1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsAfter you donate, your blood is identified, transported to a processing centre, tested, and often separated into red cells, platelets and plasma. Components that pass the required checks are labelled, stored at controlled temperatures and sent to hospitals when requested. Hospital staff perform additional checks before anything is transfused.
The broad route is similar in many countries, but the infectious-disease tests, processing specifications and storage limits depend on the blood service and jurisdiction. The journey below describes the main stages, with United Kingdom and United States figures clearly identified where they differ.
1. The donation and its samples are given a traceable identity
Immediately after collection, the blood bag and the tubes taken for laboratory testing receive matching identifiers. In many services this is a barcode or another unique donation number. The identifier follows the donation from the collection venue through processing, testing, storage and delivery, while the sample tubes remain linked to the same record.
This traceability is a safety control, not just an administrative convenience. Staff can connect a laboratory result to the exact component made from that donation and can identify where that component was sent. NHS Blood Donation describes barcode traceability from the donation venue through processing and delivery.

Transport protects the donation before processing
Collected blood is moved from the donation session to a processing site in conditions intended to protect its quality. The accompanying samples travel to a laboratory. Transport requirements vary by component and service, but the objective is consistent: keep the donation identifiable and within the conditions needed for reliable testing and processing.
2. Whole blood is turned into components
A standard whole-blood donation contains several useful components. At the processing centre, the unit may be centrifuged so its layers can be separated into red blood cells, platelets and plasma. White blood cells are removed from many components by filtration, a process called leukocyte reduction. Reducing leukocytes is intended to lower certain transfusion reactions and other risks.
Separating the donation means a patient receives the part that addresses the clinical problem rather than unnecessary volume. Red cells, platelets and plasma have different functions, so the same collection can support different types of treatment. NHS Blood Donation says one donation can help up to three patients; that is a stated potential, not a guarantee that every donation will produce three usable patient doses.
What each component is used for
| Component | What it provides | Examples of clinical need |
|---|---|---|
| Red blood cells | Cells that carry oxygen | Blood loss, surgery or anaemia when a patient needs more oxygen-carrying capacity |
| Platelets | Cell fragments that help form clots | Support for patients with low platelet counts or bleeding risk, including some people receiving cancer treatment or undergoing surgery |
| Plasma | The liquid portion containing proteins and clotting factors | Selected bleeding disorders and situations in which clotting-factor replacement is required |
Not every donation yields components that can all be issued. The final products depend on the collection method, the condition of the unit, test results and the blood service’s specifications.

3. Laboratories test the donation before it can be released
Testing has two linked purposes: establish information needed to select a compatible component, and look for infectious agents or other findings that make the unit unsuitable for transfusion. Donor screening at the appointment and laboratory testing after collection are overlapping safeguards. The exact panel is set by the jurisdiction and may change as regulations and epidemiology change.
Blood group and antibody information
Routine testing establishes the donation’s blood group. UK NHS material also describes antibody screening. Those results become part of the information used when a hospital requests blood for a patient.
Infectious-agent screening
The UK NHS lists testing for syphilis, hepatitis B, hepatitis C, hepatitis E, HIV and HTLV. Additional tests, such as malaria or West Nile virus testing, may be used when a donor’s travel history, location or other indication makes them appropriate. This should not be read as a universal list for every country.
The US Food and Drug Administration describes donor screening and testing as overlapping safeguards and requires all applicable infectious-agent tests to be negative before blood is suitable for transfusion. A reactive, positive or otherwise unresolved result prevents that unit from being released; the donor is handled through the notification and follow-up process used by the relevant blood service.

