Sam’s last choice is a deliberate distraction. The creature’s approach and the cut to credits strongly suggest that she dies, but the film does not depict the attack. Writer-director Michael Sarnoski discussed the ending as containing death, while explaining that he wanted Sam’s final decision to feel earned and triumphant.

Reuben dies earlier in the film. Noise from a backup generator draws a creature into the group’s shelter, where it kills him.
Why does Sam stay behind?
Sam knows she is dying and chooses how to spend the time she has left: helping Eric and Frodo escape. Sarnoski told GamesRadar+: “I wanted an ending that on paper seems kind of bleak, just like the rest of the movie. In the end, she commits suicide, but I wanted it to feel triumphant and feel like this was her decision. And this was something that she earned over the course of this of this story.”

What does Frodo mean to Sam?
Frodo is more than someone Sam wants to protect. Sarnoski has described the cat as a connection to Sam’s past and as part of the connections she becomes willing to make with other people. Handing Frodo to Eric therefore serves two purposes: it gives him a better chance of surviving, and entrusts someone else with a bond Sam values.
