To enroll in the World Trade Center Health Program (WTCHP), first identify whether you are an FDNY responder, another WTC responder, a WTC survivor, or a Pentagon/Shanksville responder. Confirm that your activity or presence, location, dates, and—when required—hours satisfy the applicable rules. Then collect supporting records, complete the matching application, and submit it through the OASIS online portal or by mail or fax. Online applicants must print, hand-sign, and upload a signature form; electronic signatures are not accepted.
Identify the application category that fits you
The Program uses separate applications because the qualifying events and evidence differ by group. Choosing the wrong category can delay processing, so establish your status before gathering documents. If more than one description seems to fit, compare the work or presence that actually supports your claim and use the form designed for that circumstance rather than filing several incomplete applications.
| Category | Who it generally covers | Application approach | Health-care pathway after enrollment |
|---|---|---|---|
| FDNY responder | Eligible members of the Fire Department of the City of New York who performed qualifying 9/11-related duties | FDNY responder application with records supporting the required activity, site, dates and hours | Annual monitoring and treatment for certified WTC-related conditions |
| WTC general responder | Responders who were not applying through the FDNY category and performed qualifying work at a covered New York City site | General responder application with documentation of work, location, dates and hours | Annual monitoring and treatment for certified WTC-related conditions |
| WTC survivor | A person who was present in the New York City Disaster Area because of work, residence, school, childcare or adult daycare, or who was in the dust or dust cloud on September 11, 2001 | Survivor application with evidence of qualifying presence and circumstances | Screening-eligible survivors receive one initial health evaluation; survivors who become certified-eligible can receive annual monitoring and treatment |
| Pentagon/Shanksville responder | Responders who performed qualifying duties at the Pentagon or the Shanksville, Pennsylvania crash site | Pentagon/Shanksville responder application with records tied to the site, dates, activity and hours | Annual monitoring and treatment for certified WTC-related conditions |
The Program’s official Apply materials list these four groups. A person can meet a broad description and still be ineligible: the eligibility rules require every applicable element to be established, and enrollment does not guarantee that a condition will be certified or that a particular treatment will be covered.
The main comparison is not simply “responder versus survivor.” Responder applications focus on qualifying duties and the hours spent at a covered site. Survivor applications focus on why and where the person was present, such as residence, employment, school, childcare, adult daycare, or exposure to the dust or dust cloud. The application form, evidence, and health pathway follow that distinction.

Check the four eligibility elements before applying
For responder categories, evaluate the same four questions the Program uses. Survivors must document the qualifying kind of presence and the applicable location and time requirements. Treat this as a pre-filing test: if an element is unclear, look for a record that addresses it before submitting the form.
1. What qualifying activity or circumstance applies?
Responders must show that they performed a qualifying response, recovery, cleanup, support or related duty recognized by the Program for their category. Survivors must show a qualifying reason for being present—such as working, living, attending school, using childcare or adult daycare—or presence in the dust or dust cloud on September 11, 2001. A general connection to the events, without a qualifying activity or circumstance, is not enough.
2. Where did it occur?
The location must match the category’s covered site or area. For WTC survivors, the relevant area is the New York City Disaster Area identified by the Program. Pentagon and Shanksville responders use the requirements specific to those sites. A general statement that you were “in New York” is not enough; records should identify the covered site or area whenever possible.
3. When were you there?
Your dates must fall within the period recognized for the applicable activity or presence. Give exact dates when you can, and explain any uncertainty rather than guessing. The application and supporting records should tell the same chronological story. If records use a deployment period rather than individual shifts, include the document that explains that period and identify the dates you personally served.
4. How many hours were involved?
Responder eligibility can depend on the required number of hours at a qualifying site. Document the hours with schedules, time records, deployment paperwork or another reliable source. The hours rule does not apply in the same way to every survivor circumstance, so use the survivor form’s instructions for your situation. Do not assume that a full-time job, a general assignment, or a single visit automatically proves the hours requirement.
The Program can request clarification or additional documentation. Meeting one, two or three elements does not establish eligibility if a fourth applicable element remains unproven. Your goal is a file in which the activity or circumstance, place, dates, and hours (where applicable) can be matched without the reviewer having to infer missing facts.
Gather evidence that matches your category
Collect records before opening the application. The forms cannot be processed without required supporting documentation, and a well-organized submission makes it easier for the Program to verify the claim. Read the instructions for your exact form first; the examples below illustrate the kind of record that can establish a requirement, but they do not replace the form’s instructions.
Responder evidence
Useful records are those that identify the qualifying activity, site, dates and hours. The regulations give examples including:

