Start with Medicare Care Compare, but do not choose a North Dakota nursing home from its overall star rating alone. Build a short list, compare the separate staffing, health-inspection, and quality ratings, open the underlying findings and dates, then verify what you see through a visit, North Dakota Health and Human Services (HHS) survey information, and the Long-Term Care Ombudsman. The goal is not to find a facility with the highest single score; it is to determine which home can safely meet one person’s clinical, cognitive, rehabilitation, language, and daily-living needs.
What the Medicare ratings actually tell you
Medicare Care Compare presents one overall five-star rating and separate ratings for health inspections, staffing, and quality measures. Those categories answer different questions. The overall rating is a useful screening signal, while the separate categories show where a facility is strong or weak. A home can look acceptable overall while having a staffing or inspection history that deserves closer attention.
The health-inspection rating is a summary of more than one event. Medicare incorporates the current standard inspection, prior inspections, complaint inspections, and infection-control inspections. Because several years and inspection types can be combined, the star is not a substitute for reading the individual reports. Open each finding, note its date and circumstances, and ask whether the cited problem was corrected and how the facility prevents a recurrence.
The staffing rating also combines several measures rather than a single head-count. It includes registered-nurse (RN) hours per resident day, total nurse staffing hours per resident day, weekend staffing, nurse turnover, and administrator departures. Higher staffing ratings generally indicate more nursing time and less turnover in the measures shown, but they still do not describe the exact team working a particular resident’s shift.

Quality measures provide another perspective on outcomes and processes. Use them to identify questions for the nursing director and to check whether the home fits the prospective resident’s condition. Ratings should be considered with other information, not treated as a guarantee of a specific experience.
Build a defensible North Dakota shortlist
Use the same process for every facility so that a polished website or a single impressive visit does not dominate your decision.
- Define the resident’s needs first. Write down diagnoses, mobility and transfer assistance, medication complexity, wound or diabetes care, oxygen or other treatments, dementia-related behaviors, rehabilitation goals, preferred language, diet, and the level of help needed with bathing, toileting, and eating. These requirements determine which staffing and service questions matter most.
- Search Medicare Care Compare by location. Search the city, county, or ZIP code and include nearby communities if transportation or family visits make that practical. Record each facility’s overall, health-inspection, staffing, and quality-measure ratings as they appear on the current profile.
- Open the category details. Do not copy only the overall star. Save the inspection dates and findings, staffing measures, turnover information, and quality-measure definitions for each candidate. Facility-page data can change, so record the date you checked it.
- Remove obvious mismatches. Confirm that the home accepts the resident’s payment source, provides the required level of nursing or rehabilitation, and can manage any specialized treatment. A high rating cannot compensate for a service the facility does not offer.
- Arrange more than one visit when possible. A daytime tour may not show nights, weekends, meal service, or shift-change conditions. Ask for a second visit at a different time or, if that is impossible, ask specifically how those periods are staffed and supervised.
Compare staffing beyond the star
Read every staffing measure
| Measure | What it indicates | Question to ask the facility |
|---|---|---|
| RN hours per resident day | Average RN time reported for residents, adjusted by Medicare’s methodology. | How many RNs are scheduled on days, evenings, nights, and weekends, and who is available when the RN is off the unit? |
| Total nurse staffing hours per resident day | Average hours from the nursing staff categories included in Medicare’s measure. | Which licensed nurses and nurse aides are included, and how does the schedule change for high-acuity residents? |
| Weekend staffing | Whether reported nurse coverage is maintained on weekends rather than only on weekdays. | What is the usual weekend assignment, and how are call-outs covered? |
| Nurse turnover | How often nursing employees leave during the period measured. | What has turnover been over the past year, and how many current staff members have worked here for at least a year? |
| Administrator departures | Leadership stability in the measure period. | Who is the current administrator, how long have they served, and how are clinical decisions communicated during a transition? |
Medicare cautions that hours-per-resident-day figures are averages. They do not necessarily show how many people are physically present at a particular time or how much care one resident receives. Reported staffing can include nurses who perform administrative duties, and the figures are case-mix adjusted. Ask for the actual assignment pattern, not just the posted average.
Match coverage to the person’s workload
There is no universally correct staffing-hours number. Federal law requires enough staff to care safely for residents, but there is no current federal standard that establishes one best nursing-home staffing level for every resident. A person who needs two-person transfers, frequent toileting, complex skin care, or close behavioral supervision may require a different practical level of coverage than someone who is largely independent.

