How to Compare UK Fertility Clinics: Questions About Consent, Risks, Costs and Complaints

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Compare fertility clinics on regulation, suitability, consent, safety, total cost, access and complaints handling—not on one headline success rate. In the UK, start by checking the clinic’s Human Fertilisation and Embryology Authority (HFEA) licence, inspection report and licensing decisions. Then ask for a personalised treatment plan, the outcome measure behind every quoted rate, written information about consent and storage, a clear risk-management plan and an easy-to-find complaints procedure.

This guide is UK-specific. HFEA licensing, NHS funding rules, waiting times and complaint routes do not automatically apply in other countries, and some services in a patient’s journey may be outside HFEA regulation.

What to compare before choosing a fertility clinic

A clinic is not simply a success-rate number. A meaningful comparison asks whether the clinic is licensed for the service you need, whether its experience fits your diagnosis and age, how clearly it explains choices and risks, what the complete treatment will cost, how reachable it is, and what happens if something goes wrong.

Use the same questions at every clinic and record answers in writing. Marketing pages often describe a package, a selected year or a particular patient group. Those figures may not describe the treatment, time period or patients being offered to you.

Comparison area Evidence to request Why it matters
Regulation HFEA licence, latest inspection report and licensing decision Shows whether the relevant licensed activities are authorised and what the regulator required the clinic to improve.
Treatment fit Recommended treatment, alternatives and patient-specific rationale A clinic’s case mix and methods affect outcomes; a different clinic may be more suitable for your circumstances.
Outcomes HFEA-standardised births per egg collection and births per embryo transferred, with the matching treatment, year and patient group Different denominators answer different questions and are not personal predictions.
Consent and support Copies or explanations of consent forms, storage rules, withdrawal process and counselling provision Consent can affect storage, use of embryos or gametes, and legal parenthood.
Safety Common side effects, serious risks, OHSS prevention and out-of-hours response You need to know what is expected, what is urgent and who will act.
Cost and access Personalised itemised quote, medication and storage charges, waiting times and opening hours Advertised packages may omit necessary or likely costs.
Complaints Written policy, named contact, response times and escalation routes A transparent process makes it possible to challenge treatment, billing or communication problems.

How to check regulation and inspection records

Confirm the right HFEA licence

Use the HFEA clinic directory to confirm that the clinic is licensed for the specific activity you are considering. A clinic may offer several services, while only some activities fall within the licence. The HFEA sector report explains that a patient journey can include licensed and independent services; only licensed clinics may carry out the licensed activities described by the regulator. Do not assume that every consultant, laboratory, counselling service or testing provider connected with a clinic is covered in the same way.

Ask the clinic to state which organisation holds the licence, which site will perform each procedure and where laboratory work will take place. If treatment is split between sites, check the licence status and responsibility for each stage.

Read both the inspection report and the licensing decision

An HFEA inspection report describes areas of good practice and areas needing improvement. The licensing committee makes the final licensing decision. Read both documents on the individual clinic page rather than relying on a clinic’s summary of its regulatory status.

Look for the inspection date, the services inspected, required actions, deadlines and any follow-up decision. An improvement requirement is not automatically evidence of unsafe current care; it is evidence that the regulator identified a gap and specified what had to change. Conversely, a favourable inspection is not a guarantee of a particular outcome for you. Ask the clinic what it changed in response to findings and how it checks that the change is sustained.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Understand what regulatory records can and cannot tell you

Inspection records concern compliance with standards and the operation observed at the time. They do not rank clinics by personal success rate, and they cannot predict how your treatment will go. Treat the regulatory record as one essential part of due diligence alongside clinical fit, consent, safety, cost and communication.

How to interpret clinic success rates

Use the HFEA’s standardised measures

The HFEA publishes clinic treatment, pregnancy and birth data for January 2022 through December 2024 on its clinic comparison pages. It reports births per egg collection procedure, births per embryo transferred and multiple-birth rates. These are population statistics, not an individual patient’s probability of having a baby.

