There is no reliably proven, low-risk way for an adult with typical anatomy to make the penis permanently larger. Among nonsurgical choices, penile traction has the clearest (but still low-certainty) signal for modest length gains after months of daily use. Vacuum erection devices have not produced reliable lasting lengthening, and pills, creams, supplements, and adult hormone therapy have not been established as enlargement treatments. Surgery and injections can change appearance or girth in selected cases, but evidence is limited and complications can be serious. The right first step is standardized measurement and a consultation with a qualified urologist or sexual-medicine clinician—not buying a product or booking an irreversible procedure.
Start with the question: length, girth, or a medical condition?
“Penis enlargement” describes several different goals. A person may want more visible flaccid length, greater stretched or erect length, increased circumference (girth), or treatment for a penis partly hidden by surrounding tissue. These are not interchangeable outcomes, and a procedure designed to reconstruct a buried penis is not the same as cosmetic augmentation in an adult whose anatomy is typical.
Measure before deciding
The European Association of Urology (EAU) recommends measuring stretched length from the skin junction, or dorsally from the pubic bone to the tip of the glans. Flaccid, erect, and girth measurements can add context. Use the same position, ruler and technique each time; pressing through the pubic fat pad versus measuring only the visible shaft can produce different numbers. A clinician can document the measurement and check for conditions such as a buried penis, curvature, scarring or erectile dysfunction.
Measurements should be interpreted with sexual function, expectations and the reason for concern. If the penis is normal-sized but the worry is persistent, intrusive or out of proportion to what others observe, ask for validated screening for body dysmorphic disorder (BDD) and, when indicated, a mental-health referral. The EAU specifically recommends this pathway rather than escalating to cosmetic treatment.

How the options compare
| Option | Intended change | Invasiveness and burden | What the evidence shows | Durability and major concerns |
|---|---|---|---|---|
| Penile traction device | Length, mainly flaccid or stretched; some studies measured erect length | External device worn for prolonged periods, commonly at least four hours daily | Small prospective studies reported gains after six months; evidence is poor, heterogeneous and lacks randomized trials | Durability after stopping is not established; discomfort and skin problems are possible, and girth gain has not been demonstrated |
| Vacuum erection device | Erection support; proposed lengthening | External pump used on a schedule | In an EAU-summarized study of 27 men, three 20-minute sessions weekly for six months did not significantly increase flaccid or stretched length | Useful in some erectile-dysfunction or rehabilitation settings, but not proven cosmetic enlargement |
| Cosmetic surgery (for example, suspensory-ligament release or suprapubic procedures) | Exposed or measured length | Invasive operation and recovery | Selected procedures may change visible length, but evidence and safety are uncertain; the AUA says adult suspensory-ligament division has not been shown safe or efficacious for lengthening | Potential scarring, altered angle or function and other complications; requires extensive counseling |
| Soft-tissue fillers (HA, poly-L-lactic acid, PMMA) | Girth | Injection procedure; repeat treatment may be needed | Reported studies show girth increases, but evidence is low-level and long-term data are limited | Temporary pain and swelling are reported; outcomes depend on material, technique and follow-up |
| Subcutaneous fat injection | Girth | Invasive harvesting and injection | The AUA says it has not been shown safe or efficacious | Fat survival and complications are unpredictable; do not treat it as an established option |
| Silicone, paraffin or petroleum jelly injections | Girth | Injection | Not a safe alternative; the EAU advises against these materials | Can cause chronic inflammation, swelling, infection, Fournier’s gangrene and reconstructive surgery |
| Pills, creams and supplements | Usually marketed for length or erections | Noninvasive to take or apply | No reliable enlargement efficacy is established in the reviewed clinical guidance | Ingredients, interactions and contamination vary; marketing claims are not proof |
| Adult testosterone or other hormones | Not an enlargement treatment after puberty | Medication with medical risks | The EAU recommends against hormonal therapy to increase penile size after puberty | Hormones have separate indications in selected children with micropenis or disorders of sex development |
These rows summarize different studies and indications; they are not a head-to-head ranking. A reported change in one small study cannot be used to promise the same result from another device, material, surgeon or patient.
Penile traction: the conservative option with the clearest length signal
A traction device applies a controlled, sustained stretch to the penis. It is external and does not add girth. The EAU gives a weak recommendation to consider traction as a conservative lengthening option because the available studies are small, heterogeneous and generally lack randomized controls.
What the small studies reported
- In a prospective study of 21 participants summarized by the EAU, using traction for at least four hours per day for six months was associated with a reported 2.3 cm increase in flaccid length and 1.7 cm increase in stretched length. No significant girth change was found.
- A separate prospective study summarized by the EAU reported mean six-month gains of 1.7 cm flaccid, 1.3 cm stretched and 1.2 cm erect length.
These are study-specific averages, not pooled estimates or guarantees. The guideline does not establish that every commercially sold device produces these results, that gains persist indefinitely after discontinuation, or that the same schedule is suitable for every person.

How to approach traction safely
- Get a baseline examination. A urologist can confirm the measurement, identify curvature or skin disease, and discuss whether lengthening is medically appropriate.
- Clarify the endpoint. Record whether the goal is visible flaccid length, stretched length or erect length; do not substitute a girth goal for a length study.
- Use the device exactly as instructed. The studies involved prolonged, repeated use over six months. Increasing tension or wearing it longer to chase a faster result has not been shown to work better.
- Monitor tissue. Stop and seek clinical advice for persistent pain, numbness, bruising, skin breakdown, color change or a new deformity. A device should not interfere with urination or normal circulation.
- Reassess rather than assume success. Have measurements repeated with the original technique. If the burden is high or there is no meaningful change, continuing indefinitely is not evidence-based.
Vacuum erection devices: useful for erections, not proven enlargement
A vacuum erection device draws blood into the penis to help create an erection. That medical use—often for erectile dysfunction or rehabilitation—should not be confused with permanent cosmetic growth. In the EAU’s summary, 27 men whose stretched length was below 10 cm used a vacuum device three times weekly for 20 minutes over six months. The study found no significant increase in flaccid or stretched length.

