Immaculate (2024) is a compact, graphic religious-horror film about Cecilia, a devout American nun who joins an isolated convent in the Italian countryside and discovers that its apparent miracle is connected to institutional violence. Sydney Sweeney carries the film as Cecilia, a woman whose faith is gradually forced into conflict with bodily autonomy and survival.
The film works best as convent horror, body horror, and psychological horror rather than as a conventional supernatural-possession story. Its central question is not simply whether Cecilia is carrying a holy child, but who has the authority to define her body, her faith, and her future.

Spoiler warning: The first sections below are spoiler-light. The section titled Full spoilers: Immaculate’s ending explained discusses the pregnancy, the convent’s experiment, and Cecilia’s final attempt to escape.
What is Immaculate about?
Cecilia is a young American woman whose religious vocation takes her to a remote convent in Italy. The institution appears beautiful and orderly, with ornate interiors, formal religious rituals, and an atmosphere of quiet devotion. That calm gradually gives way to unease as Cecilia encounters restricted areas, medical procedures, unexplained rules, and behavior that suggests the convent is concealing something violent.

- Horror
The turning point comes when Cecilia is discovered to be pregnant despite having no consensual sexual explanation for the pregnancy. The convent presents the event as a divine sign and treats Cecilia as specially chosen. As the story develops, however, the language of miracle increasingly functions as a tool of control.
The official synopsis from distributor Neon establishes the film’s basic setup: an American nun moves to a quiet Italian convent, where the apparent sanctuary becomes a nightmare. Is Immaculate a supernatural movie?
It uses supernatural and religious imagery, but it should not be described as a straightforward possession movie. The film initially encourages Cecilia—and the audience—to consider a miraculous explanation for her pregnancy. Its later revelations place institutional experimentation, coercion, and reproductive control at the center of the explanation.
That distinction matters. The convent’s claims are not automatically validated because they use the vocabulary of sainthood, divine selection, or religious destiny. In Immaculate, sacred language can conceal a material program carried out by people who have power over vulnerable women.
The film also should not be presented as a true story, a remake, or a direct adaptation of a specific Catholic legend. It borrows religious iconography and invokes ideas associated with the Virgin Mary, but it is an original horror story rather than a doctrinal retelling.

What is the “miracle” pregnancy?
Cecilia’s pregnancy is treated by the convent as proof that she has been divinely selected. Her virginity becomes part of the institution’s narrative: the pregnancy is framed as an extraordinary religious event rather than as a medical or criminal mystery.
The film eventually reframes that narrative. The pregnancy is connected to a deliberate institutional project involving experimentation and coercion. The convent is not merely waiting for a miracle to happen; it is participating in a program intended to create or preserve a child regarded as religiously significant.
This is why the film’s horror is more complicated than “a nun becomes pregnant through supernatural forces.” The institution turns an unexplained pregnancy into an ideological justification for controlling Cecilia. Whether the convent’s beliefs contain any genuine supernatural element is less important than the concrete fact that its leaders use those beliefs to excuse experimentation and bodily violation.
Why does the convent want Cecilia?
Cecilia is valuable to the convent because she can be made into the vessel for its religious project. Her identity as a devout young woman allows the institution to present obedience as holiness. The convent’s authority depends on persuading her that resistance would mean resisting God rather than resisting the people who are abusing her.

That manipulation gives the film its institutional horror. Cecilia is not simply trapped in a haunted building. She is surrounded by people and systems that know how to reinterpret her fear as spiritual weakness, her questions as disobedience, and her body as property belonging to a larger cause.
The exact mythology is intentionally less important than the power structure. Immaculate does not spend its brief runtime building an expansive supernatural universe. Instead, it concentrates on what happens when an institution has medical access, religious authority, and physical control over the person it claims to protect.
Why does Cecilia fight back?
Cecilia’s transformation is from obedient participant in the convent’s religious order to someone determined to preserve her own life and autonomy. Her resistance is not presented as the acquisition of magical powers. It is a physical and moral refusal to accept the role assigned to her.

The final act turns that refusal into a survival sequence. Cecilia rejects the convent’s interpretation of her pregnancy and attempts to escape its control. The violence becomes increasingly graphic and bodily, making the conflict impossible to separate from the pregnancy itself: the institution’s theological claims are expressed through medical procedures, physical confinement, blood, and pain.
The ending’s force comes from that reversal. The convent wants to transform Cecilia into evidence of its sacred mission. Cecilia instead insists that she is a person whose body cannot be claimed by the institution.
What does the ending mean?
The ending can be read as a rejection of authoritarian institutions that convert suffering into proof of faith. The convent calls Cecilia chosen, but being “chosen” gives her no meaningful agency. The label is useful to the people controlling her because it makes exploitation appear like honor.
It also gives the film’s title an ironic charge. Immaculate invokes the religious language surrounding immaculate conception and Marian imagery, but the movie uses that language as part of a horror framework. It is not offering a theological explanation of Catholic doctrine. It is showing how familiar religious symbols can be weaponized to make reproductive coercion look sacred.

