What Happened to Theranos? Blood-Testing Claims, Evidence, and Trial Verdicts Explained

Theranos collapsed after regulators and prosecutors challenged its promise that a few drops of fingerstick blood could deliver fast, inexpensive and accurate results across a wide range of tests. The U.S. Securities and Exchange Commission (SEC) alleged that the company and its leaders raised more than $700 million through false or exaggerated statements about the technology, business and finances. Federal trial evidence described analyzers with much narrower capabilities than the public pitch suggested.

The legal outcomes were not identical. Elizabeth Holmes was convicted of defrauding investors, acquitted on patient-related counts, and left without unanimous verdicts on three investor counts. Ramesh “Sunny” Balwani was convicted of conspiracy and wire fraud involving both investors and patients. Those distinctions matter: the public story is about one company, but the criminal verdicts addressed different conduct and different evidence.

What Theranos said it could do

The fingerstick promise

Theranos presented a system built around collecting a very small blood sample, placing it in a proprietary cartridge, and processing it with a company analyzer. Investor-facing materials described the approach as faster, cheaper and more accurate than conventional laboratory testing while offering a far broader menu of tests. The SEC’s 2018 complaint records those representations as allegations challenged by the regulator, not as independently validated capabilities: SEC complaint.

An account published by the U.S. Food and Drug Administration (FDA) says Theranos representatives claimed the device could run more than 1,000 tests for diseases and conditions including cancer, diabetes and HIV from a few drops of blood. That “more than 1,000” figure is a reported company claim. It is not an established count of tests the system could reliably perform: FDA account.

Why the pitch attracted attention

A small sample could have made blood testing less intimidating, particularly for people who dislike venous draws or need repeated monitoring. A single retail visit that produced many results also implied convenience and lower cost. Those benefits were part of the appeal to patients, doctors and investors, but the public claims were presented more broadly than the supporting validation evidence ultimately demonstrated.

How the proposed system was supposed to work

  1. Collection: blood would be obtained through a fingerstick rather than the larger draw commonly associated with conventional laboratory testing.
  2. Preparation: the sample would go into a special cartridge or container designed for Theranos’s platform.
  3. Analysis: a proprietary analyzer would process the sample and return results for numerous assays.

The SEC complaint describes this architecture in the context of what investors were told. The sources do not establish a complete technical specification, a validated performance rate, or an independently confirmed list of assays that worked on the platform. There is therefore no responsible accuracy percentage to quote.

Could Theranos really run hundreds of tests from a finger prick?

The defensible answer is narrower than the original marketing. Theranos claimed a broad menu, including more than 1,000 tests, but trial evidence summarized by the Department of Justice (DOJ) described analyzers with significant limitations. The DOJ’s account says the devices could not compete with conventional laboratories in the way Holmes and Balwani represented them: DOJ Holmes sentencing account.

That does not prove that every test result produced by Theranos was wrong. It establishes a mismatch between the sweeping public representations and the capabilities described in court. The available sources also do not provide a single, reliable accuracy statistic covering the platform as a whole.

Promised method versus established record

Question Theranos representation What the cited record establishes
Sample collection A few drops from a fingerstick The SEC and FDA describe this as the company’s proposed or claimed method; the conventional-test volume difference is not stated in the cited sources.
Test breadth More than 1,000 tests, including serious diseases and conditions The FDA reports that representatives made this claim; it is not a validated count of reliably available tests.
Analyzer A proprietary cartridge-and-analyzer system The SEC complaint describes the investor-facing system; DOJ trial summaries describe significant limitations in analyzer capability.
Speed and price Faster and cheaper than conventional laboratories These were representations challenged by the SEC; the cited material does not supply a verified, apples-to-apples price or turnaround comparison.
Accuracy More accurate than conventional testing No suitable, source-backed accuracy percentage is established in the cited record.
Validation A system ready for broad clinical use The SEC complaint and DOJ trial summaries describe regulatory allegations and trial evidence, not a complete independent validation record.

What regulators alleged about the business

In March 2018, the SEC announced civil fraud charges against Theranos, Holmes and Balwani. The agency alleged that they raised more than $700 million from investors through false or exaggerated statements about the technology, the company’s business relationships and its financial performance. “More than $700 million” is the SEC’s allegation in that civil action, not a finding that every dollar was obtained through a proven false statement: SEC enforcement release.

The SEC’s complaint is useful for separating the components of the pitch. Investors were allegedly led to believe that fingerstick samples could support comprehensive testing, that the results were faster, cheaper and more accurate than conventional laboratory work, and that Theranos had business and technical achievements it had not actually attained. Civil allegations and criminal convictions overlap in subject matter, but they are not interchangeable legal findings.

How the criminal cases developed

Separate trials, separate evidence

Holmes and Balwani were prosecuted in the same overall case but tried separately. The DOJ case page records the federal proceedings and the different count outcomes: DOJ case page. Keeping the trials separate prevents a common error: treating Balwani’s patient-related convictions as though the same verdict applied to Holmes.