Surgery: why cosmetic procedures differ from reconstruction
Cosmetic length procedures
Procedures discussed in the EAU guideline include suspensory-ligament release, ventral phalloplasty and suprapubic lipoplasty, liposuction or lipectomy. Depending on anatomy, they may alter the amount of shaft that is visible or the way length is measured. They do not create a simple, predictable increase in erectile tissue. Complications can include scarring, infection, altered erection angle, instability, dissatisfaction and the need for revision; the guideline calls for extensive risk discussion.

The American Urological Association states that suspensory-ligament division in adults has not been shown to be safe or efficacious for increasing length. That statement is a reason to treat marketing promises cautiously and to seek an independent urologist rather than relying on a clinic that sells the operation.
Reconstructive indications
Surgery can be appropriate when the clinical problem is a buried penis, congenital micropenis, traumatic tissue loss or another defined abnormality. The purpose is function, hygiene, urination, sexual function or restoration of anatomy—not routine enhancement of a normal-sized penis. A reconstructive assessment may involve weight and skin management, scar treatment, urethral evaluation or staged reconstruction.
Penile prosthesis implantation treats erectile dysfunction; it is not a lengthening procedure. The EAU advises against recommending prosthesis implantation, penile disassembly or sliding techniques to people seeking length increase alone.

Injections and other girth procedures
Fillers
Studies of hyaluronic acid (HA), poly-L-lactic acid and polymethylmethacrylate (PMMA) have reported increased circumference. The EAU classifies the evidence as low level and notes that long-term data remain limited. Reported short-term effects with soft-tissue fillers include pain and swelling. The material, injection plane, sterility, operator training and ability to manage a complication all matter; results from one selected cohort cannot be generalized to every filler or provider.
Ask how the product is identified, whether it is approved for this use in your jurisdiction, what happens if nodules or infection develop, and how follow-up is provided. “Temporary” does not mean risk-free, and a product that can be dissolved or revised still may injure tissue.
Materials the EAU says to avoid
Do not accept silicone, paraffin or petroleum jelly injections as cheap or permanent solutions. The EAU advises against them because they can trigger chronic inflammatory reactions, severe swelling, infection, Fournier’s gangrene and reconstructive surgery. Treatment of a complication may require removal of damaged tissue and multiple operations.

Fat injection and graft techniques
The AUA and Urology Care Foundation describe subcutaneous fat injection as a procedure that has not been shown safe or efficacious for increasing girth. Fat can be reabsorbed or form irregularities, and the reviewed guidance does not establish a reliable long-term cosmetic result. The EAU considers graft procedures and some other girth techniques experimental and advises against using them as routine enhancement.
Pills, creams, supplements and hormones
Products sold as enlargement pills, topical creams or supplements often combine a promised size change with claims about testosterone, blood flow or sexual performance. The reviewed clinical material does not establish any of these categories as effective enlargement treatments. Without product-specific, reliable evidence, it is not possible to claim that a particular ingredient is effective or safe; supplements can also interact with medicines or be contaminated.
Adult testosterone is not a way to enlarge a normally developed penis. The EAU’s explicit recommendation is: “Do not use testosterone therapy or other hormonal therapies to increase penile size in males after puberty.” Hormonal treatment has a separate role in selected children with micropenis or disorders of sex development under specialist care.
A practical decision process
- Define the outcome. Choose length, girth, erection quality, exposure of a buried penis or reconstruction of lost tissue. One intervention will not reliably solve all of these.
- Confirm anatomy and health. Bring standardized measurements and a medication list to a board-certified urologist or sexual-medicine clinician. Address erectile dysfunction, curvature, pain, obesity-related concealment and skin conditions first.
- Screen expectations. If distress persists despite normal measurements, request BDD screening and counseling before an irreversible intervention.
- Prefer the least invasive evidence-supported path. For a cosmetic length goal, traction is the only conservative option with a guideline-discussed signal, and its expected gain is modest and uncertain. For girth, no option has strong long-term evidence.
- Demand a complication plan. Before any injection or operation, ask who treats infection, asymmetry, nodules, scarring, loss of sensation or erectile problems, and what the total cost of revision care would be.
- Obtain independent opinions. A clinician who does not sell the device or procedure can help distinguish a medical indication from a commercial promise.
Questions to ask at a consultation
- Which measurement method are you using, and what is my measured stretched length?
- Is my concern cosmetic, or is there a buried penis, micropenis, curvature, erectile disorder or tissue loss to treat?
- What change is documented for this exact technique, in patients like me, and over what follow-up period?
- Is the evidence prospective, randomized or only a small case series?
- What complications have you seen, how often, and who provides emergency and revision care?
- For a filler, what is the exact material, its regulatory status for penile use where I live, and whether it can be removed?
- What happens if I stop traction, the filler wears off, or the result is asymmetric?
Bottom line
Measure first and separate a normal-size concern from a reconstructive problem. Traction may produce a modest length change after at least several hours of daily use for months, but the evidence is limited and it has not demonstrated girth gain. Vacuum pumps help some men obtain erections but have not shown reliable lasting lengthening. Cosmetic surgery, fat injection and fillers carry meaningful uncertainty and complications; some materials and techniques are specifically advised against. Silicone, paraffin and petroleum jelly injections are dangerous, and adult hormones, pills, creams and supplements are not established enlargement treatments. A qualified urologist or sexual-medicine clinician should help you weigh expectations, alternatives and risk before any invasive treatment.