Why platelets receive an additional microbiology check
Platelets are stored at room temperature, which creates a different bacterial-growth risk from refrigerated red cells. NHS Blood and Transplant says platelet donations receive bacterial testing. That test is part of the service’s release controls for this component.
4. Processing and testing have a defined timetable
Operational schedules differ, but the UK NHS publishes a useful example of how quickly the early stages occur. NHS Blood and Transplant says donations are processed within 27 hours of collection. It also says microbiology and blood-group testing can take up to 72 hours to complete, and NHS Blood Donation says most components are ready for issue by day three.
These figures describe that service’s workflow, not a worldwide deadline. A component may remain unavailable until every required result and quality check is complete, even if the physical separation happened earlier.
5. Components are stored under different conditions and have different expiry dates
Storage is matched to the component’s biology. Refrigerated red cells, room-temperature platelets and frozen plasma do not share one shelf-life rule. The figures below are labeled examples from the named services, not global limits.

| Blood service and jurisdiction | Red cells | Platelets | Plasma | Cryoprecipitate |
|---|---|---|---|---|
| UK NHS examples | Up to 35 days | Up to 7 days | Up to 3 years | Not stated in the cited NHS material |
| US American Red Cross examples | Up to 42 days | Up to 5 days | Up to 1 year | Up to 1 year |
The number of days is a maximum for the specified product under that service’s rules. It does not mean every unit remains available for the full period, nor that a component can be kept longer by another blood service. Temperature monitoring, packaging, processing method and local regulation all affect release and expiry decisions.
Why short platelet shelf life affects supply
Platelets expire sooner than the other examples and must be held at room temperature. Their limited life and bacterial-testing requirements make scheduling and inventory rotation especially important. Hospitals and blood services therefore coordinate requests closely so that platelet units are used before their service-specific expiry.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.6. Hospitals request the component they need
Once a component has passed release requirements, it enters the blood service’s inventory. Hospitals order products according to their patients’ needs rather than receiving an entire donation automatically. A request might call for red cells for blood loss or anaemia, platelets for bleeding risk during cancer treatment or surgery, or plasma for a clotting-factor problem.
The blood service selects an available component that meets the request and sends it under the required transport conditions. Inventory systems preserve the donation identifier so the receiving hospital can maintain traceability.

Checks happen again at the hospital
Delivery is not the same as transfusion. Hospital staff carry out further checks before connecting a component to a patient. They verify the product and its identifiers, review the laboratory information required by local policy and confirm that the component is intended for that patient. The exact bedside and compatibility procedure varies by jurisdiction and institution, but a unit is not treated as ready for use merely because it has arrived.
7. What happens if a test result is positive or inconclusive?
A donation with a required reactive, positive or unresolved result is not released for transfusion. The component is managed according to the service’s safety procedures rather than being placed in ordinary hospital inventory. The donor may be contacted through the service-specific notification process, which can include information about the finding and advice on follow-up.
Policies differ, so a general explanation cannot promise the same notification wording, confirmatory tests or follow-up timetable everywhere. The consistent safety principle is that a unit does not enter the transfusion supply until all required release criteria are met.
8. Whole-blood donation is not the same as apheresis
The sequence above primarily describes a whole-blood donation. In an apheresis donation, a machine collects selected components—such as platelets or plasma—while returning other blood elements to the donor during the session. Because the collection method and starting product differ, processing steps, yields and schedules can differ as well.

It is therefore inaccurate to assume that every donation follows an identical separation pattern or produces the same combination of products.
9. Why the process matters to patients
Three features make the system useful beyond the individual donation. First, traceability connects a component to its test results and movement history. Second, separation allows one collection to address different clinical needs instead of giving every patient whole blood. Third, controlled storage and expiry rules preserve quality while blood services manage a perishable inventory.
NHS Blood and Transplant reports handling around 5,000 donations per day across three manufacturing sites. At that scale, barcode records, laboratory workflows, temperature controls and inventory rotation are essential to move donations safely from collection sessions to hospitals.
Quick Recap
10. The key limits to keep in mind
- “Up to three patients” is potential, not a promise. A donation may produce multiple components, but usable yield depends on processing and test results.
- Test panels are jurisdiction-specific. The UK list and US requirements illustrate safeguards; they are not one worldwide checklist.
- Storage durations are product- and service-specific. The UK and US examples differ, so a number from one service should not be applied to another.
- Release is not the same as transfusion. Hospitals perform additional checks after delivery.
- Artificial blood is not yet a replacement. NHS Blood and Transplant states: “The human body is so complex and ingenious that artificial blood cannot (yet) replace donated blood.”
