It can also cause a heart attack, stroke, seizure, dangerous heart rhythm, coma or sudden death during an episode of use—even when use has not been prolonged. Repeated exposure can alter the brain’s reward and stress systems, leading to tolerance, withdrawal and cocaine use disorder. Longer-term harm also depends on whether cocaine is snorted, smoked or injected, and pregnancy creates additional risks for the pregnant person and fetus.
There is no established safe dose and no reliable way to predict an individual outcome from amount alone. Route, frequency, other drugs or alcohol, underlying health conditions and the contents of an illicit supply all affect risk. This is general health information, not a personal diagnosis.
What cocaine does in the short term
Cocaine is a powerful stimulant. Its immediate effects can feel rewarding, but the same biological changes can become dangerous quickly.
Psychological and behavioral effects
- Euphoria, increased energy and unusual alertness
- Talkativeness and heightened sensitivity to sights, sounds or touch
- Restlessness, irritability and impulsive or erratic behavior
- Anxiety, panic, suspiciousness and paranoia
- Tremors, agitation or, at higher levels of toxicity, hallucinations and loss of contact with reality
A person may move from stimulation to fear, confusion or agitation as the effects wear off or as more cocaine is taken. Binge use is especially associated with escalating irritability, panic, paranoia and psychosis.

Physical effects
Cocaine constricts blood vessels and commonly raises heart rate, blood pressure and body temperature. Pupils may become dilated. Nausea, abdominal pain and reduced appetite can occur. These changes increase strain on the heart and brain, particularly in someone with cardiovascular disease, high blood pressure or another medical condition.
How route changes the timing
The faster cocaine reaches the brain, the more intense and shorter-lived the high generally is, according to the National Institute on Drug Abuse (NIDA). Its report gives the following illustrative durations:
| Route | Typical reported high | What the figure means |
|---|---|---|
| Smoking | About 5–10 minutes | An illustrative duration reported by NIDA, not a safety limit or prediction for an individual. |
| Snorting | About 15–30 minutes | An illustrative duration reported by NIDA; effects vary with amount, product and the person. |
A brief high does not mean the cardiovascular or neurological danger has ended. Re-dosing to prolong a short effect can increase total exposure and may produce rapidly changing blood pressure, temperature and mental state.
Why a single episode can become an emergency
Acute cocaine toxicity can produce a dangerous heart rhythm, heart attack, stroke, seizure, coma or sudden death. These events can occur unexpectedly; previous uneventful use does not establish that a later episode is safe.

Warning signs that need emergency care
- Chest pain, pressure or severe shortness of breath
- Collapse, fainting, unresponsiveness or a seizure
- Sudden weakness, numbness, trouble speaking, severe headache or other possible stroke signs
- Very fast or irregular heartbeat
- Severe confusion, extreme agitation or loss of awareness
- Very high body temperature
- Breathing difficulty, blue or gray lips, or signs of shock
Call local emergency services immediately for these symptoms. Do not assume they are ordinary intoxication, wait for the high to pass or leave the person alone. Tell clinicians what was taken, how it was used and whether alcohol or another drug may be involved; accurate information helps them treat complications.
Long-term effects of repeated cocaine use
Longer-term effects are not simply a permanent version of the initial high. Repeated exposure can produce changes in brain systems that regulate reward, stress, motivation and impulse control.
Brain adaptation, tolerance and sensitization
NIDA describes a less responsive reward system and a more sensitive stress system after repeated use. Natural rewards may feel less satisfying, while not using can bring negative mood, irritability or anxiety. Tolerance may develop, so a person takes more or uses more often to pursue the same effect.
At the same time, sensitization can make some toxic reactions occur with less cocaine. Anxiety, convulsions or other severe effects may become easier to trigger. Therefore, an experienced user is not protected from overdose simply because a previous amount seemed manageable.

