume.
  4. Start a simple record. Write the date of every call, the person’s name, the number or email used, what was requested, and the next follow-up date. This prevents a long wait from becoming a series of unanswered messages.

These routes can proceed at the same time. Requesting a public evaluation does not cancel a medical appointment, and remaining on a specialist list preserves an option that may provide a different kind of assessment or treatment planning.

Choose the public route by your child’s age

Child’s age Who to contact What to request How eligibility is decided Diagnosis or referral requirement
Under 3 Your state or territory early-intervention program A developmental evaluation and information about available early-intervention services The program evaluates the child; eligibility, services, and family costs differ by state or territory. Eligible services may be free or reduced cost. A parent can request an evaluation. A doctor referral or formal autism diagnosis is not required to ask.
Age 3 or older Your local public school system, public elementary school, or district Child Find contact An evaluation for developmental, learning, communication, or related educational needs The school system applies its education-eligibility rules and identifies supports available through the public system. You can ask even if the child is not enrolled there. A medical autism diagnosis is not required to request the evaluation.

These are separate public systems. A school evaluation addresses educational eligibility and supports; it is not the same as a clinical diagnosis. Early-intervention eligibility likewise depends on the program’s evaluation rather than on a diagnosis obtained elsewhere. Local rules, timelines, services, and contacts change, so confirm current details with the agency in your state or territory.

If your child is under 3: request early intervention

Every U.S. state and territory has a publicly funded early-intervention program. Contact the program directly and say that you are a parent requesting a developmental evaluation. You do not have to wait for a specialist appointment or a written autism diagnosis before making that request.

What to have ready for the first call

  • Your child’s name, birth date, address, and preferred language.
  • A short description of the concerns that prompted the call, using examples from more than one setting when possible.
  • Any medical records, prior screening results, therapy notes, or evaluation reports you already have.
  • Your pediatrician’s contact information, if you want the program to coordinate records.
  • Questions about the evaluation process, family participation, possible costs, and the services offered if your child qualifies.

Eligibility is determined by the program’s evaluation. Some families qualify because of a diagnosed condition; others qualify because testing shows a developmental delay or another state-defined basis. Do not predict eligibility from a phone conversation. Ask when the evaluation will occur, who will conduct it, and how you will receive the decision.

If your child is 3 or older: contact Child Find or the district

Call the local public school or district office and ask for the person who handles Child Find or developmental evaluations. Explain that you are requesting an evaluation for your child’s communication, learning, behavior, development, or other needs. Make the request even if your child attends a private school, preschool, home-based program, or no school; the district can explain the local process.

Keep medical and school processes distinct

A school system decides whether a child meets its criteria for special-education or related educational services. A clinician decides whether the child meets medical diagnostic criteria. One decision does not automatically produce the other. You may pursue both evaluations, share information with each team when you authorize it, and ask each system what it can provide while the other process continues.

Can services begin before an autism diagnosis?

Often, yes, for a specific concern. The CDC notes that treatment for concerns such as a language delay may not need to wait for a formal autism diagnosis. A speech-language evaluation or another targeted service may therefore be worth requesting now.

“Often” does not mean every child qualifies or every provider has an opening. Programs use their own eligibility standards, insurance rules, geographic limits, and staffing capacity. When you call, describe the need rather than leading only with the suspected diagnosis: for example, “My child has very few spoken words and becomes frustrated when trying to communicate. How can we request a speech-language evaluation?” Ask whether the provider will accept a referral from you, a doctor, a school, or a public program, and what documentation is required.

Keep the specialist waitlist active and look for an earlier opening

Call the clinic periodically, following whatever interval it gives you. Ask four focused questions:

  1. Can the appointment be moved earlier if another family cancels?
  2. Does the clinic maintain a cancellation or short-notice list, and how should you join it?
  3. Which records, questionnaires, videos, or teacher reports should be sent before the visit?
  4. Should you notify the clinic if your child’s symptoms, safety needs, insurance, phone number, or availability changes?

Be realistic about what this can accomplish. Persistence may uncover an opening, but no clinic can promise that calling will shorten the wait. If you accept a short-notice appointment, confirm the date, arrival time, location, expected duration, payment or insurance information, and whether all caregivers need to attend.

A concise message you can use

“My child is scheduled for an evaluation on [date]. We can accept a short-notice appointment. Please add us to any cancellation list you use and tell me what records or forms you need. My best phone number is [number], and I will check back on [date].” Keep the bracketed details as editable fields rather than sending them literally.