Another reasonable reading focuses on institutional abuse more broadly. The film presents a supposedly protective community whose privacy, hierarchy, and rituals help conceal violence. Cecilia’s body becomes institutional property, and her apparent consent is manufactured through isolation and spiritual pressure.
These interpretations are compatible rather than mutually exclusive. The film supports readings about religious authority, reproductive control, medical abuse, bodily autonomy, and the conflict between individual conscience and authoritarian structures. It does not require one single political or theological interpretation to make its central horror clear.

Why Sydney Sweeney’s performance matters
Sydney Sweeney is the film’s central dramatic and physical presence. Cecilia begins as someone who believes obedience and faith can provide safety. As the convent’s behavior becomes more threatening, Sweeney has to communicate suspicion, terror, physical pain, and resistance with increasingly little room for conventional dialogue.
Reviews widely identified Sweeney’s performance as the film’s main strength, particularly in the intensely physical final act. The performance also has a production significance: Sweeney served as a producer as well as starring in the film, helping revive a project she had encountered years earlier.
Sweeney and director Michael Mohan had previously worked together on The Voyeurs (2021). Their collaboration on Immaculate helped bring the older screenplay to the screen, with Mohan directing and Andrew Lobel credited as writer. The project was produced through Black Bear Pictures and related companies, while Neon acquired U.S. distribution rights in December 2023. Black Bear’s production page provides additional production context.
The film’s 1970s Italian-horror influence
Immaculate draws on the visual and atmospheric language of classic 1970s Italian horror without being a remake or direct adaptation. Its remote countryside setting, candlelit interiors, dark wood, formal habits, religious architecture, and carefully composed corridors create a sense of beauty that is inseparable from menace.
The setting is important because the convent initially looks like a place of order. The ornate rooms and rituals promise protection, but they also make the institution feel sealed off from ordinary scrutiny. The more beautiful and ceremonial the surroundings become, the more disturbing it is when the film reveals the violence operating beneath them.
Cinematographer Elisha Christian worked with Mohan on the film’s controlled visual style. The production notes from SXSW’s feature on the film discuss the filmmakers’ interest in Italian-horror locations and imagery. The production also emphasized extremity: writer Andrew Lobel described Sweeney as fearless, while the filmmakers deliberately pushed the story toward graphic physical horror rather than leaving the climax at the level of suggestion.

- Religious horror: It places faith, ritual, sainthood, and religious authority inside a story about manipulation and violence.
- Convent horror: The isolated institution, restricted spaces, formal rules, and hidden history create the film’s central environment of confinement.
- Body horror: Pregnancy, medical intervention, blood, bodily violation, and the graphic birth-related climax make the body the primary site of terror.
- Psychological horror: Cecilia must determine whether the people around her are protecting her, deceiving her, or actively preparing her for exploitation.
- Institutional horror: The most frightening force is not simply an individual monster but a system capable of making abuse appear righteous.
It is a particularly strong fit for viewers who prefer intense atmosphere, a focused central performance, and a short, escalating horror story. It is less suitable for viewers expecting extensive mythology, a carefully explained supernatural cosmology, or a long investigation into the convent’s history.

In the United States, a Blu-ray release dated June 11, 2024 was listed by Blu-ray.com as a Decal Releasing disc with an 89-minute runtime, Region A designation, 1080p video, English audio, and English and Spanish subtitle options. Those are specialist-database-listed specifications rather than a guarantee that every current retailer carries the same edition.
If you want a permanent copy, Own Immaculate on Blu-ray is the most stable format-based recommendation, though inventory and pricing should be checked before purchase. Neon’s official page also provides a “Watch Now” option. Because the film’s release information differs by territory, readers outside the United States should verify local theatrical, physical-media, and digital options independently.
Should you watch Immaculate?
Watch it if you want a visually controlled, 89-minute horror film built around an isolated convent, religious symbolism, reproductive terror, and a demanding lead performance. Its strongest material arrives when Cecilia’s supposed sanctity is revealed as a mechanism for control and when Sweeney’s performance carries the story into its graphic final movement.

Frequently Asked Questions
Is Immaculate based on a true story?
No. The film should not be described as a true story, a remake, or a direct adaptation of a specific Catholic legend. It uses religious imagery and the idea of a virgin pregnancy as part of an original horror story.
Is Cecilia’s pregnancy supernatural?
The film initially presents the pregnancy as a possible miracle, but its revelations connect it to an institutional program involving experimentation and coercion. It is therefore misleading to describe the pregnancy as purely supernatural.
What is Immaculate about thematically?
Its central themes include institutional abuse, reproductive coercion, bodily autonomy, the weaponization of religious symbolism, medical abuse, and the conflict between individual conscience and authoritarian structures.