- Site credentials or access records.
- Comparable official documentation that ties you to the covered work, place and period.
Submit legible copies and retain the originals. If a single document does not establish every required element, combine records that fill the gaps. For example, a roster may establish assignment and dates while time records establish hours. A site credential may show access to a location but still need a separate document showing the dates or duties. Do not alter a record to make it fit; explain discrepancies in the space provided or in an attached statement.
Survivor evidence
Survivor documentation should establish both the qualifying presence and why you were there. Depending on the circumstance, that may include records connected to employment, residence, school attendance, childcare or adult daycare, or evidence placing you in the dust or dust cloud on September 11, 2001. Follow the survivor application’s instructions for acceptable documentation and provide a clear explanation when records are unavailable. Explain the relationship between each attachment and the circumstance you selected rather than sending an unlabeled stack of papers.
Prepare a consistent record set
Use the same name, address, dates and identifying details across the application and attachments. Make a list of every document you are sending. Keep a complete copy of the application, signature page and evidence; you may need it for follow-up or your first appointment. If a record contains information that is not relevant, do not redact it in a way that removes the dates or identifying details needed to verify your claim.
Use the correct application and submission method
The Program provides separate forms for FDNY responders, other WTC general responders, survivors and Pentagon/Shanksville responders. Start from the official WTCHP Apply page and select the form for your category. You can submit online through OASIS or use a downloaded paper form. The application channel does not change the eligibility rules or the need for supporting records.
Online enrollment through OASIS
- Open the WTCHP online application in OASIS and select the category that matches your qualifying circumstances.
- Enter your personal information and describe the activity or presence, location, dates and hours requested by the form.
- Upload the supporting records required for your category. Use clear scans or photographs in the file types and size limits shown by the portal.
- When the portal generates the signature form, print it.
- Sign the printed form with ink. Electronic or typed signatures are not accepted.
- Scan or photograph the ink-signed form and upload it where OASIS instructs. The portal states that it will not process an online application without this form.
- Review every field and attachment, submit the application, and save the confirmation and a full copy of what you sent.
Before submitting, check that every uploaded page is readable, that the signature page belongs to the application you completed, and that no attachment was accidentally omitted. Do not email an application. Email is available for questions, not for filing, and sensitive health information should not be sent by email.
Paper enrollment by mail or fax
- Download the form for your category and complete every applicable section.
- Attach copies of the evidence requested in the instructions.
- Sign the paper form in ink. Electronic signatures are not accepted for paper submissions either.
- Mail the package to WTC Health Program, P.O. Box 7000, Rensselaer, NY 12144, or fax it to 1-877-646-5308.
- If you use a shipping service or certified mail, use 327 Columbia Turnpike, Rensselaer, NY 12144.
- Keep the completed form, attachments and proof of mailing or fax transmission.
Use one submission method for a complete application unless the Program tells you otherwise. Sending duplicate or conflicting versions can make it harder to determine which information is current. If you fax, retain the transmission confirmation; if you mail, retain the receipt and a copy of the exact package.
What happens after the Program receives your application
Processing and requests for information
Processing begins when the Program receives the application. Staff may call, email or write if information is missing or unclear. Respond promptly and keep copies of every response. If you move, change your phone number or change your email address while the application is pending, update the Program so that you do not miss a request. A request for more records is not itself a denial; it is an opportunity to complete or clarify the file.
Eligibility decision
The Program sends a written eligibility decision. An approval explains the next steps for health-care access. A denial letter explains the available appeal options and how to proceed. Read the letter carefully; an appeal is governed by the instructions and deadlines in that notice. Keep the decision with your application records, even if you plan to appeal.