Binges, psychosis and daily functioning
During a binge, sleep deprivation and repeated dosing can intensify restlessness, panic, suspiciousness and psychosis. Hallucinations or a loss of contact with reality can endanger the person and others. Continued use may interfere with work, school, relationships, finances and basic health care.
Withdrawal and cocaine use disorder
When dependence has developed, stopping can bring depression, fatigue, restlessness, increased appetite, bad dreams, insomnia and slowed thinking. Craving can be strong even after the stimulant has left the body. MedlinePlus describes cocaine use disorder as a chronic brain disorder in which use continues despite substantial health or daily-life problems; it is not a moral failing.
Withdrawal is often emotionally difficult. Severe depression, suicidal thoughts or inability to stay safe require urgent professional help. A clinician can assess whether symptoms reflect withdrawal alone or another medical or psychiatric condition.
Route-specific long-term damage
The route of administration adds its own risks beyond cocaine’s effects on the heart and brain.

| Route | Potential longer-term harms |
|---|---|
| Snorting | Nasal tissue damage, recurrent nosebleeds, reduced sense of smell and difficulty swallowing. |
| Smoking | Cough, asthma, breathing problems and increased risk of pneumonia. |
| Injecting | Collapsed veins, scarring, skin or soft-tissue infections and increased risk of HIV and hepatitis C when equipment or supplies are contaminated or shared. |
Repeated use can also contribute to poor nutrition and movement disorders. The absence of a route-specific symptom does not rule out systemic injury: heart, blood-vessel and brain complications can occur with any route.
Recognizing and responding to a cocaine overdose
MedlinePlus describes overdose or severe intoxication as potentially involving stroke, seizure, arrhythmia, heart attack, confusion, loss of awareness, very high temperature, an abnormal or very fast heart rate, breathing difficulty or death.
- Call emergency services now if the person has chest pain, a seizure, collapses, cannot be awakened, has severe confusion, is breathing abnormally or feels dangerously hot.
- Stay with the person. Keep the area as calm and safe as possible, and remove immediate hazards.
- Check breathing and responsiveness. Follow the emergency dispatcher’s instructions. If the person is not breathing normally and you are trained, begin resuscitation.
- Do not give more drugs, alcohol, food or drink. Do not try to force vomiting or make the person “sleep it off.”
- Share relevant details with clinicians. Include the route, approximate timing, amount if known, other substances and existing medical conditions.
Emergency treatment focuses on monitoring vital signs and managing the affected organs and symptoms. MedlinePlus does not identify a specific medicine that reverses cocaine overdose. Prompt medical care is therefore essential, even if symptoms appear to improve.

Combining cocaine with alcohol or another drug can increase overdose risk. Mixtures can make the effects harder to recognize and can place additional stress on the heart, breathing and brain. Do not use a prior personal “safe” amount to judge a combination.

No source reviewed provides a population percentage that can predict an individual’s chance of a heart attack, stroke or overdose.
Pregnancy-related risks
Cocaine crosses serious medical boundaries during pregnancy. A Substance Abuse and Mental Health Services Administration treatment reference describes toxicity affecting multiple organ systems, including high blood pressure, heart attack, kidney failure, reduced blood flow to the heart and brain, stroke, seizure and death. It also describes a well-documented association with placental abruption and preterm birth.
A pregnant person who has used cocaine should contact an obstetric or other health professional promptly for individualized care. Emergency symptoms such as abdominal pain, vaginal bleeding, chest pain, severe headache, seizure or reduced consciousness require emergency services.
Treatment and recovery
Professional help is available for cocaine use disorder. MedlinePlus describes behavioral therapies as established treatments and states that no medicines treat the disorder itself. A health professional can assess withdrawal, depression, sleep, nutrition, other substance use and medical complications, then recommend an appropriate level of care.

What an assessment may address
- How often and by which route cocaine is used
- Craving, loss of control and consequences at home, work or school
- Chest, breathing, neurological, nasal, skin or infectious symptoms
- Alcohol, prescription medicines and other drugs
- Pregnancy, mental-health symptoms and suicide risk
Stopping can be safer with support, especially after heavy or frequent use, severe depression, psychosis, heart symptoms or a history of seizures. For non-emergency help, contact a healthcare provider or a local substance-use treatment service. The guidance summarized here comes from U.S. sources, so people elsewhere should use their own local emergency and treatment systems.
How to interpret the risks
Short-term intoxication, acute toxicity, cumulative route-related injury, dependence and withdrawal are different processes. A person can experience one without all the others, and the absence of an obvious long-term symptom does not show that the heart or brain is unharmed. Conversely, possible effects are not inevitable for every person; the evidence supports serious risk, not a guaranteed outcome for each user.
The practical rule is to treat chest pain, seizure, collapse, severe confusion, breathing trouble or extreme overheating as a medical emergency. For ongoing use, cravings, withdrawal or health problems, early professional support can reduce the chance that a treatable condition becomes a crisis.
