Spoilers for the ending of Alien: Romulus. Kay uses the recovered black substance to treat her injuries, but the film shows the treatment leading to a rapid, horrific birth. The creature—known as the Offspring—kills Kay and attacks Rain and Andy. Rain ejects it into space, then leaves with Andy for Yvaga, hoping to find a way to repair him. Director Fede Álvarez has connected the Offspring to the Engineers and black substance in Prometheus, while stopping short of presenting every detail of its biology as settled canon.
What happens at the end of Alien: Romulus?
As Rain prepares to leave the station with Andy and Kay, Kay’s condition worsens after she uses the recovered substance. She gives birth to an egg-like pod, and the creature inside develops rapidly into the humanoid Offspring. It attacks the survivors and kills Kay. Rain fights to get it away from the ship and ultimately ejects it into space. The plot sequence is recapped by Slashfilm.
Rain and Andy survive. Their ship continues toward Yvaga, and Rain records a final voice log in which she says she intends to find a way to repair Andy. The ending therefore gives Rain an immediate escape, not a neat resolution: she has lost Kay and is still carrying the responsibility of keeping Andy functioning.
Why did Kay’s baby become the Offspring?
The film shows a direct sequence: Kay takes the black substance to treat her injuries, her pregnancy accelerates, and the Offspring emerges. That makes the substance the story’s cause of the transformation, but the movie does not provide a detailed biological account of how each stage works.

Álvarez’s explanation is a creative-intent explanation rather than a complete in-universe mechanism. In an interview with Variety reported by The Independent, he said he hoped viewers would recognize the connection to the Engineers and described the black goo as the root of the Xenomorphs and Engineers. He said the mixture made sense as a new species. Read his explanation in The Independent.
What is the creature at the end of Alien: Romulus?
The Offspring is best described as a new, human-like creature combining visual and conceptual elements associated with humans, Xenomorphs, and Engineers. Álvarez said it was probably a new species and pointed to the Engineers and the substance from Prometheus; he did not identify it simply as an Engineer or give a definitive taxonomy for it.

Effects supervisor Shane Mahan described the design as a considered fusion. The team weighed a more Xenomorph-like appearance against an Engineer-like one, then settled on a more human look with Engineer-like black eyes. Mahan also said rapid growth was chosen to make the creature’s development plausible within the story’s short timeline. Those production comments explain the look and pacing, not broader rules for franchise biology. Space.com’s interview with Mahan discusses those design choices.
How does Alien: Romulus connect to Prometheus?
The clearest connection is the black substance and Álvarez’s stated intention for the Offspring to evoke the Engineers. His comments link the creature’s conception to the ideas and imagery of Prometheus, but they do not establish that the Offspring is a direct copy of a specific creature from that film or fully explain how the substance produces this exact outcome.

The ending also echoes the film’s opening image: a spacecraft heading into space. Álvarez told Collider he wanted the opening and ending images to bookend the film around the same concept; an accessible reproduction of that interview appears on Filmibee. That is a production explanation of the visual structure, distinct from the events themselves.
Does Rain survive at the end of Alien: Romulus?
Yes. Rain survives the confrontation and leaves with Andy for Yvaga after ejecting the Offspring into space. The final voice log makes clear that she plans to seek a way to repair Andy, so their survival is paired with an unresolved practical problem.

Why does the ending feel like a return to horror?
Álvarez described his goal as bringing Alien back to the simplicity of the franchise’s beginning, where it was “above all things, a horror movie.” The Walt Disney Company published that statement on August 16, 2024, alongside his comments about combining practical effects, animatronics, and miniatures with computer-generated work. His stated goal helps frame the finale’s emphasis on bodily transformation and a creature that becomes more threatening as the characters try to escape; it is not evidence that every viewer will interpret the sequence the same way. See The Walt Disney Company’s interview with Álvarez.
Alien: Romulus was directed by Fede Álvarez and co-written by Álvarez and Rodo Sayagues. The official 20th Century Studios listing gives its release date as August 16, 2024, its rating as R, and its runtime as 1 hour 59 minutes. The studio page also provides the official credits and premise.