Prepare information that makes the evaluation useful

Specialists and public evaluators need concrete observations, not a perfect narrative. Gather medical records and previous evaluations, then make brief notes about what happens in real situations.

Record patterns across settings

  • Communication: words, gestures, pointing, responding to a name, understanding directions, and how your child asks for help.
  • Social interaction and play: interest in other people, imitation, turn-taking, pretend play, and reactions when play changes.
  • Behavior and regulation: triggers, duration and frequency of distress, repetitive movements or routines, transitions, and what helps your child recover.
  • Daily development: sleep, eating, toileting, dressing, motor skills, safety awareness, and any loss of a skill.
  • Context: whether the same pattern appears at home, childcare, school, with relatives, or in busy public places.

Use dates and examples: “During the last two weeks, my child pointed to request a snack twice but pulled an adult by the hand most other times.” Avoid labeling an event as simply “bad behavior” when you can describe what occurred before, during, and after it. Bring reports from childcare or school if the other setting observes different behavior.

Write your questions before the appointment. Consider bringing another trusted adult who can take notes, help with transportation, or stay with siblings. Ask the clinic in advance whether interpreters, telehealth, questionnaires, or videos are accepted.

Support development during the wait

Everyday interaction can support communication and learning while you wait. The CDC suggests reading, singing, talking, playing, making crafts, doing chores together, and going outside. Follow your child’s interests, keep activities manageable, and build in pauses so the child can communicate or choose what happens next.

Make activities responsive, not demanding

  • During a book, pause and let your child turn the page, point, gesture, or make a sound before you continue.
  • In play, imitate what your child does and add one small variation, rather than taking over the activity.
  • During chores, use short phrases and offer simple choices such as “red cup or blue cup?”
  • On a walk, notice what captures your child’s attention and name it without requiring eye contact or a verbal response.

These are supportive interactions, not a replacement for evaluation, speech therapy, occupational therapy, behavioral support, or another individualized service. Do not treat a child’s response to a home activity as proof for or against autism.

The CDC Milestone Tracker app covers ages 2 months through 5 years. It can help you organize observations and discuss milestones with a professional, but it is a tracking aid, not a diagnostic tool. Reading together is optional; no paid book, app, or tracker is required.

The CDC also maintains a state and territory early-intervention contact directory. Because agency names, eligibility rules, phone numbers, and service capacity can change, verify the current contact when you call. Keep the name of the agency, the person who answered, and the next step in your log.

Common mistakes that slow families down

  • Waiting for the diagnosis before requesting anything: Public evaluations and some targeted therapies can be requested first.
  • Calling only one system: A medical clinic, early-intervention program, school district, and speech provider may each have a different process. Contact the ones that match your child’s age and needs.
  • Assuming a school evaluation is a medical evaluation: Ask what question each evaluator is answering and what decision it can make.
  • Sending a long, undated story: A short timeline with concrete examples is easier for a team to use.
  • Promising your child a particular service: Request an evaluation and ask about options; eligibility and capacity are determined by the relevant program.
  • Stopping the specialist waitlist after one new referral: Keep appointments active until you understand what the alternate pathway offers and have a confirmed plan.

What the waiting period does—and does not—tell you

Waits can be long, but there is no established U.S. national average wait time for autism services in the guidance cited here. The CDC’s description is broader: “Unfortunately, families may have to wait many weeks or sometimes months before they are able to get an appointment to see a specialist or start intervention services for their child’s developmental problem.” That statement describes developmental services generally, not a measured autism-specific statistic.

Use the waiting period to keep parallel processes moving, document changes, and obtain help for immediate functional needs. A long wait does not establish a diagnosis, predict an outcome, or mean that your child cannot benefit from support before the appointment.

A practical follow-up rhythm

  1. Today: Contact the child’s doctor, start the age-based public request, and call the specialist clinic.
  2. Within one week: Send requested records, complete forms, and confirm that each agency received your request.
  3. Every few weeks: Check the clinic’s earlier-opening policy and follow up with the public program according to its stated timeline.
  4. Whenever circumstances change: Report new safety concerns, skill loss, major behavior changes, new insurance information, or a change in contact details.
  5. Before each evaluation: Update your notes with recent examples and write the three questions that matter most to your family.

CDC guidance on accessing autism services was updated April 15, 2025 and reviewed October 2, 2026; its early-intervention directory and “Why Act Early” guidance were updated February 17, 2026. Local rules and openings can change sooner, so confirm details directly with the current state, territory, district, or clinic contact.
