FUT and FUE describe how donor follicles are removed; implantation is a separate surgical step. “DHI” is not a distinct transplant method according to the International Society of Hair Restoration Surgery (ISHRS), but a term some practitioners use for an implantation approach. The right choice depends on your diagnosis, donor supply, hair-loss pattern, hairstyle, long-term plan, and the surgical team’s expertise—not on a technique label alone. The practical comparison is therefore: what will be harvested, how will it be implanted, what scar pattern and shaving are involved, what is included in the quote, and how will the plan hold up if hair loss continues? NHS guidance and the ISHRS patient guide provide the clearest baseline for those questions.

What you are actually comparing

A hair transplant has two main surgical phases:

  • Harvesting: removing hair-bearing follicular units from the donor area, usually at the back or sides of the scalp.
  • Recipient-site creation and implantation: preparing tiny sites in thinning areas and placing the grafts so the angle, direction, density, and hairline look natural.

FUT (follicular unit transplantation, also called strip harvesting) and FUE (follicular unit excision) primarily describe the first phase. The same grafts still have to be prepared and placed. Implantation may use forceps, implanter devices, or other instruments, so a clinic’s use of a term such as “DHI” does not by itself tell you how the donor area will be harvested or prove a better result.

ISHRS states that “In either case DHI is not a hair transplant method and should not be marketed as such.” Ask a clinic to describe each surgical step in plain language rather than accepting a package name.

FUT, FUE and DHI terminology side by side

Comparison point FUT (strip harvesting) FUE (follicular unit excision) “DHI” terminology
What is removed A strip of hair-bearing scalp is excised and divided into individual grafts. Individual follicular-unit grafts are removed one at a time with small punches. Not a separate donor-harvesting method according to ISHRS.
How grafts are implanted Grafts are placed into small recipient cuts or sites. Grafts are placed into recipient sites; the implantation instrument can vary. May refer to immediate implantation after extraction or use of a sharp implanter, depending on the practitioner.
Scar pattern One linear scar at the strip site, closed with sutures or another closure technique. Many small, scattered punch scars; FUE is not scar-free. Scar pattern depends on the actual harvesting method.
Shaving The whole head need not be shaved; the strip area may be trimmed, according to the NHS. The NHS describes shaving the back of the head. Claims about no-shave procedures are not universal clinical rules.
Main planning question Would a linear scar work with your hairstyle and future procedures, and is strip harvesting suitable for your donor area? Is the donor area appropriate, and can extraction be performed without excessive thinning or visible punch scarring? What exactly does the clinic mean, and who performs extraction, site creation, and placement?

The 2024 ISHRS FUT clinical-practice guideline notes that technique and instruments should match the surgeon’s experience, the team’s expertise, and individual patient characteristics. It also describes strip harvesting as potentially efficient for obtaining grafts over a patient’s lifetime, while still requiring careful consideration of suitability and the linear scar. That is a planning trade-off, not a universal recommendation.

How FUT works, and when its trade-offs matter

The procedure

With FUT, the surgeon removes a narrow strip from the donor scalp. The strip is dissected under magnification into follicular-unit grafts, while the donor wound is closed. Recipient sites are then created in the thinning area and the grafts are inserted.

Potential advantages

  • The donor hair is collected as a strip rather than by making hundreds or thousands of individual punch entries.
  • A strip session can be efficient when a substantial number of grafts is needed and the patient is a suitable candidate.
  • The rest of the head does not necessarily need to be shaved; the strip zone may be trimmed.

Important limitations

  • The scar is linear. Its final appearance depends on closure, healing, scalp laxity, hair characteristics, and future procedures.
  • Very short hairstyles may make a linear scar easier to see than longer hair would.
  • Ask how the clinic plans to protect the donor area if you might need another transplant later.

FUT is not automatically “old-fashioned,” and it is not automatically better for high graft numbers. The relevant question is whether the surgeon can obtain the needed grafts while preserving donor density and creating a scar you can accept.

How FUE works, and what “scarless” claims leave out

The procedure

With FUE, the surgeon uses a small punch to separate and remove individual follicular units from the donor area. The grafts are inspected and prepared, recipient sites are made, and the grafts are placed.

Potential advantages

  • There is no single strip scar.
  • Scarring is distributed as many small marks, which may suit people who want to wear shorter hairstyles.
  • The method can be useful when a linear scar is undesirable, provided the donor area and extraction plan are appropriate.

Important limitations

  • FUE still creates scars—small punch scars rather than one line.
  • Overharvesting can produce visible donor thinning, patchiness, or a moth-eaten appearance.
  • Extraction quality depends heavily on the surgeon’s judgment, punch selection, hair angle, graft handling, and the team’s training.
  • The NHS describes shaving the back of the head for FUE; ask exactly how much will be shaved and whether a limited-shave plan is clinically appropriate.