“Births per egg collection” relates the number of births to collection procedures. “Births per embryo transferred” relates births to transfer procedures. A clinic quoting one measure should tell you which denominator it uses; ask why that measure is relevant to the treatment being proposed. A rate based on transfers cannot answer the same question as a rate based on all collection cycles, including cycles in which no embryo was transferred.

Match the number to your circumstances

Ask for the treatment type, patient age band, use of donor material if relevant, fresh or frozen transfer, and the period covered. Then ask the clinician to interpret the figure in light of your diagnosis, ovarian reserve, sperm factors, previous treatment and other circumstances. Clinic rates do not predict an individual result.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The HFEA warns that it is extremely difficult to compare clinic success rates fairly. Clinics treat different patient populations, use different referral patterns and may have different proportions of complex cases. A lower published rate does not automatically mean poorer care. Results from a clinic with a small number of cycles can be unstable: a few births more or fewer can move the percentage substantially.

Detect selective presentation

Check that the claim refers to the specific clinic, not a group’s best-performing site; to the stated years, not a selected favourable year; and to the same treatment and patient group you are considering. Be cautious when a page highlights a subgroup without showing the wider denominator or presents pregnancy rates while leaving birth rates unclear.

HFEA data are snapshots and may be updated retrospectively. Record the date you viewed the page and ask the clinic whether its quoted figure is taken directly from the latest HFEA dataset.

Questions about consent, storage and counselling

Identify every decision covered by consent

Written consent is required for fertility treatment and for specified matters involving storage, donation, surrogacy, information disclosure and, where applicable, legal parenthood and withdrawal of consent. Ask which decisions you are consenting to now and which will require a separate form later. Request copies before treatment so you have time to read them rather than signing during a rushed appointment.

  • Which decisions am I consenting to today?
  • What choices remain open after this form is signed?
  • What happens to eggs, sperm or embryos that are already in storage?
  • What are the storage period, renewal process and fees?
  • How and when can I change or withdraw consent?
  • What treatment or storage consequences follow if consent changes?

Ask about death, separation and changed plans

Consent can determine what happens to stored material if you die, separate from a partner or no longer want treatment. In some circumstances it also affects legal parenthood. Ask the clinic to explain the practical effect of each option in plain language and to tell you when a new form is required.

Do not rely on an informal promise that a partner or relative will decide later. The clinic must follow the valid consent on its records. Tell staff promptly about changes in relationship status, contact details or intentions, and ask for written confirmation of any update.

Check counselling arrangements

Licensed clinics must offer you the chance to receive counselling. Ask whether counselling is provided by someone independent of the treatment team, how appointments are arranged, whether a partner can attend, and whether support is available after an unsuccessful cycle, a pregnancy loss or a decision to stop treatment. Counselling is not a substitute for medical information; you should be able to obtain both.

Questions about treatment risks and emergency care

Request an individual risk explanation

Ask what benefits and risks apply to the treatment recommended for your circumstances, including risks to you and to a resulting child. HFEA guidance expects pre-treatment information about likely outcomes and treatment risks. Ask the clinician to distinguish common, usually manageable side effects from uncommon emergencies and from risks that depend on your diagnosis or medication.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Ask specifically about ovarian hyperstimulation syndrome

Ovarian hyperstimulation syndrome (OHSS) is a key example to discuss. Ask how the clinic assesses your risk before and during stimulation, what changes it can make to reduce that risk, which symptoms require urgent contact and where you should go outside office hours. Obtain a written number or service name for evenings, nights and weekends. Find out whether the clinic has arrangements with a hospital for assessment if symptoms become severe.

Clarify other safety questions

  • Which symptoms should prompt a same-day call, an emergency call or attendance at hospital?
  • Who reviews blood tests and scans, and how quickly are results communicated?
  • Who is responsible when a procedure is performed at another site?
  • How are medication errors, laboratory incidents or unexpected results disclosed?
  • What follow-up is included after egg collection, transfer or a negative test?

A clinic should be able to explain how it learns from incidents and mistakes without blaming you for raising a concern.