Elizabeth Holmes’s verdict

The Holmes jury found her guilty of conspiracy to commit wire fraud against investors and of three wire-fraud counts involving investors. It found her not guilty on the patient-related counts and on one additional investor count. The jury could not reach unanimity on three other investor counts, so those counts did not produce convictions. The DOJ’s sentencing account describes the verdict and the consequences of the investor fraud conviction: DOJ Holmes sentencing account.

Accordingly, saying that Holmes was convicted of “defrauding patients” is inaccurate. Her convictions concerned investors; the jury’s acquittals on patient counts are a material part of the result, not a footnote.

Sunny Balwani’s verdict

Balwani’s jury found him guilty of conspiracy and wire fraud involving investors and patients. The DOJ’s verdict announcement describes the findings: DOJ Balwani verdict account. His convictions therefore covered patient-related conduct that was not established against Holmes in her trial. A DOJ sentencing release reported that Balwani received a sentence of more than 12 years: DOJ sentencing account.

What happened to patients?

The FDA’s retrospective emphasizes patients and doctors who relied on Theranos’s celebrated testing claims. The concern was not merely that a startup had overstated a business plan; laboratory results can influence diagnoses, medication decisions and whether a patient seeks further care.

The DOJ sentencing account describes trial evidence involving one person whose Theranos results were markedly different from earlier conventional-test results. That example illustrates why an unexplained discrepancy could be serious, but it does not justify saying that every Theranos result was inaccurate or that every patient was harmed. The cited material does not establish a universal error rate or a complete catalog of clinical injuries.

FDA Assistant Commissioner for Criminal Investigations Catherine A. Hermsen characterized the government’s concern this way: “When individuals jeopardize patient health and put profits above the public health, the FDA will continue to investigate and bring them to justice.” That is an agency official’s statement about the case, not a neutral finding that all patient outcomes were negative. The quotation appears in the DOJ sentencing release: DOJ sentencing account.

Why conventional laboratory comparison matters

Theranos’s story is often reduced to the image of a tiny blood sample, but the central issue was whether the entire analytical chain could produce dependable results at clinical scale. A fair comparison asks five questions:

  • How was the sample collected, and was the amount sufficient for each assay?
  • How many tests could the platform actually run, rather than advertise?
  • Were results generated on the proprietary analyzer, on third-party instruments, or through a mixture of methods?
  • How did results compare with established laboratory methods under controlled conditions?
  • What independent validation, quality controls and regulatory documentation supported clinical use?

The available legal and agency summaries answer those questions unevenly. They document the sweeping representations and describe evidence of limited analyzer capability, but they do not provide a complete assay-by-assay validation table or a single platform-wide accuracy number. That limitation should be stated rather than filled with speculation.

What happened after the verdicts?

A 2025 Associated Press report said the U.S. Court of Appeals for the Ninth Circuit upheld Holmes’s conviction, Balwani’s conviction and a restitution order: Associated Press report. Appeals, custody administration and restitution enforcement can change, so any newer procedural development should be checked against the latest court record before publication.

A 2026 decision by the U.S. Department of Health and Human Services’ Departmental Appeals Board summarizes the Holmes verdict and evidence in an administrative context: HHS decision. It is a later administrative source, not a replacement for the trial record or the original regulatory inspection documents.

The cited material does not supply a complete chronology of every CMS inspection finding or sanction notice. It is therefore more accurate to describe the documented criminal and appellate outcomes than to invent a detailed regulatory timeline.

What the Theranos case changed about evaluating health-tech claims

Marketing is not validation

A dramatic demonstration or a large test-menu claim does not establish analytical validity. Readers, clinicians and investors need evidence showing how a device performs for each assay, how specimens are handled, how failed runs are detected and how results compare with accepted methods.

Scale magnifies small errors

A discrepancy that seems minor in a product demonstration can become consequential when thousands of people receive results that affect treatment decisions. The patient account in the DOJ summary explains why validation and quality control must be evaluated in clinical context, not only by engineering spectacle.

Legal responsibility depends on the count and the proof

The split Holmes verdict demonstrates that a scandal narrative cannot substitute for count-by-count analysis. Holmes’s investor convictions, patient acquittals and unresolved counts must remain distinct from Balwani’s convictions involving both investors and patients.

Numbers need labels

“More than $700 million” is an SEC allegation about money raised through the alleged fraud. “More than 1,000 tests” is an FDA-reported company claim. Neither number should be presented as an independently verified measure of technical performance. Clear labels preserve what the evidence actually says.

The bottom line on Theranos

Theranos did not disappear because the idea of smaller blood samples was inherently impossible. It failed because its public claims about breadth, speed, cost and accuracy outran the capabilities and validation described in the evidence. Regulators alleged investor deception; prosecutors secured different convictions against Holmes and Balwani; and patient testimony and trial evidence showed why unsupported laboratory claims can carry real consequences. The most accurate account keeps those claims, allegations, evidence and verdicts separate.