A promise of “no scars” should prompt a precise explanation of where the punches will be placed, how many grafts are planned, and what the donor area may look like in short hair.

What clinics mean by DHI

DHI is commonly presented as a named technique, but ISHRS classifies it as terminology for an implantation approach rather than a distinct transplant method. One practitioner may use it to mean placing a graft immediately after extraction; another may mean loading grafts into a sharp implanter device. Neither description tells you whether the donor hair was harvested by FUT or FUE.

Ask these four questions:

  1. Is the donor hair being removed by strip harvesting or individual punches?
  2. Who makes the recipient sites?
  3. Who loads and places each graft?
  4. What evidence supports any claim about survival, density, pain, shaving, or faster recovery?

Marketing pages may advertise shorter recovery windows or higher survival figures, but those claims should not be treated as general clinical facts without independent comparative evidence. Compare the actual operation, the surgeon’s role, and the follow-up plan—not the label.

How surgeons decide which approach fits

The Aesthetic Society says selection depends on the balding or thinning area and the desired look. The evaluation includes hair quality, the pattern of loss, and the donor location. A responsible consultation should also address:

  • Diagnosis: Hair loss should be identified before surgery; active or unusual loss may need medical evaluation.
  • Donor density and quality: Fine, curly, tightly angled, or already depleted donor hair can change extraction risk and graft estimates.
  • Extent and pattern of loss: A large area may require staged procedures rather than one attempt to cover everything.
  • Hairstyle: Your usual length affects how a linear scar or scattered punch scars may appear.
  • Future loss: Native hair around transplanted grafts can continue to thin. The hairline and density should be designed for a changing scalp, not only today’s photographs.
  • Team capability: The plan should reflect who actually performs extraction, recipient-site creation, graft handling, and implantation.

There is no universally best technique in the reviewed guidance. A surgeon should explain why the proposed method fits your anatomy and long-term goals, and what alternative would be considered if the first option were unsuitable.

Cost: what the published figure does—and does not—tell you

NHS guidance says: “A hair transplant in the UK can cost anywhere between £1,000 and £30,000, depending on the extent of hair loss, the type of procedure you have, and the quality of the clinic and its team.” The page was last reviewed on 29 September 2023. This is a broad UK patient-guidance range, not a current quotation, a worldwide average, or a reliable way to compare FUT, FUE, and DHI.

Cost question Why it changes the total What to request in writing
How much hair needs treatment? Larger areas and higher density targets can require more grafts or more than one session. The estimated graft range, the area to be treated, and the assumptions behind the estimate.
Which harvesting method? Operating time, staffing, instruments, and surgeon involvement can differ. The named procedure and a plain-language description of harvesting and implantation.
Who performs each step? Surgeon-led care and delegation to other staff have different safety and accountability implications. The surgeon’s identity and exactly which steps the surgeon will perform.
What is included? Consultations, medication, aftercare, follow-up, and revision policies may be handled separately. An itemized quote covering the operation, follow-up visits, aftercare instructions, and any stated additional charges.
Could you need another session? The NHS notes that multiple sessions may be needed for a large area; the Aesthetic Society describes plans that can span one to two years. Whether the quote is for one session only and how future sessions would be assessed and priced.

Do not infer that FUE is always cheaper than FUT, or that a DHI-labelled package is worth more. The reviewed sources do not establish an apples-to-apples price comparison or a current international average. Compare the scope of care, graft estimate, surgeon involvement, and follow-up before comparing numbers.

Recovery: separate the milestones

“Recovery time” can mean several different things: time away from work, the period when grafts need protection, visible scabbing, return to exercise, early shedding, first regrowth, or the point at which the final result can be judged. These milestones do not occur on the same day.

Milestone General NHS guidepost How to use it when planning
Time off work Some patients may need 1–2 weeks. Plan around swelling, scabs, discomfort, the visibility of the donor area, and the physical demands of your job.
Graft protection Grafts are not secure during the first 2 weeks. Follow the clinic’s instructions for sleeping, touching, headwear, washing, and activity.
Gentle washing Gentle hand washing may begin from day 6. Use the treating surgeon’s exact method; do not substitute a generic online routine.
Early symptoms Temporary tightness, aching, swelling, and scabbing are common. Ask which symptoms are expected and which require an urgent call.
First new hair New hair usually starts appearing after about 4 months. Early photographs may show little visible change; do not judge the final density at this stage.
Full result The NHS says full results should be visible at 10–18 months. Schedule meaningful outcome assessment only after the time your surgeon considers appropriate.

