What was Robert Downey Jr.’s gag?
Downey played Wayne Gale, a sensationalist television journalist. In the reported scene, Gale interviews Mickey Knox in prison. Downey dipped his shirt tail in fake blood and drew it through his fly, creating the visual suggestion of a bloody penis.

Why did Oliver Stone object at first?

The objection was about tone: Stone apparently felt the gag veered into slapstick. The account does not portray his response as a lasting rejection, however. It says he later asked to see the bit again, and that the final scene included it.