How to assess treatment add-ons

An add-on is an extra test, medicine, device or laboratory technique offered alongside standard treatment. Before agreeing, ask four separate questions:

  1. What evidence supports this add-on for patients like me?
  2. What benefit is expected, and how large or uncertain is that benefit?
  3. What are its risks, side effects and opportunity costs?
  4. What is the total price, and does declining it change the standard treatment plan?

Ask whether the evidence concerns live birth, pregnancy, miscarriage or another outcome. A test that changes a laboratory measurement is not automatically proven to increase the chance of a healthy birth. Request written information and time to decide. You should not be made to feel that refusing an add-on means you are refusing proper care.

Compare prices on a like-for-like basis

Insist on a personalised, fully costed plan

NHS guidance advises private patients to request a personalised, fully costed treatment plan explaining exactly what is included, such as fees, scans and necessary medicine. Ask for the plan to name every expected consultation, procedure, laboratory step and follow-up. The Competition and Markets Authority (CMA) warns that advertised packages can omit essential or likely elements.

Cost item Question to ask
Initial assessment Are consultation, history, examination and baseline tests included?
Medication Is medicine included, capped or charged separately? Which drugs are likely for this protocol?
Monitoring How many scans and blood tests are expected, and what is the fee for each?
Procedures Are egg collection, anaesthesia, sperm preparation and embryo transfer itemised?
Embryology Are fertilisation, culture, freezing and thawing included?
Storage What is the initial storage period, renewal fee and charge for transport or disposal?
Unplanned care What happens financially if a cycle is cancelled, medication changes or OHSS requires care?
Add-ons Are optional extras excluded from the headline price, and what is each one’s total cost?

Compare total expected cost, not the headline package. Put two clinics’ plans in the same spreadsheet and mark each item as included, excluded, uncertain or dependent on results. Ask the clinic to update the estimate if your medication dose, number of scans or embryo-storage needs change.

Check NHS eligibility separately

NHS eligibility criteria and waiting lists vary across UK areas. Your local commissioning arrangements determine whether treatment is funded, how many cycles may be offered and what conditions apply. Check your local NHS route rather than inferring eligibility from a national summary or from a private clinic’s statement.

Compare waiting times, opening hours and communication

Ask how long it normally takes from referral to consultation, from decision to treatment and between cycles. Confirm appointment hours, travel requirements, parking or transport issues and whether monitoring can be done near your home. A clinic that is difficult to reach may create practical and emotional strain during frequent scans and medication changes.

How to compare complaints procedures

Read the clinic’s written policy before treatment

The CMA says a complaints policy should be written, easy to find and understand, and should not discourage a patient from complaining or escalating. Check where to submit a complaint, whether email or post is accepted, who acknowledges it, the expected response time and how an independent review or appeal works.

Ask whether the same route covers clinical care, billing, advertising and an individual doctor. Keep copies of consent forms, invoices, appointment notes, emails and the dates of phone calls. A concise chronology makes it easier to identify what happened and what remedy you want.

Use the route that matches the problem

Issue Possible route Important limit
Licensed clinic’s standards or conduct HFEA, after using the clinic’s own process where appropriate Regulatory action is not the same as compensation or a personal treatment decision.
Doctor’s professional conduct General Medical Council (GMC) The GMC regulates professional registration and conduct, not every billing or contractual dispute.
Misleading advertising Advertising Standards Authority (ASA) Provide the advert and explain which claim may mislead.
Consumer-law concern Competition and Markets Authority (CMA) information and enforcement channels The CMA cannot intervene in an individual dispute.

