Spoilers for the Tokyo Ghoul and Tokyo Ghoul:re manga. This is an editorial ranking, not an official or consensus list. It weighs personal loss, helplessness, the future a character loses, and the death’s effect on the story. The scope is the manga, not anime-only events.
Sui Ishida’s series begins with Ken Kaneki caught between humans and ghouls, and tragedy runs through both the original manga and its sequel, Tokyo Ghoul:re. These deaths hurt for different reasons: some devastate a family or a close-knit group, while others expose the violence sustaining the larger conflict.
Which Tokyo Ghoul death was the saddest?
There is no canon answer. The ranking below puts the greatest weight on intimate relationships and the sense of a life cut short, while also considering how brutally or unexpectedly the death occurs and what it changes in the story. The series sometimes leaves a character’s fate uncertain or later complicates it; the list includes only deaths treated as deaths in the manga.

1. Koutarou Amon’s apparent death and the loss of certainty
Amon’s apparent death at Kaneki’s hands is one of the original manga’s most painful turning points because it brings two people who recognize each other’s humanity into direct conflict. Kaneki is trying to protect those he loves; Amon has devoted himself to fighting ghouls. Their encounter makes the cost of that conflict personal rather than abstract. The story later reveals that Amon survived, so this is not a final death. It belongs among the series’ most tragic apparent deaths precisely because both characters—and readers—have reason to believe the other has been lost.

2. Kureo Mado
Mado’s death in his confrontation with Touka and Hinami is tragic in part because it leaves no clean division between grief and justice. He is an intimidating investigator who has inflicted suffering on ghouls, but he is also a husband and father. Hinami’s role in his death ties the encounter to the earlier murder of her mother, Ryoko. The loss deepens the Fueguchi family’s tragedy and shows how a revenge-driven cycle can bind victims and perpetrators together without repairing what either side has lost.

3. Ryoko Fueguchi
Ryoko is killed by Mado while protecting Hinami. Her death is especially painful because it is an act of maternal sacrifice: she tries to give her daughter a chance to escape, and Hinami is left to survive without her. The violence is not a distant consequence of the human-ghoul conflict; it tears apart a small family in front of the people who might have helped them. Ryoko’s loss also helps shape Hinami’s grief and the choices she makes afterward.

4. Seidou Takizawa’s transformation and loss of his former life
Takizawa’s fate is not a straightforward death: he survives, but torture and experimentation remake him into an artificial ghoul and rupture his former identity. His friends and colleagues had known him as an anxious, often overlooked investigator; afterward, he becomes frighteningly violent and estranged from that life. Ranking him here is a deliberate exception to a list of literal deaths. His story represents the destruction of a person’s former self, not a confirmed final death, and the manga continues to follow him after his transformation.

5. Yukinori Shinohara’s apparent death
Shinohara is left in a vegetative state after his confrontation with the Owl, and the CCG and those close to him have to live with the possibility that he will never return to them. He had been a mentor and steady presence for younger investigators, especially Juuzou Suzuya. The apparent loss changes Juuzou, whose devotion to Shinohara makes the silence and uncertainty especially painful. Because Shinohara does not die in the manga, this is an apparent death and lasting absence, not a confirmed death.

6. Ginshi Shirazu
Shirazu dies during the Quinx Squad’s battle with Nutcracker. He is still young, and his urgent wish to help his family gives his death a painfully ordinary human dimension: he wants a future in which he can provide for the people he loves. His loss hits his squadmates hard, particularly Urie, whose response exposes how much their rough partnership had come to matter. Shirazu’s death also makes the costs of the CCG’s experiments and the Quinx program harder for the surviving members to ignore.

7. Karren von Rosewald (Kanae)
Karren’s death is tragic because the loyalty she gives to the Rosewald family is bound up with a desperate search for belonging. She acts as Kanae for Shuu Tsukiyama, whose protection and affection become the center of her life. In the end, her devotion helps save him, but it cannot restore the family history and identity she has lost. Her sacrifice changes Tsukiyama: the person he had treated as a subordinate becomes someone whose love and choice he must finally recognize.

8. Arima Kishou
Arima’s death in his final confrontation with Kaneki is both a personal loss and the end of a role he has carried for years. To the CCG, he is the seemingly unbeatable investigator; to Kaneki, he becomes a guide who helps him understand the system that shaped them both. Arima’s disclosure of the truth behind his position and his choice to entrust the future to Kaneki turn their battle into a painful act of handover. His death removes one of the most formidable figures in the story while making clear how much of his life was constrained by the organization he served.

9. Eto Yoshimura
Eto’s story ends with her death after her conflict with Furuta. She is both a feared ghoul and the daughter of Yoshimura, whose abandoned family history is part of the damage she carries. Her rebellion against the structures controlling ghouls gives her a wider purpose, but it does not undo the isolation and violence that formed her. Her end lands as the collapse of a dangerous, complicated figure who wanted to challenge the order of the world and could not escape the cruelty built into it.

10. Kuzen Yoshimura’s apparent death
Yoshimura is captured after fighting the CCG’s Owl operation, and his fate is treated as a devastating loss by the people who knew him at Anteiku. He had offered Kaneki and other ghouls a rare refuge, while his past as a feared fighter and his separation from Eto left him with profound regrets. The story later reveals that he is kept alive as a body used in experimentation, rather than having died at the time of his capture. This is therefore an apparent death, and the continuing horror of his condition is part of why his fate remains so tragic.
Why some of the most tragic fates are not final deaths
Tokyo Ghoul repeatedly uses apparent death, survival, and bodily transformation to unsettle the boundary between being alive and being lost. Amon survives his apparent death; Shinohara remains alive but unresponsive; Yoshimura is captured and exploited rather than killed; and Takizawa survives a transformation that destroys his former life. Those cases should not be labeled final deaths. Keeping the distinction clear makes the confirmed deaths—and the different kinds of suffering around them—more meaningful.
Reading the manga after these spoilers
The story spans Tokyo Ghoul and its sequel, Tokyo Ghoul:re, which concludes in Volume 16. VIZ’s official catalogs list the original Tokyo Ghoul manga and the Tokyo Ghoul:re volumes, including the sequel’s final volume. These are useful starting points for revisiting the context around the deaths and apparent deaths discussed here.
