David Lochridge, OceanGate’s former director of marine operations, raised concerns about Titan’s safety before its 2023 implosion, according to a Vanity Fair feature. The word “fraud” in the headline is a colleague’s characterization, not a finding by the Coast Guard or a court. The Coast Guard’s 2025 investigation later found serious failures and said Stockton Rush’s negligence contributed to four deaths, but it did not determine that Rush committed a crime.
Who was the colleague, and what did he say?
The colleague is David Lochridge, OceanGate’s former director of marine operations. Vanity Fair’s account reports that Lochridge raised concerns about Titan’s design and condition and submitted a 10-page quality-control inspection report in 2018. The word “fraud” should be read as an attributed accusation, not as an established legal conclusion. The account describes Lochridge’s concerns; it does not itself establish that Rush committed fraud.
What did the Coast Guard find about Titan?
On August 5, 2025, the U.S. Coast Guard Marine Board of Investigation released its report on the Titan casualty. The Coast Guard said the Board identified inadequate design, certification, maintenance, and inspection processes as primary contributing factors. It also cited a toxic workplace culture, gaps in domestic and international rules for submersible operations and novel designs, and an ineffective whistleblower process. The Board’s account also describes failures to respond to warning signs after the 2022 Titanic expedition. Titan’s real-time monitoring produced data that should have been analyzed and acted on, but OceanGate did not act on it, perform preventative maintenance, or properly store the submersible during the extended off-season before the 2023 expedition, according to the Coast Guard’s summary.
Board chair Jason Neubauer said, “This marine casualty and the loss of five lives was preventable.” The report includes 17 safety recommendations and is described by the Coast Guard as more than 300 pages long; the announcement does not give an exact page count.

Did the report find Rush guilty of fraud or a crime?
No. The Board said Rush, who was both OceanGate’s CEO and Titan’s acting master or pilot, exhibited negligence that contributed to the deaths of the other four people aboard. It said that, had Rush survived, the Board would have recommended that the Department of Justice consider a separate criminal investigation. The report leaves the decision about whether a crime occurred to DOJ and says that determination will not be made in this instance because Rush died. The Marine Board’s report is the source for this distinction.
Accordingly, it would be inaccurate to say the Coast Guard found Rush guilty of fraud or proved that he committed it. The official findings concern negligence and OceanGate’s practices; the report did not adjudicate fraud.

How to separate the allegation from the official findings
- Colleague’s account: Lochridge’s safety concerns and 2018 report are described in Vanity Fair’s reporting.
- Investigative findings: The Coast Guard Board documented failures involving Titan’s design, oversight, maintenance, inspection, monitoring data, and OceanGate’s workplace and whistleblower processes.