Check absences, agency use, and continuity
Ask how the home handles sick calls, vacancies, vacations, and unexpected admissions. Find out whether overtime, agency nurses, floating aides, or supervisors fill gaps, and whether those replacements know the residents’ care plans. Ask how often assignments change and whether a consistent team is built around each neighborhood or hall. Frequent changes do not automatically prove unsafe care, but they increase the importance of clear handoffs and supervision.
Read inspection findings as safety evidence
Separate the inspection types
Review the standard survey together with complaint and infection-control inspections. A complaint investigation may concern one resident or one event, while a standard survey examines broader systems. Infection-control findings can reveal problems with hand hygiene, personal protective equipment, isolation practices, or outbreak response. The type of inspection helps you understand what the citation can and cannot show.
Record the details behind each citation
- Date: Note when the problem was observed and whether it is recent or several years old.
- Deficiency: Record the rule or care practice that was not met, using the report’s description rather than a shortened label.
- Harm and scope: Determine whether the report describes actual harm, potential for harm, how many residents were affected, and whether the issue was isolated or systemic.
- Correction: Look for the facility’s plan of correction, follow-up survey, or other evidence that the problem was addressed.
- Pattern: Check whether similar findings recur across different survey cycles or complaint investigations.
A citation count alone is not a severity scale. One serious finding can matter more than several minor paperwork deficiencies, and an old citation may not describe current conditions. Bring the report to the facility and ask what changed, who monitors the fix, and how residents and families are notified if the problem returns.
Use the survey report, not just a dashboard
North Dakota HHS’s Health Facilities Unit conducts the Medicare and Medicaid certification survey work for the federal Centers for Medicare & Medicaid Services. Its materials include routes to skilled nursing facility survey results and a guide titled How to read your Skilled Nursing Facility Survey Report (2567). Use that guide to understand the report’s terminology and to verify the facility’s explanation against the state’s documented findings.
Assess quality and resident fit
Quality measures are most useful when tied to the resident’s priorities. Review measures relevant to falls, pressure injuries, rehospitalization, vaccination, medication use, mobility, and other conditions shown on the current facility profile. Check the definition and time period for each measure; a percentage without its measure description can be misleading.

- Shows on the go: Take your TV to-go when traveling—without needing to log into someone else’s device.
Then ask whether the home can deliver the services the resident actually needs:
- Can it manage the resident’s diagnoses, medications, treatments, and emergency escalation plan?
- Is physical, occupational, or speech therapy available at the frequency ordered?
- How are dementia care, wandering risk, behavioral symptoms, and meaningful activities handled?
- Can staff communicate in the resident’s preferred language and accommodate cultural or religious practices?
- How are diets, allergies, swallowing precautions, and weight changes monitored?
- What is the process for updating the care plan when the resident’s condition changes?
Ask to see a sample care-plan meeting process, not another marketing brochure. The facility should be able to explain who attends, how family input is recorded, and how staff on every shift learn about changes.
Use North Dakota’s state and ombudsman resources
Medicare’s provider-data guidance recommends supplementing Care Compare with the State Survey Agency, the Long-Term Care Ombudsman, and other local information. North Dakota HHS links to the state Long-Term Care Ombudsman program. An ombudsman can provide a resident-focused perspective on concerns, complaint processes, rights, and communication with a facility.
Contact the ombudsman before deciding if you need help understanding a repeated complaint, a difficult response from management, or a resident-rights question. Ask what information can be shared publicly and what must remain confidential. Treat the ombudsman as an independent information source, not as a substitute for reading the facility’s current survey records or visiting.
Make the visit a structured safety check
CMS advises consumers to use the comparison website together with a nursing-home visit and state or local organizations, including the State Ombudsman program. A visit cannot prove that every shift operates well, but it can reveal whether the facility’s answers match what you observe.

