That makes the film look more dreamlike and romantic than the first Joker, but the footage does not establish whether its dances or relationship scenes are literal events, fantasy, or some mixture of both.
What the trailer establishes—and what it leaves open
The official title is Joker: Folie à Deux, Todd Phillips’s follow-up to Joker, with Joaquin Phoenix returning as Arthur Fleck and Lady Gaga joining him. The official Warner Bros. trailer is the primary source for what appears on screen; the official DC film page supplies the studio’s synopsis and cast information.
DC’s synopsis places Arthur at Arkham awaiting trial for his crimes as Joker. It says he encounters love and “the music that’s always been inside him.” That is the official setup, not confirmation that any particular moment in the trailer is real within the story—or that Arthur’s connection with Quinzel works like a conventional romance.
Is Joker: Folie à Deux a musical?
The trailer is unmistakably music-forward, but calling the film a full musical goes beyond what its director said. At CinemaCon, Todd Phillips described it as “a movie where music is an essential element” and added, “To me that doesn’t veer very far from the first film,” according to the Associated Press.

That wording suggests music is woven into the storytelling rather than offering a simple genre label. The trailer’s songs and dance imagery may express Arthur’s inner life or the film’s subjective perspective; they do not by themselves prove that characters routinely break into conventional musical numbers.
Who is Lady Gaga playing?
TechRadar identifies Gaga’s character as Harleen Quinzel, also known as Harley Quinn, and reports that this film’s version is among the inmates at Arkham. That character identification comes from secondary reporting, rather than the brief wording of DC’s official synopsis. In the trailer, Quinzel is presented in relation to Arthur, but the footage does not settle how much of their apparent intimacy is grounded in events as they happen.

What does the trailer song tell us?
TechRadar identifies the trailer song as “What the World Needs Is Love.” Its use contributes to the footage’s contrast between institutional surroundings and romantic, heightened imagery. The song helps create a mood; it is not evidence that the film endorses a fairy-tale interpretation of Arthur and Quinzel.
Why the trailer feels like a fantasy
The promotional footage juxtaposes Arkham and Arthur’s Joker presentation with music, dance and images of his connection with Quinzel. That contrast invites a reading of the story as fantasy-tinged or subjective. It remains an interpretation of a trailer, not proof that a specific scene is imagined, that the relationship is unreal, or that the characters share a clinical condition.

The title’s phrase “finds his princess” is promotional framing, not a reliable guide to the relationship’s nature. The trailer promises a powerful connection, but it does not establish a conventional romance or tell viewers how much of what they see reflects Arthur’s perception.
How the sequel’s setup differs from the first film
The first Joker centered on Arthur’s isolation and transformation. The sequel’s official setup keeps him in an institutional setting while bringing Quinzel and music into the story. Phillips’s comments place that musical element in continuity with the first film rather than describing a complete departure from it.

The trailer’s most important shift is therefore one of emphasis: Arthur is no longer presented only in relation to his own unraveling, but also through a charged bond and music-led imagery. Whether that imagery depicts external events or a subjective world is a question the promotional footage leaves open.
