Claire survives a devastating infection but loses an arm; Glassman declines further cancer treatment, and Shaun learns to respect that choice. Years later, Shaun is chief of surgery and honors Glassman’s legacy.
What happens in “Goodbye”?
The series ends with two crises that Shaun cannot solve in the same way. Claire Browne is critically ill with an aggressive infection, while Dr. Aaron Glassman’s cancer has returned. Glassman has only months to live and refuses more treatment because he does not want to spend his remaining time sick and in pain. The official episode listing identifies “Goodbye” as Season 7, Episode 10, and describes the doctors weighing their futures while working on an important case.
Shaun initially approaches both situations as medical problems with solutions. Claire’s condition demands an extreme intervention: she survives, but loses an arm. Glassman ultimately performs the procedure himself, protecting Shaun’s career. The crises force Shaun to confront the limits of his clinical skill and the difference between saving someone and deciding what is best for them.
What happens to Glassman?
Glassman’s cancer is terminal, and he does not want further treatment. Shaun struggles to accept that refusal, but eventually understands that continuing to push for a cure would disregard Glassman’s wishes. He promises to be present for him and to share the time they have left. Glassman dies before the finale’s later-years epilogue.

The choice is central to Shaun’s emotional arc. Showrunner Liz Friedman described the more interesting challenge for Shaun as dealing with “feelings and emotions and empathy,” given his considerable medical, diagnostic, and surgical abilities. The finale makes that challenge concrete: Shaun has to respond to Glassman as a person making his own decision, not as a case to solve.
Does Claire survive?
Yes. Claire survives the life-threatening infection, though the intervention costs her an arm. Shaun is willing to risk his career in trying to save her; Glassman steps in to perform the procedure. The finale thus places Shaun’s professional judgment alongside his love and friendship for Claire, rather than treating the medical stakes as separate from the people involved.

Creator David Shore connected the ending to the pilot by revisiting its boardroom. He said Shaun had started his journey there and returned to consider whether he would end it to save Claire, and whether the price was worth it. The boardroom reprise makes the decision feel like a culmination of the series, not simply another hospital emergency.
Where does Shaun end up?
Several years after Glassman’s death, Shaun is chief of surgery at San Jose St. Bonaventure Hospital. He gives a TED Talk in Glassman’s honor, reflecting on how Glassman shaped his life. Shaun and Claire have also established the Dr. Aaron Glassman Foundation for Neurodiversity in Medicine.

Shaun and Lea have a daughter as well as their son, Steve. The finale’s time jump shows Shaun moving forward as a parent and leader while carrying Glassman’s influence into his work. Freddie Highmore said Shaun’s growth reflects that the idea autistic people cannot change or learn is a misconception; his comment concerns Shaun’s character arc, not a general clinical claim about autistic people.

Why did the show end after seven seasons?
ABC announced on January 11, 2024, that Season 7 would be the final season. The finale aired May 21, 2024. The announcement confirms the series was concluding, but does not establish a ratings or financial reason, so it would be inaccurate to assign one.
According to TheWrap’s interview with the creative team, an earlier endpoint had been Shaun becoming a father, which happened in Season 6. Continuing for a seventh season let the show explore him as a father before bringing his story to a close.