Expedited enrollment for medical necessity
The Program can consider expedited enrollment when there is a medical-necessity reason, including a newly diagnosed cancer, active cancer treatment, a terminal illness or hospice care when the condition is believed to be related to 9/11 exposure. If this applies, identify the urgent circumstance through the application process or when speaking with Program staff. Expedited consideration does not remove the underlying eligibility requirements, and it does not turn an otherwise incomplete application into an automatic approval.
What WTCHP enrollment provides
Screening for eligible survivors
Survivors who are eligible for screening receive one initial health evaluation. This evaluation is an entry point for assessing potential WTC-related health needs; it is not the same as automatic certification of every condition. The one-time initial evaluation should therefore be distinguished from the continuing monitoring available after certification.
Monitoring and treatment
Responders and survivors who are certified-eligible can receive annual monitoring examinations and medical and mental-health treatment for certified WTC-related conditions. Certification and coverage are separate decisions from initial enrollment, so an enrollment approval does not by itself guarantee treatment for an unrecognized condition. Monitoring is an annual pathway for eligible responders and certified-eligible survivors; screening-eligible survivors who have not become certified-eligible are described as receiving the initial evaluation.
Limited federal health coverage
The Member Handbook describes the Program as limited federal health coverage. It does not replace primary health insurance, and care must be obtained through participating providers. Ask the Program or your care team how a particular service is handled before assuming that enrollment will pay for it. Enrollment also does not create compensation eligibility or substitute for another benefits program.
WTCHP enrollment does not enroll you in the VCF
The September 11th Victim Compensation Fund (VCF) is a separate program with different eligibility criteria and a different enrollment process. Completing a WTCHP application does not create a VCF claim or enroll you in the VCF. If you are considering compensation, review the VCF’s own requirements and registration process separately; do not assume that an approval from one program transfers to the other.
Common problems and how to avoid them
Choosing a general responder form when an FDNY form applies
FDNY responders have a dedicated application. If your qualifying service was through the FDNY, use that form rather than treating yourself as a general responder.
Submitting an unsigned online application
Completing OASIS fields is not enough. Print the generated signature form, sign it in ink and upload it. A typed name, electronic signature or an omitted signature page can prevent processing.
Sending evidence that proves presence but not the required activity or hours
Responder records need to connect you to the qualifying work, site, dates and hours. Add records that fill each missing element instead of relying on a single document that only shows employment or general deployment. For survivors, connect the presence to the qualifying reason or dust exposure described on the form.

Assuming a broad location is sufficient
Being somewhere in the New York region does not by itself identify a covered site or the New York City Disaster Area. Use records that name the site, address, assignment or other location detail requested by the applicable form.
Using email as the application channel
The Program does not accept applications by email. Use OASIS, mail or fax, and reserve email for general questions. Do not include sensitive health information in an ordinary email.
Assuming enrollment guarantees covered treatment
Enrollment, certification of a WTC-related condition and authorization of covered care are related but distinct steps. The eligibility page expressly warns that enrollment does not guarantee certification or treatment.
Losing the submitted record
Keep the application, evidence, signature form, confirmation and mailing or fax proof together. Bring a copy to your first appointment if the Program accepts you.

Frequently Asked Questions
How do I know which WTCHP application to use?
Choose among the FDNY responder, WTC general responder, WTC survivor, and Pentagon/Shanksville responder applications. Match the form to the activity or presence that you can document; responder forms focus on qualifying duties, site, dates and hours, while the survivor form focuses on qualifying presence and circumstances.
What proof does a responder need?
Provide records that connect you to the qualifying activity, covered site, dates and required hours. Examples in the regulations include pay stubs, an official personnel roster, an employer statement under penalty of perjury, site credentials, or comparable official documentation. You can combine records when one document does not establish every element.
Can I apply online?
Yes. Submit through the OASIS portal, upload the required supporting records, then print the generated signature form, sign it in ink, and upload it. The portal will not process the online application without that signed form, and electronic signatures are not accepted.
Where do I send a paper application?
Mail it to WTC Health Program, P.O. Box 7000, Rensselaer, NY 12144, or fax it to 1-877-646-5308. For shipping or certified mail, use 327 Columbia Turnpike, Rensselaer, NY 12144. Keep proof of mailing or fax transmission.
Does WTCHP enrollment also enroll me in the VCF?
No. The September 11th Victim Compensation Fund has different eligibility criteria and a separate enrollment process. A WTCHP application or approval does not create a VCF claim.
What if I have an urgent 9/11-related medical condition?
The Program can consider expedited enrollment for medical-necessity situations such as newly diagnosed cancer, active cancer treatment, terminal illness or hospice when the condition is believed related to 9/11 exposure. Expedited review does not remove the underlying eligibility requirements.
The Bottom Line
Use the category-specific WTCHP form, prove the applicable activity or presence, location, dates and hours, and submit a complete ink-signed application through OASIS, mail or fax. Keep every record, answer follow-up requests, and remember that WTCHP enrollment and VCF registration are separate.
