Orphan (2009) is not confirmed to be based on a specific real-life case. Screenwriter David L. Johnson said Esther grew out of his concern that children were being pushed to grow up too quickly—not from a search for failed adoptions. The often-mentioned Barbora Škrlová case is an unconfirmed connection, while Natalia Grace’s story happened after the film was released.
What inspired the story of Orphan?
The film’s creators described a fictional story with a thematic starting point, not a dramatization of an identified case. In a September 4, 2009 interview with The Tech, screenwriter David L. Johnson said Esther came from his concern about children being made to grow up too fast. “It’s something that I find disturbing and Esther was the result of that concern, rather than anything to do with adoption,” he said. Johnson said he researched adoption issues to treat the subject sensitively, but described the film’s events as “pretty unlikely.” He also stressed that the story was not intended to imply that adopted children are more dangerous or that adoption is unusually perilous. In his words, it was “never a story about an unsuccessful adoption,” but about a girl who was “wise beyond her years.”
Director Jaume Collet-Serra likewise described the script as original. In an April 28, 2009 interview, he said he generally did not look at other films in the same genre for inspiration and wanted to stay true to the script. Read Collet-Serra’s interview about Orphan.

Was Orphan inspired by Barbora Škrlová?
Barbora Škrlová is often cited as a possible real-life parallel because accounts of her case describe an adult posing as a child. But the connection to Orphan has not been confirmed by the filmmakers. A 2023 ComingSoon article notes that some sources make the association while also reporting the lack of creator confirmation. It is more accurate to call Škrlová a case readers compare with the film than to say the film is based on her story. See ComingSoon’s discussion of the claim.
Could Natalia Grace’s story have inspired the film?
No. The chronology rules that out: Orphan premiered in 2009, and Natalia Grace was adopted by the Barnetts in 2010. A 2024 scholarly book discusses later comparisons between the film and allegations in Grace’s case, but a resemblance in premise does not establish that one story caused the other, nor does it verify the allegations. What is the film’s premise?
The 2009 psychological thriller follows Kate and John Coleman, who adopt Esther after the loss of an unborn child. The story turns on Esther’s concealed identity. How to assess claims that a film is based on a true story
- Check chronology: A case that occurred after a film’s release cannot be its source.
- Separate confirmation from resemblance: A creator’s account carries different weight from a later comparison or an unconfirmed attribution.
- Compare only what is established: Similarity in the premise of an adult allegedly posing as a child does not prove shared origins, equivalent facts, or the truth of allegations.