Ask the clinic how it records incidents and shares learning. You can complain even if you are still receiving treatment; ask whether the complaint will affect your care and who will remain your clinical contact.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A step-by-step comparison process

  1. Write your priorities. Note the treatment you are considering, travel limits, funding status, need for donor or fertility-preservation services, counselling needs and any previous treatment.
  2. Shortlist licensed providers. Verify the HFEA licence for each relevant site and service.
  3. Read the regulatory record. Save the latest inspection report and licensing decision, including any required improvements and follow-up.
  4. Request identical information. Ask each clinic for a personalised plan, itemised quote, outcome measures, consent documents, risk information, counselling details and complaints policy.
  5. Check the denominator. For every outcome figure, record treatment, patient group, period, clinic site and whether the rate is per collection or per transfer.
  6. Test the safety plan. Ask about OHSS, urgent symptoms, laboratory incidents and out-of-hours contacts.
  7. Price the whole pathway. Include medicines, monitoring, procedures, freezing, storage, transport, likely changes and optional add-ons.
  8. Score evidence, not impressions. Use the same written criteria for every clinic and leave an item blank rather than guessing.
  9. Take a second opinion. Ask another qualified clinician to review the proposed treatment when the diagnosis is complex, an add-on is expensive or the explanation is unclear.
  10. Recheck before signing. Confirm that consent forms, costs, storage instructions and contact details still reflect your plans on the day treatment starts.

A practical clinic comparison worksheet

Copy this table for each clinic. The purpose is not to create a universal “best clinic”, but to expose unanswered questions and differences that matter to you.

Question Clinic A Clinic B Evidence or follow-up
HFEA licence and site Record Record Check directory and scope.
Latest inspection finding Record Record Read report and licensing decision.
Recommended treatment and alternatives Record Record Ask why this plan fits your circumstances.
Outcome denominator and period Record Record Match HFEA measure, treatment and patient group.
Consent and storage explanation Record Record Ask about withdrawal, death, separation and fees.
Counselling access Record Record Check timing and independence.
OHSS and emergency plan Record Record Obtain out-of-hours contact.
Fully costed total Record Record Include medicine, scans, storage and likely extras.
Waiting time and practical access Record Record Check monitoring, travel and opening hours.
Complaints policy Record Record Note contact, deadline and escalation.

Warning signs during comparison

  • A clinic quotes a high rate without naming the denominator, treatment, patient group or time period.
  • A group-wide result is presented without identifying the site that would treat you.
  • The price is advertised as a package but medication, scans, storage or likely procedures are not explained.
  • You are asked to sign consent immediately or cannot obtain a copy of the form.
  • An add-on is described as essential without evidence, uncertainty or a separate price.
  • No one can explain who answers urgent questions overnight or at weekends.
  • The complaints policy is hidden, vague, unusually short or framed as a reason not to escalate.
  • Staff give conflicting answers about storage, withdrawal, parenthood or who holds responsibility for laboratory work.

Questions to take to your consultation

Consent

  • Which decisions am I consenting to now, and which need separate consent?
  • What happens to stored eggs, sperm or embryos if I die, separate from a partner or change my plans?
  • How and when can I withdraw or change consent, and what treatment consequences might follow?
  • What counselling is available, and can I speak with someone independent of the treatment team?

Risks and outcomes

  • What benefits and risks apply to my diagnosis, age and previous treatment?
  • Which symptoms require urgent contact, and who is available outside office hours?
  • How do you assess and manage OHSS risk?
  • Which HFEA measure are you quoting, and what denominator does it use?
  • If you recommend an add-on, what evidence, uncertainty, expected benefit and total cost should I understand?

Cost, access and service

  • Can you list every expected fee, medicine, scan and procedure in a personalised plan?
  • What is excluded from the advertised package, and which extras are likely for me?
  • What are the expected waits, appointment hours and arrangements for urgent questions?
  • Who explains changes and sends results or treatment updates?

Complaints and safety records

  • Where can I read the latest inspection report and licensing decision?
  • What is the written complaints policy, who responds and how can I escalate?
  • Who handles a complaint about treatment, billing, advertising or an individual doctor?
  • How does the clinic explain and learn from incidents or mistakes?

Making the final decision

Choose the clinic whose evidence best matches your circumstances and whose explanations you can understand, not the clinic with the most dramatic headline percentage. A sound decision has a verified licence, a treatment plan justified for you, transparent consent and storage arrangements, a credible safety response, a complete cost estimate, workable access and a complaints process you can use. If a material question remains unanswered, pause and obtain clarification or a second opinion before signing consent or paying for an add-on.
