In a November 12, 2024 interview published after 9-1-1: Lone Star season 5, episode 7, co-showrunner and executive producer Rashad Raisani previewed two developing stories: Tommy Vega’s breast cancer diagnosis and the strain building for Judd Ryder. He said Tommy’s absence would reshape the 126 and Nancy Gillian’s role, while Judd’s struggles would surface more openly in episode 10. Read Patrick Gomez’s interview.
Tommy steps away, leaving Nancy to lead
Tommy, played by Gina Torres, steps away from her EMT captain role while battling breast cancer. Raisani described her absence as a “profound loss” for the 126 and called her “sort of the spiritual beating heart of that firehouse.”
With Tommy gone, Nancy, played by Brianna Baker, has to step into a leadership role without the person she depends on most. Raisani framed this as a turning point for Nancy, while emphasizing that the cancer story would affect the entire team rather than Tommy alone.
The 126 will support Tommy as she faces coverage obstacles
Raisani previewed an episode 8 storyline in which the 126 stands up for Tommy amid insurance coverage denials and technicalities. He also said the production worked with Stand Up to Cancer on the show’s cancer stories, seeking to make them feel plausible, humane, and correct.
#1 Best Overall
That was a preview, not a description of Tommy’s whole season arc. The interview was published in November 2024, after episode 7, and its episode 8 developments were still ahead at that point.
Judd’s trouble is building toward episode 10
Judd, played by Jim Parrack, is struggling while his wife Grace is away working with Mercy Ships. The interview points to him drinking alone at TK’s birthday party and to earlier scenes that planted clues about his state of mind.
Rank #2
Raisani said a fuse had been quietly lit in Judd and that the pressure would come into the open in episode 10. His remarks described a seeded storyline moving toward a payoff, not a standalone episode announcement.

Spoilers for the penultimate episode: In a January 28, 2025 interview with Variety, syndicated by Yahoo Entertainment, Raisani discussed a later phase of Tommy’s story. Her cancer had worsened despite treatment, and her fate appeared uncertain before the series finale. He also discussed bringing her late husband Charles back as part of an arc returning to their relationship. Read the later interview.

Rank #3
The November interview is a season 5 preview from the point after episode 7; the January interview addresses events around the penultimate episode and finale. They should not be treated as the same announcement: the earlier episode 8 and episode 10 previews do not, by themselves, explain how Tommy’s story ends.
