The announcement said recent performances worsened his condition and were followed by hearing, vision and balance problems. Those statements describe Joel’s publicly disclosed experience; they do not establish that every symptom is caused by NPH or reveal his current medical status.
What brain condition does Billy Joel have?
Joel’s representatives announced on May 23, 2025, that doctors had diagnosed him with normal pressure hydrocephalus, commonly abbreviated NPH. His team said that performing had exacerbated the condition and led to hearing, vision and balance problems. Doctors advised him to stop performing while he underwent physical therapy, and scheduled appearances were canceled, according to the Associated Press.
That is the confirmed public disclosure about his diagnosis. It is not appropriate to infer additional diagnoses, severity or prognosis from the announcement.
What is normal pressure hydrocephalus?
Normal pressure hydrocephalus is a condition in which cerebrospinal fluid (CSF) builds up in the brain’s ventricles, causing them to enlarge. Despite the name, pressure readings are not necessarily persistently high. CSF surrounds and circulates through the brain and spinal cord; an imbalance in its production, flow or absorption can contribute to ventricular enlargement.

Barrow Neurological Institute’s 2025 patient handbook describes three commonly associated symptom groups:
- Walking and balance: short, shuffling or unsteady steps, difficulty turning, falls or a feeling that the feet are stuck.
- Cognition: slowed thinking, reduced attention or changes in short-term memory.
- Bladder control: urgency, frequency or incontinence.
Columbia neurosurgeon Guy McKhann has emphasized gait and balance changes as particularly characteristic, often occurring with variable memory and bladder symptoms. These patterns can also occur with aging, medication effects, stroke, Parkinson’s disease, Alzheimer’s disease and other conditions. A symptom checklist cannot diagnose NPH.

How NPH is evaluated
There is no single test that proves someone has NPH. Northwestern neurological surgery expert Matthew Potts put it this way: “The important thing is getting the right diagnosis,” and “There is not one test that says ‘Yes, this is NPH.’” Clinicians combine the history, examination and test results to decide whether the overall picture fits.
Clinical assessment
A specialist typically documents the timing and pattern of walking, thinking and bladder changes, reviews other illnesses and medicines, and looks for alternative explanations. A person’s functional change over time matters as much as any isolated symptom.

Brain imaging
CT or MRI scans may show enlarged ventricles or other features that support an NPH evaluation. Enlarged ventricles alone do not establish the diagnosis, because they can occur for other reasons.
Temporary CSF drainage
Some evaluations include a lumbar puncture (“spinal tap”) that removes CSF temporarily. Clinicians then compare walking, balance or other functions before and after drainage. Improvement can support the case for treatment, but the test is interpreted alongside the rest of the evidence rather than used as an automatic yes-or-no answer.

Is there treatment for NPH?
For selected patients, doctors may recommend a shunt. This implanted system diverts CSF from the brain’s ventricles to another part of the body, commonly the abdomen, where the fluid can be absorbed. Shunt designs and valve settings vary, and a neurosurgical team weighs likely benefit against complications and the person’s other medical conditions.
What may improve
Response is variable. Columbia’s clinical explanation notes that walking and balance often improve more clearly than bladder or cognitive symptoms, although individual outcomes differ. Treatment cannot reverse decline caused by a separate disorder such as Alzheimer’s or Parkinson’s disease, and a shunt is not a guarantee of complete recovery.

Risks and follow-up
Shunt surgery carries risks, including infection, bleeding, blockage, over-drainage or under-drainage, and the possibility of needing further procedures. People who receive a shunt require specialist follow-up to assess symptoms and device function. These decisions are individualized medical care, not consumer treatment choices.
What Billy Joel has said about his health
| Date | Public information |
|---|---|
| May 23, 2025 | Joel’s team announced an NPH diagnosis, said performances had worsened his condition and reported hearing, vision and balance problems. The team also said he would stop performing while undergoing physical therapy. |
| July 2025 | In interviews reported by the Associated Press, Joel said, “My balance sucks. It’s like being on a boat,” and also said, “I know a lot of people are worried about me and my health, but I’m OK.” He said the condition was still being worked on. |
| September 18, 2026 | Publicnow distributed a press release reporting that Joel had undergone brain surgery after his NPH diagnosis. That account was not independently corroborated by a direct statement or medical source published by September 2026. |
Joel’s July comments are dated personal remarks, not a clinical prognosis. They do not establish his present condition, whether treatment worked or what care he received.
What can—and cannot—be concluded about the reported 2026 surgery
The Publicnow-distributed release is a report, not independently confirmed evidence of a specific procedure. It does not justify claims about the type of surgery, its reason, complications, recovery or current outlook. In particular, readers should not assume that the report proves Joel received a shunt, even though shunt surgery is one treatment used for some people with NPH.
When symptoms warrant medical evaluation
New or worsening trouble walking, repeated falls, changes in thinking or loss of bladder control deserve evaluation by a qualified clinician. Those symptoms have many possible causes, some urgent and treatable. A clinician can determine whether brain imaging, neurological consultation or a CSF-drainage assessment is appropriate.
Information about NPH does not diagnose Billy Joel or any other individual. Only his treating professionals can assess his condition and prognosis.
