Common Hair Transplant Side Effects and When to Contact Your Surgeon

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Tightness, aching, swelling, itching, crusting, scabbing and temporarily reduced sensation are usually part of early hair-transplant healing. They should gradually settle while the grafts are protected. Contact your treating clinic promptly for severe or worsening pain, marked redness, persistent fever, excessive bleeding, green or yellow pus, or any unexpected symptom. Difficulty breathing or another severe systemic reaction requires emergency care.

Which side effects are usually normal?

A transplant creates hundreds or thousands of small wounds in the donor and recipient areas. Local inflammation is therefore expected, especially during the first several days. The exact intensity depends on the procedure, the number of grafts, your healing response and the instructions used by your surgeon.

Effect What it can feel or look like What to do
Tightness and aching A stretched, tender or bruised feeling in the scalp, sometimes with throbbing Use only the pain-control plan supplied by your clinic and avoid rubbing or pressing the grafts
Swelling Localized puffiness around the treated scalp during the first few days Follow your surgeon’s positioning, cooling and activity instructions; report swelling that is severe, rapidly worsening or accompanied by other warning signs
Itching A prickly or itchy sensation as wounds begin to close Do not scratch, pick or lift crusts; ask the clinic what cleansing method is permitted
Crusting or scabbing Small crusts over recipient sites and possible crusting in the donor area Let them loosen according to the prescribed washing routine rather than removing them forcefully
Reduced sensation Numbness or altered feeling in the donor or recipient area Protect numb skin from heat, friction and accidental injury, and mention persistent or worsening sensory change at follow-up
Scars A donor or recipient scar can remain after healing Ask how the selected technique and your individual healing pattern may affect scar visibility

These effects are common early findings, not a guarantee that every patient will experience all of them. A symptom that is mild and improving is generally less concerning than the same symptom becoming stronger after an initial improvement.

Recovery timeline: what happens when?

First few days

Tightness, aching, throbbing, swelling and tenderness are common. Crusts or scabs may form around the implanted grafts and in the donor area. Keep the scalp protected exactly as instructed; the newly placed follicles are vulnerable to friction, pressure and dislodgement.

First two weeks

Grafts are not secure for roughly the first two weeks. Washing, sleeping positions, headwear, exercise and work restrictions vary by clinic, so your surgeon’s written protocol takes priority over general advice. Avoid touching or scratching the recipient area unless a clinician has shown you how.

Around day six

NHS patient guidance says gentle hand washing may begin around day six, but this is not a universal schedule. Some surgeons use a different cleansing method or timing based on graft placement and wound condition. Confirm the exact water pressure, cleanser, fingertip technique and drying method with your treating team.

After a few weeks

The transplanted shafts often shed. This can look alarming, but shedding after a few weeks can be part of the normal cycle after implantation; the follicle can remain in the skin even when the visible hair falls out.

Around four months

New hair commonly starts to appear at about four months. Growth is gradual and uneven, so isolated hairs or thin areas at this stage do not by themselves show that the procedure has failed.

Ten to eighteen months

Full results are commonly visible between 10 and 18 months. The timing differs by person and by treated area. Keep scheduled reviews so the surgeon can assess growth, skin healing and the stability of your existing hair.

When should you contact your surgeon?

Contact the clinic that performed the transplant as soon as possible if you have severe pain or any unexpected symptom. Do not wait for a routine appointment when a symptom is intense, rapidly worsening or accompanied by systemic illness.

Warning sign Why it needs prompt assessment
Severe pain, or pain that is getting worse Disproportionate or escalating pain can indicate a complication rather than routine healing
Severe redness or redness that is spreading or intensifying Marked inflammation may need examination for infection or another wound problem
Fever that does not go away Persistent fever is not an expected minor scalp symptom and should be assessed
Excessive bleeding Bleeding that does not settle with the clinic’s instructed pressure technique requires clinical advice
Green or yellow pus oozing from scalp cuts Colored pus is a recognized sign of infection
Any symptom you did not expect Your surgeon knows the technique, anesthetic and your medical history and can decide whether examination is needed

Difficulty breathing, collapse, confusion or another severe systemic symptom after the procedure warrants emergency services rather than waiting for a callback. Such symptoms can signal a serious reaction, including a problem related to anesthesia or another urgent medical condition.

How to protect grafts while they heal

  1. Use your clinic’s written instructions as the controlling plan. Generic timelines cannot account for graft angle, donor-site treatment, medical conditions or the number of follicles moved.
  2. Wash only as directed. When your surgeon permits cleansing, use the specified water pressure and touch. NHS guidance describes gentle hand washing around day six, but your clinic may alter that timing.
  3. Do not pick crusts or scratch an itch. Mechanical removal can injure healing skin and disturb grafts.
  4. Follow activity restrictions. Ask when exercise, heavy lifting, swimming, helmets and other sources of pressure or contamination can resume.
  5. Protect numb areas. Reduced sensation can make heat, friction or accidental knocks harder to notice.
  6. Use only recommended products. Some patients receive a recovery or hair-growth spray, but the NHS does not specify a universal brand or formula. Use a product only if your treating team tells you to do so.
  7. Keep follow-up appointments. A clinician can distinguish normal crusting or shedding from infection, poor healing or inadequate growth.

