The episode’s most painful revelation is not simply that Joan once told Amy to leave Michael. It is that Amy is beginning to remember Joan as a consequential voice in the collapse of her marriage—just as Joan returns to Westside with new authority over her.
What Amy remembers about Joan
“Delusions of Grandeur,” which aired September 30, 2025, centers its emotional turn on the memory Amy lost after her car accident. In the final scene, fragments of a dinner from five years earlier come back: Joan told Amy she had no choice, and advised her to leave Michael. According to TV Insider’s recap, Joan argued that Michael held Amy back and that Amy saw him as the man who let their son die. (TV Insider)
When Amy asks Joan about the memory, Joan responds that they had many dinners and choices, and says they will think about it. The exchange leaves Amy with an unsettling question rather than a complete explanation: what did Joan say, and how much did it matter?
The distinction matters. The episode reveals Joan’s counsel, not proof that she alone ended Amy and Michael’s marriage. Michael’s account also points to a relationship already failing. Joan’s advice changes the story’s emotional map, but it does not erase the couple’s own grief and choices.

Why Joan’s new role makes the memory sting
Joan is not only a figure from Amy’s past. She returns to Westside as Amy’s mentor and takes the position of chief of internal medicine. That combination gives their reunion a double edge: Amy must navigate the memory of Joan’s influence while working with her in a setting where Joan now has institutional authority. The episode’s premise and Joan’s appointment are summarized by Rotten Tomatoes. (Rotten Tomatoes)
That makes the recovered dinner more than a piece of backstory. Amy is trying to rebuild her own sense of the past, but the person who may help her interpret it is also implicated in it. Joan’s response does not settle whether she acted out of care, certainty, or something more complicated; the unresolved tension is the point.

Megan’s case echoes the personal story
The medical storyline follows Megan, a woman who believes she is pregnant but learns she has a tumor. Amy and Gina wrestle with whether to conceal the truth temporarily in an effort to protect her, as described in Absolute Geeks’ episode recap. (Absolute Geeks) This is a recap of the episode’s drama, not medical guidance.
The case gives the hour a useful thematic counterweight. Amy and Gina must consider whether withholding painful information can ever be an act of kindness, while Amy’s own story turns on what Joan told her—and what Amy can now remember. The parallel is not a neat answer to either dilemma. Instead, the case presses the characters to confront the cost of deciding what another person is ready to hear.

Amy and Gina try to repair what is broken
Megan’s case also brings Amy and Gina’s damaged friendship into view. TVBrittanyF notes that Amy apologizes to Gina, while Absolute Geeks highlights Gina’s image of repairing the friendship with “a thousand stitches.” (TVBrittanyF; Absolute Geeks) The image suits an episode interested in repair as slow, deliberate work rather than a single cathartic conversation.
That thread also prevents Amy’s memory from becoming the only measure of the hour. The episode asks whether relationships can be mended when people have hurt one another, but does not pretend that an apology or a newly recovered memory can instantly restore trust.

Does the slower pace work?
After the season premiere, “Delusions of Grandeur” slows down to spend time with character conflicts. Absolute Geeks rated it 4.1 out of 5 and praised its emotional excavation and interpersonal tension. (Absolute Geeks) That is one outlet’s rating, not a consensus score.
TVBrittanyF provides a useful counterpoint: the conflicts may feel familiar, with Amy again at the center and some characters seeming to slide back into earlier dynamics. (TVBrittanyF) Both reactions fit the episode. Its quieter pace gives the dinner memory room to land, but the return to established relationship tensions may feel less like fresh momentum than renewed emphasis on old patterns.

Other threads—including TJ’s recovery after being shot, Sonya’s criticism of Amy, and Michael’s effort to manage work and family—remain in the background of the reviews’ accounts. They widen the episode’s picture of a hospital and family system under strain, though the recovered memory is its clearest dramatic pivot. (Absolute Geeks)
