The episode seems to gather Teddy, Phil, Susie, Lilly, and Ronnie into a group of young investigators. Then a creature erupts from a movie screen and kills Teddy, Phil, and Susie. Lilly and Ronnie survive—and Lilly discovers she is holding Susie’s severed hand.
What happens in the Welcome to Derry premiere?
Matty disappears on the road out of Derry
The story begins in 1962 with Matty Clements trying to leave Derry, Maine. A family gives him a ride, but the encounter turns grotesque: the pregnant woman gives birth in the car to a winged creature. Matty disappears, and his body is not found.
Lilly and the others follow a clue to the theater
Four months later, Lilly, Teddy, and Phil investigate Matty’s disappearance. Lilly hears Matty singing “Ya Got Trouble,” from The Music Man, through a drain. That clue leads the children back to the theater where Matty was last seen. Ronnie lets them inside and starts the film.
The creature attacks through the screen
Matty appears on the screen, holding a bundled baby. The creature bursts through the image and attacks the children. Teddy, Phil, and Susie die; Lilly and Ronnie survive. The closing image is Lilly realizing that the hand she is holding belongs to Susie.

Who dies, and what is the shocking twist?
The twist is that Teddy, Phil, and Susie—introduced as though they might form the continuing child-hero group—are killed in the premiere’s theater sequence. The attack connects the creature from Matty’s opening story to the children’s investigation, while abruptly overturning the expectation that this newly assembled group is safe.
The creature does not appear as Pennywise in his familiar clown form in the premiere. The theater attack is part of the episode’s horror, but it is more accurate to describe what happens than to label it Pennywise’s “most gruesome kill.”

Why did the creators choose to kill the apparent leads?
The deaths were a deliberate creative choice, not an accidental cliffhanger. Jason Fuchs wrote an earlier version of the pilot with more survivors. Co-showrunner Brad Caleb Kane proposed killing characters who seemed to be the leads. Fuchs told TV Guide that his initial reaction was horror because he loved the characters, but he came to believe the harsher ending was necessary to create the intended effect.
Executive producer Barbara Muschietti said the ending “sets the tone, so we don’t forget for a second how unpredictable IT is,” in an October 27, 2025 TV Guide interview. In a Television Academy interview published October 28, 2025, she said, “We wanted to bring some subversion to the audience, to this story,” adding, “No one here is safe.” Those remarks explain the creators’ intention; they do not establish how every viewer responds.

Where does the premiere fit in the IT timeline?
The series is a prequel to Andy Muschietti’s 2017 and 2019 films. Its premiere is set in 1962, 27 years before the 1989 setting of the first film. The series expands Derry’s past and the mythology associated with Stephen King’s novel, but the film adaptations and the novel do not share the same timeline. The premiere makes clear that Matty’s disappearance and the theater attack are connected, but it does not settle the full nature of the creature or what happened to Matty after the opening encounter. Its final shock is not a complete explanation of Derry’s evil; it is a warning that the series may not protect characters simply because they appear central.