Individual healing varies, and your surgeon’s aftercare instructions take priority. A clinic that promises a universally “instant” result or a fixed recovery period is oversimplifying a process that includes wound healing, possible shedding, gradual growth, and maturation.

A practical comparison process before you book

1. Confirm the diagnosis and the goal

Write down what you want to improve—hairline, crown, temple recession, diffuse thinning, or camouflage of a scar—and ask what diagnosis explains the loss. A transplant redistributes existing donor hair; it does not create an unlimited supply.

2. Get a donor-area assessment

Ask for an estimate of available donor grafts, the safe extraction zone, expected density, and how the donor area would be preserved for future loss. Request photographs or diagrams that show both the recipient plan and the donor design.

3. Demand a technique-specific explanation

Have the clinic state whether harvesting is FUT or FUE, what “DHI” means in that clinic, which instruments are used, and who performs each step. If the explanation relies on a brand name instead of surgical details, pause before paying a deposit.

4. Compare complete written plans

Place quotations side by side. Check graft estimate, number of sessions, surgeon involvement, anaesthesia, medications, aftercare, follow-up, treatment of complications, and revision terms. A lower headline price may omit important care.

5. Check credentials and accountability

Ask about the surgeon’s qualifications, experience, number of procedures, complications, follow-up arrangements, and patient-satisfaction data. ISHRS advises consumers to confirm that the doctor—not an unlicensed technician—will perform the surgery.

6. Verify local regulation

In England, the NHS advises checking that the hospital or clinic is registered with the Care Quality Commission (CQC), and that doctors are registered with the General Medical Council (GMC) and licensed to practise. Outside England, use the equivalent regulator and licensing register in your country; the England-specific checks do not automatically apply elsewhere.

7. Plan for continuing hair loss

Ask how the hairline and density will look if surrounding native hair thins. A long-term plan may involve medical treatment, conservative density, staged surgery, or accepting a smaller area of coverage. The NHS specifically advises planning so the transplant continues to look natural as time passes.

Questions that expose weak consultations

  • What diagnosis explains my loss, and is surgery appropriate now?
  • Why do you recommend FUT or FUE for my donor area and hairstyle?
  • What exactly does your clinic mean by DHI?
  • How many grafts are planned, and what is the safe donor limit?
  • Will my head be shaved, and which areas?
  • What scar pattern should I expect, and how might it look with my usual haircut?
  • Who performs extraction, recipient-site creation, graft preparation, and implantation?
  • What complications do you see, and how are they managed?
  • When can I wash, exercise, wear headwear, and return to work?
  • What happens if native hair continues to thin or the result is inadequate?
  • What follow-up is included, and who do I contact outside office hours?
  • Can I speak with patients whose hair type, loss pattern, and procedure are similar to mine?

Warning signs and common comparison mistakes

  • Choosing by label alone: “DHI,” “no scar,” or “maximum density” does not describe the full operation.
  • Calling FUE scar-free: Individual punches leave scars, and excessive extraction can make them conspicuous.
  • Comparing prices without scope: A graft estimate, surgeon role, aftercare, and revision policy may differ between quotes.
  • Believing a single recovery number: Returning to desk work is not the same as graft security or final growth.
  • Ignoring donor limits: Overharvesting can compromise future options, regardless of the technique name.
  • Planning only the present hairline: Continuing native-hair loss can make an initially dense transplant look unnatural later.
  • Accepting technician-led surgery without clarity: You should know which licensed professional is responsible for each surgical act.
  • Assuming a quote is current everywhere: The NHS range is UK guidance reviewed in 2023 and should not be used as an international benchmark in 2026.

How to interpret results over time

Do not compare clinics using one early photograph or a promised graft-survival percentage. A fair assessment considers the hairline design, direction and angle, density relative to the donor supply, donor-area appearance, ongoing native-hair loss, and the time elapsed since surgery. The NHS guideposts—first new hair around four months and full results at 10–18 months—explain why a result can look incomplete for much of the first year.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Ask how the clinic documents outcomes and complications, and whether examples show comparable hair texture, curl, colour contrast, loss pattern, and lighting. A before-and-after image is useful only when the conditions and timing are clear.

Bottom line: compare the plan, not the slogan

FUT and FUE are different donor-harvesting strategies, each with a distinct scar pattern and practical trade-offs. DHI is a contested marketing label for implantation approaches, not a third harvesting method. Cost depends on location, extent of loss, procedure, clinic, team, and the scope of care; the only broad published figure here is the NHS’s UK range of £1,000–£30,000, reviewed 29 September 2023. Recovery should be planned in stages: potentially 1–2 weeks away from work, two weeks of graft protection, early growth from about four months, and final assessment at 10–18 months. Choose only after a qualified surgeon explains the diagnosis, donor limits, technique, risks, long-term design, and follow-up in writing.
