Elisabeth Sparkle is an aging public figure who accepts a treatment that creates a younger version of herself, Sue. That rule makes the premise more than a simple wish to look young. Elisabeth and Sue are not independent strangers; they are two versions of one identity, forced to manage a shared life on a schedule. The arrangement turns self-improvement into surveillance and competition: each week makes the other version’s existence harder to ignore.
Why does the film reach beyond horror?
Fargeat has described the shift in her work as a move from external judgment to internalized judgment. Speaking to the Sitges Film Festival, she said that in Revenge perceptions come from external sources—men—while in The Substance, “it’s a matter of self-image.” The film’s horror can therefore be read as an escalation of a familiar pressure: being evaluated by others until their standards become part of how you evaluate yourself. Fargeat put the central idea bluntly in a September 2024 Vogue interview: “The movie is fundamentally about the violence of control.” The imagined treatment is not presented as a real medication or a one-to-one warning about any particular drug. Fargeat said she wrote the script about five years before that interview, before Ozempic became prominent, and linked its premise to a longer history of appetite-suppression and weight-loss products. Why was Demi Moore the right choice for Elisabeth?
Fargeat said the role called for an iconic actress associated with public scrutiny, and that she wanted Moore for that reason. Casting Moore gives the film’s concern with aging and visibility a particular resonance: the viewer sees a performer whose career has unfolded in public inhabit a character whose public identity is bound up with being looked at. That is a casting rationale Fargeat has stated, not evidence of Moore’s private motivation for accepting the role.

Moore told Variety that body horror was outside her usual familiarity and that the project could have worked or become a disaster. She also said the experience gave her greater appreciation for herself as she is. Describing her career, she said: “I’m at a point where I’m writing my own story, as opposed to my story being dictated to me based on my age.” Her comments connect the role to agency without suggesting that the film offers an easy resolution to the pressures it depicts. The film uses body horror, prosthetics, and nudity, but Fargeat has described those choices as part of the character’s vulnerability and the difference between being alone in one’s body and being seen through a camera’s gaze. Where was the film made, and what recognition did it receive?
Although the story evokes Los Angeles, the French CNC says the American-British production was shot entirely in France, in a Paris Region studio and on locations on the Côte d’Azur; production recreated Los Angeles in the Alpes-Maritimes.
