Start a dated symptom record today and take it to your appointment. Note bleeding patterns, hot flashes and night sweats, sleep, mood, concentration, vaginal and urinary changes, sexual symptoms, and other physical changes. For each entry, add when it started, how often it happens, how long it lasts, how severe it is, whether it is changing, what you tried, and how it affects daily life. Two menstrual cycles gives a useful pattern, but do not delay care while trying to create a perfect diary.
A symptom record helps a clinician distinguish patterns from isolated events, identify problems that need separate evaluation, and discuss treatment options. There is no routine test that confirms perimenopause; assessment usually combines age, symptoms, menstrual history, contraception, medical history, and examination when appropriate.
Why a dated record is more useful than a symptom list
Perimenopause symptoms can vary from day to day and from cycle to cycle. A list that says “hot flashes, poor sleep and mood changes” does not show whether they occur together, whether they follow a bleeding change, or whether they are becoming more disruptive. Dates and patterns give your clinician something that can be assessed alongside your history.
The American College of Obstetricians and Gynecologists (ACOG) describes symptom tracking as a way to make it easier for you and your ob-gyn to discuss concerns and develop a management plan together. A dated record also helps you remember details that are easy to lose during a short appointment.

Use two linked records:
- A daily log for regular observations such as bleeding, sleep, mood and symptoms that occur every day.
- An event log for intermittent events such as a sudden hot flash, a palpitation episode, bleeding after sex or a severe headache.
Paper, a spreadsheet, a notes app or a health app can all work. The best method is the one you can complete consistently and show to a clinician without exposing information you do not want to share.
What to record each day
1. Bleeding and cycle information
Record the first and last date of every period. For each day of bleeding, note intensity using words or a scale you can repeat consistently, such as light, medium or heavy. Also record:
- Cycle length, counted from the first day of one period to the first day of the next.
- Spotting before or after a period.
- Clots, including whether they are new or unusually large for you.
- Bleeding between periods.
- Bleeding after sex.
- Any month in which a period is late, unusually short or unusually long.
Do not treat irregular timing as proof that a symptom is hormonal. Bleeding that is much heavier than usual, occurs between periods or after sex, or appears after 12 months without a period needs medical assessment.
2. Hot flashes and night sweats
For each episode, record the time, approximate duration, severity and possible trigger. A useful entry might say “9:40 p.m., moderate, about three minutes, after a glass of wine,” rather than simply “night sweats.” Count episodes during the day and overnight. If you wake because of sweating, record the awakening and how long it took to return to sleep.

Frequency varies widely. ACOG reported in 2025 that about one third of women have more than 10 hot flashes per day; that figure is a population estimate, not a threshold for diagnosis or treatment.
3. Sleep
Record when you tried to sleep, estimated time to fall asleep, number and timing of awakenings, night sweats, final wake time and how rested you felt. Note whether insomnia began before or after another symptom and whether a change in routine, caffeine, alcohol, illness or stress could have contributed.
4. Mood, thinking and concentration
Use the same simple scale each day, such as 0 to 10, and add a short description. Track low mood, anxiety, irritability, tearfulness, loss of interest, concentration problems and memory lapses. Describe an example when possible: forgetting a familiar word during a meeting is more informative than “brain fog.” Record whether the problem affected work, relationships, driving, exercise or household tasks.

Persistent or severe low mood, or thoughts of self-harm, are reasons to seek prompt help rather than assuming the cause is perimenopause.
5. Vaginal, urinary and sexual symptoms
Record vaginal dryness, burning, irritation or pain; pain with sex; changes in libido; urinary urgency, frequency, leakage or repeated urinary symptoms. Note what makes a symptom better or worse and whether it interferes with intimacy, exercise, sleep or daily activities. These symptoms can have several causes, so precise descriptions help your clinician decide what to evaluate.
6. Other physical changes
Add headaches, palpitations, joint or muscle pain, hair or skin changes and weight change. For palpitations, record the time, duration, activity, associated dizziness or breathlessness and whether your pulse felt fast or irregular. For headaches, note location, severity, duration, visual symptoms, nausea and any new pattern.
Chest pain, fainting, severe breathlessness or a new neurological symptom warrants urgent medical attention rather than routine diary keeping.
7. Context, treatments and impact
At the bottom of each day, record medicines, supplements, contraception, alcohol, caffeine, major stress, illness, exercise and anything you tried for relief. Write whether the intervention helped, did nothing or caused a problem. For every symptom, answer four questions:
- When did it begin?
- How often does it occur and how long does each episode last?
- Is it improving, worsening or staying about the same?
- What effect does it have on sleep, work, relationships, exercise or other activities?
Those details mirror questions in ACOG’s symptom checklist, including how long a problem has been present and whether it is getting worse over time.
A practical tracking template
Use one row per day and add an event entry whenever something important happens. Keep ratings consistent; a “7” should mean roughly the same level each time.

