What happened in Matthew Perry’s final hours?
The available timeline is approximate. The sequence of events before Perry was found comes from an Associated Press account based on court documents, the medical examiner’s investigation and law-enforcement reports; it is not an official minute-by-minute reconstruction.
About 8:30 a.m.
AP reports that Perry’s assistant, Kenneth Iwamasa, administered an injection using syringes associated with doctor Salvador Plasencia and ketamine AP says came from Jasveen Sangha. This is AP’s account of investigative and court records, not a finding in the medical examiner’s report.
About 11 a.m.
Perry played pickleball, according to AP’s chronology.

About 12:45 p.m.
AP reports that Iwamasa gave Perry a second injection, after which Perry began watching a movie.
About 1:30 p.m.
AP says Iwamasa administered a third injection while Perry was sitting in or near the backyard Jacuzzi, then left to run errands. In AP’s account, Iwamasa recalled Perry asking, “Shoot me up with a big one.” That is reported recollection, not a recording or an official medical finding.
About 4 p.m.
AP reports that Iwamasa returned and found Perry face down in the Jacuzzi. The county medical examiner’s report says Perry was found unresponsive in the pool at approximately 1600 hours; paramedics responded and pronounced him dead.
What did the medical examiner determine?
The Los Angeles County Department of Medical Examiner listed the acute effects of ketamine as the cause of death. It identified drowning, coronary artery disease and the effects of buprenorphine as contributing factors, and classified the manner of death as an accident. These are the official medical findings; they do not, by themselves, establish how the ketamine was obtained or who supplied or administered it.

What is known about the ketamine treatment and supply?
AP reported that Perry’s last legal ketamine treatment from his regular physician occurred around October 20, and described the injections on October 28 as part of a separate, nonmedical supply chain. AP also described repeated injections during the final week. Those details are reported from investigative and court records; they are not steps established by the medical examiner’s cause-of-death finding.
Federal prosecutors separately alleged conduct involving ketamine sales and injections. The U.S. Department of Justice’s August 2024 announcement described charges and allegations, and its page was updated March 24, 2025. It is not a current court docket. AP’s May 2026 chronology reports that the prosecution concluded with sentencing. Prosecutorial allegations about the supply chain and other people’s conduct should not be confused with the medical examiner’s determination of cause and manner of death.
