In “When I Crash,” the November 13, 2025 fall finale, Jo Wilson, played by Camilla Luddington, faced serious heart complications during her pregnancy. The story moved toward a risky procedure intended to give her twins more time before birth, but the episode ended with an emergency C-section. At the time, the episode left Jo and the babies’ survival uncertain. TheWrap’s coverage and interview described the crisis and the cliffhanger.

What did Camilla Luddington mean by “brutal”?

Speaking to TheWrap on November 13, Luddington warned viewers about the next episode after the fall finale: “If there’s one word to sit on it would be ‘brutal.’ Emotionally prepare because the next episode coming up after this, it is worse, to be honest.” Her comment was a preview of the January midseason return, not a statement that Jo’s outcome would remain unresolved.

Did Jo and the twins survive?

The series returned with its midseason premiere on January 8, 2026, and the episode addressed the emergency C-section cliffhanger and the fate of Jo and the twins. E! News reported on the premiere’s answer; later, Parade described Jo as having survived heart failure during pregnancy and the emergency C-section.

Where did Season 22 go after the return?

Jo’s crisis was one part of a continuing season, rather than the season’s final event. By March 13, 2026, Season 22 had reached Episode 13, “Love the Way You Lie,” according to Shondaland’s official recap. That later episode confirms the season had progressed well beyond the January cliffhanger; it does not, by itself, establish the season’s ultimate ending or any Season 23 plans.