| Field | What to enter |
|---|---|
| Date and cycle day | Calendar date; cycle day if you are bleeding or can identify the cycle start |
| Bleeding | None, spotting, light, medium or heavy; clots; between-period or post-sex bleeding |
| Hot flashes/night sweats | Count, times, approximate duration, severity and suspected triggers |
| Sleep | Bedtime, sleep-onset estimate, awakenings, final wake time and restfulness |
| Mood and cognition | 0–10 ratings plus a short example of anxiety, irritability, low mood, concentration or memory change |
| Vaginal, urinary and sexual symptoms | Dryness, pain, urgency, leakage, libido or pain with sex |
| Other symptoms | Headache, palpitations, joint or muscle pain, hair or skin change, weight change |
| Context and actions | Medicines, supplements, contraception, caffeine, alcohol, stress, illness, exercise and what you tried |
| Impact | Effect on work, relationships, exercise, sleep or routine activities |
For an event log, add the exact time, what you were doing, the symptom’s severity and duration, associated symptoms, possible trigger and what happened afterward. A phone note can be useful for a sudden event, but transfer key details into your main log so the record remains in date order.
Choosing paper, a printable or an app
There is no medically superior format. Compare tools on whether they capture the fields you need and whether you can use them reliably.
| Option | Useful advantages | Limitations to check | Clinician sharing and privacy | Cost evidence |
|---|---|---|---|---|
| Paper journal | Fast, works offline, easy to annotate and accessible without a device | Totals and trends must be calculated manually; handwriting may be difficult to read | Bring the journal or photograph selected pages; storage is under your control | Specific product price not stated in the available guidance |
| Printed tracker | Pre-labeled fields can prompt complete entries and make dates easy to scan | Limited space and less flexibility when symptoms change | Bring completed sheets; store them securely | Printing cost not stated in the available guidance |
| Spreadsheet or notes file | Searchable, sortable and easy to total; can be exported or printed | Requires a device and a consistent naming or rating system | Check sharing permissions, backups and whether sensitive notes sync to a cloud account | Software cost not stated in the available guidance |
| Health or symptom app | Reminders, graphs and quick event entry may improve consistency | Fields, definitions and export functions vary; some may not capture urinary, sexual or mental-health details well | Read the privacy policy, data-retention terms and export options before entering sensitive information | App price and subscription terms not stated in the available guidance |
Accessibility matters. Choose large print, voice entry, high contrast or another format that you can use during a difficult symptom episode. If someone helps you keep records, decide which information may be shared with that person and with your clinician.
How long to track before the appointment
Start now, even if your appointment is next week. NHS guidance for symptom diaries commonly recommends at least two menstrual cycles to reveal a pattern. That is a useful target, not a requirement for receiving care. Bring an incomplete record rather than postponing an appointment while waiting for more data.
- Write down the first day of your most recent period and any bleeding since then.
- Enter daily symptoms at a consistent time, preferably in the evening while events are still easy to recall.
- Log intermittent episodes immediately or as soon as practical.
- At the end of each week, mark the three symptoms that caused the greatest disruption.
- Before the appointment, review the record for changes in timing, severity, frequency and impact.
If you do not have regular periods, use calendar dates and event frequency rather than trying to calculate a cycle length. Hormonal contraception can alter or suppress bleeding, so include the method and how long you have used it.
Turn the log into a one-page appointment summary
Clinicians can use detailed entries, but a short overview helps them see the main issue quickly. Prepare these items:

- Current treatments: medicines, over-the-counter products, supplements and what you tried for each symptom.
- Contraception: method, start date if known, missed doses and any recent change.
- Medical context: relevant diagnoses, surgeries, migraines, mental-health history, thyroid or bleeding conditions, and family history that may affect decisions.
- Preferences and constraints: treatments you want to discuss, treatments you cannot use, fertility or contraception priorities, and concerns about side effects or cost.
- Questions: write them down, because symptoms and options can be discussed together rather than one at a time.
ACOG’s tracker includes prompts about treatment interests. NICE guidance emphasizes individualized assessment as symptoms and circumstances change, so explain what matters most to you rather than presenting the diary as a pass-or-fail test.