How to tell normal shedding from a problem

Visible transplanted hairs commonly fall out after a few weeks. This temporary shedding does not necessarily remove the follicle, which can later produce a new shaft. New growth often begins around month four, while the final visual result may not be apparent until 10–18 months.

Call the clinic if shedding is accompanied by severe pain, worsening redness, persistent fever, pus, heavy bleeding or another unexpected change. Those associated findings, rather than hair loss alone, are the reason for urgent review. Also report a lack of progress at scheduled assessments; growth rates vary, and your surgeon can evaluate whether follicles are developing as expected.

Recognized complications and longer-term outcomes

Hair transplantation is surgery, even when performed in an outpatient setting. Recognized complications include bleeding, infection and an allergic reaction to the anesthetic. Grafts may fail to take, growth may be poor, scars may be noticeable, altered sensation may persist, and the final hairline or density may look unnatural.

The International Society of Hair Restoration Surgery (ISHRS) describes significant complications as extremely rare, but “rare” does not mean impossible. The procedure redistributes existing follicles; it cannot create new follicles. Your achievable density therefore depends on the available donor supply, the pattern of hair loss and the plan agreed with the surgeon.

Persistent numbness, a widening or painful scar, areas that remain inflamed, or a result that appears unnatural should be discussed at follow-up. Do not attempt a corrective procedure before the treating clinician has assessed healing and the maturation of growth.

FUT and FUE: how the technique affects scars and aftercare

The two commonly discussed harvesting approaches have different wound patterns. Neither is automatically safer for every patient; suitability depends on donor characteristics, hair-loss pattern, prior surgery, the number of grafts needed and the surgeon’s skill.

Consideration FUT (strip method) FUE
Donor harvesting A strip of skin containing follicles is removed and the incision is closed with stitches Individual grafts are removed one by one
Scar pattern A linear donor scar Multiple small donor scars
Shaving Shaving requirements depend on the planned incision and clinic protocol Shaving requirements depend on the extraction area and clinic protocol
Sessions The number of sessions depends on donor supply and the treatment plan The number of sessions depends on donor supply and the treatment plan
Recovery instructions Protect both the stitched strip and recipient grafts according to the surgeon’s instructions Protect the many small extraction sites and recipient grafts according to the surgeon’s instructions
Best choice Surgeon-specific; depends on anatomy, goals and previous procedures Surgeon-specific; depends on anatomy, goals and previous procedures

The NHS describes FUT as removing a strip closed with stitches and FUE as removing individual grafts that leave multiple small scars. Ask how the proposed method changes washing, stitch care, activity limits and the appearance of the donor area before consenting.

Choosing a provider reduces avoidable risk

Side-effect management begins before surgery. The NHS recommends asking prospective providers about their qualifications, experience, number of procedures performed, complication history, recommended technique, follow-up arrangements if something goes wrong and patient-satisfaction information.

Also establish who will perform each surgical step. ISHRS warns that when unlicensed personnel perform highly skilled parts of hair-restoration surgery, patients risk misdiagnosis or unnecessary or ill-advised surgery. Ask directly who designs the hairline, administers anesthesia, removes grafts, creates recipient sites and implants follicles, and what supervision applies to each task.

  • Verify the surgeon’s professional registration and relevant surgical training in your jurisdiction.
  • Request a consultation that explains donor limitations, likely density, scar placement and the possibility of future hair loss.
  • Ask for a written aftercare plan with washing, sleeping, exercise, medication and emergency-contact instructions.
  • Confirm who answers calls outside office hours and where urgent assessment takes place.
  • Review representative patient-satisfaction information without treating selected photographs as a guarantee.
  • Discuss prior scalp surgery, allergies, bleeding disorders, medicines and any condition that could affect healing.

What to tell the clinic when you call

Give the procedure date, technique if known, the areas affected and the exact time the symptom began. Describe whether it is improving or worsening, record your temperature if fever is suspected, estimate bleeding, and mention drainage color or odor. Tell the clinician what products or medicines you have used and whether you have already disturbed a crust or graft. Clear details help the team decide whether to adjust home care, arrange same-day review or direct you to emergency services.

Do not conceal an unexpected symptom because you are worried about having caused it. Early advice can prevent minor wound problems from becoming larger ones, and the treating clinic is the team best placed to interpret findings in light of the procedure performed.