Observe before asking
- Do residents appear clean, appropriately dressed, hydrated, and comfortable?
- Are call lights answered promptly, and do staff acknowledge residents by name?
- Are hallways free of trip hazards, strong odors, overflowing trash, and blocked exits?
- Are hand hygiene and infection-control practices visible between resident contacts?
- Do staff explain what they are doing before transfers, bathing, medications, or treatments?
- Are meals delivered at safe temperatures, and do residents receive help when they need it?
- Do noise, lighting, privacy, and activities fit the residents’ needs?
Ask leadership for operational answers
Speak with the administrator, director of nursing, and, when possible, a charge nurse. Ask them to walk through a real example: a resident falls at night, develops a fever, refuses medication, or needs an urgent hospital transfer. Who is called, how quickly is the family notified, and how is the care plan changed? Ask the same question to different staff members; consistent answers suggest that procedures are understood beyond the marketing or admissions team.
Ask residents and families respectfully
If residents or visiting families are willing to talk, ask whether staff respond when help is requested, whether meals and activities are dependable, whether changes in condition are communicated, and whether staff know personal preferences. Do not press anyone to disclose private medical information. One conversation is anecdotal, but several consistent accounts can identify issues worth investigating.

Decision rules for weighing trade-offs
Give safety and capability priority
Eliminate a home that cannot provide a required service, has no credible plan for a serious unresolved safety issue, or refuses to explain current inspection findings. Distance, room appearance, and amenities matter only after basic safety and clinical capability are established.

Investigate disagreement between categories
A high staffing rating with repeated recent inspection findings deserves a detailed explanation, as does a strong inspection rating paired with weak staffing and high turnover. The mismatch may have a legitimate explanation, such as timing or a resolved event, but the facility should be able to show what changed and how it is monitored.
Prefer transparent, specific answers
Choose the facility that can explain assignments, escalation, turnover, correction plans, and care-plan communication in concrete terms. Vague promises, pressure to sign quickly, or an unwillingness to provide survey information are reasons to slow down and consult the state or ombudsman.
Common comparison mistakes
- Choosing the highest overall star: The overall score hides differences among inspections, staffing, and quality measures.
- Treating staffing averages as a shift schedule: Averages do not show who is present at a particular time or how much care one resident receives.
- Counting citations without reading them: Date, harm, scope, recurrence, and correction determine meaning.
- Using an old screenshot: Ratings and facility data can change; check the current profile and record when it was viewed.
- Touring only at the most convenient time: Nights, weekends, meals, and shift changes can look different.
- Accepting “we meet the standard” as a complete answer: Ask how the home’s actual assignments meet this resident’s needs.
- Ignoring fit: A facility may be safe in general but unable to provide a needed therapy, language accommodation, behavioral-support approach, or treatment.
- Failing to close the loop: Write down unanswered questions, request documents or names, and follow up before signing an admission agreement.
Verify the choice before admission
After selecting a preferred home, repeat the highest-risk checks immediately before admission. Confirm the current administrator and director of nursing, the services available on the intended unit, the expected staffing pattern for the resident’s needs, medication and treatment arrangements, therapy schedule, visiting rules, and the process for reporting a change in condition. Ask when the initial care-plan meeting will occur and how family members will receive updates.
Keep the Care Compare record, survey reports, notes from visits, and written answers together. If the resident’s condition changes, rerun the comparison rather than assuming the original choice remains appropriate. A nursing-home decision is an ongoing safety assessment, not a one-time star-rating exercise.