Do you need a blood test to know if you are in perimenopause?
Usually, no routine blood test is required. The NHS states that there is no routine test for perimenopause and menopause. A clinician may identify perimenopause from age, symptoms and menstrual changes, while considering contraception and other possible causes. Tests may be appropriate when the history is unusual or another condition needs investigation, but a single hormone result does not replace the overall clinical assessment.
Menopause is generally defined as 12 months without a period when hormonal contraception is not affecting the cycle. Contraception can make bleeding patterns harder to interpret, so tell the clinician exactly what you use. Do not stop contraception merely to make the pattern easier to read; discuss pregnancy prevention and any change first.
The average age of the last menstrual period is 51, according to ACOG’s 2025 patient information, but an average is not a diagnostic cutoff. Symptoms can start earlier or later. NHS information published in 2026 notes that symptoms usually last seven to nine years, sometimes longer; duration differs substantially between individuals.
When symptoms should not wait for a routine appointment
Arrange medical assessment promptly for:
- Bleeding that is much heavier than your usual pattern.
- Bleeding between periods or after sex.
- Any bleeding after 12 months without a period.
- Symptoms beginning before age 45, particularly if they are sudden or marked.
- Palpitations with chest pain, fainting, severe breathlessness or significant dizziness.
- A severe, sudden or substantially changed headache, especially with neurological symptoms.
- Persistent or severe low mood, or thoughts of self-harm.
Use emergency services for immediate danger, severe chest pain, fainting, heavy uncontrolled bleeding or thoughts of acting on self-harm. A diary can support evaluation, but it should never delay urgent care.
Common tracking mistakes and fixes
Recording only the worst days
Problem: A record made after a crisis overstates frequency and misses ordinary days.
Fix: Make a brief daily entry even when symptoms are absent. “None” is useful information.
Changing the rating scale
Problem: Using “mild” one day and a numerical score the next makes trends difficult to compare.

- Powerful Pass-Through Charging: Supports up to 85W pass-through charging so you can power up your laptop while you use the hub. Note: Pass-through charging requires a charger (not included). Note: To achieve full power for iPad, we recommend using a 45W wall charger.
- Transfer Files in Seconds: Move files to and from your laptop at speeds of up to 5 Gbps via the USB-C and USB-A data ports. Note: The USB C 5Gbps Data port does not support video output.

- What You Get: Anker 332 USB-C Hub (5-in-1), welcome guide, our worry-free 18-month warranty, and friendly customer service.
Fix: Choose one scale and define it at the top of the page. Add a concrete example for unusually high or low scores.
Leaving out contraception and treatments
Problem: A clinician cannot interpret bleeding changes without knowing about hormonal contraception or recent medication changes.
Fix: Keep a current medicines and contraception list beside the diary and date every change.
Assuming every symptom is perimenopause
Problem: New bleeding, cardiovascular symptoms, severe headaches and major mood changes can have causes that need separate care.
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →Scan for outdated or missing drivers - takes under a minuteDriver Scan →Fix: Describe the symptom neutrally, include warning features and ask what else should be ruled out.
Sharing more data than necessary
Problem: An app or shared document may expose intimate information beyond the appointment.
Fix: Review privacy settings, use a passcode, export only the pages or date range needed and remove unnecessary identifiers from printed copies.
What a useful record lets you discuss
Bring the diary and ask the clinician to help interpret the pattern: whether bleeding is within an expected transition, whether another condition should be considered, which symptom deserves treatment first, and how to monitor a chosen treatment. The record is not meant to prove that every change has one cause. Its value is that it shows timing, severity, trend and impact clearly enough for a shared decision.
