Spoilers for NCIS Season 21, Episode 10, “Reef Madness.” The Season 21 finale, which aired on May 6, 2024, strands Alden Parker and Jessica Knight aboard a sinking artificial-reef ship, brings Parker’s life dangerously close to its end, and leaves Knight facing a career decision that could change the team. The immediate murder case is solved and both agents survive—but Knight’s future with the Washington team, her relationship with Jimmy Palmer, and the identity of “Lily” remain unresolved.

What happens in the ‘NCIS’ Season 21 finale?

The episode begins aboard the USS Goodell, a decommissioned Navy ship scheduled to be sunk and transformed into an artificial reef. Explosive charges have been placed aboard the vessel to open its hull before the planned sinking.

Before the ship reaches that point, the NCIS team finds three bodies inside: teacher Susan Sidor, construction worker Thomas Mackin, and an unidentified unhoused man. The victims appear to have died in completely different ways—one was stabbed, another suffered blunt-force trauma, and the third was struck by a vehicle. The apparently unrelated deaths make the case difficult to connect at first.

The investigation eventually ties the victims together and leads the team to the people responsible for the murders. But the finale’s larger dramatic engine is what happens after Parker and Knight return to the ship to investigate a clue that had initially been overlooked.

Why do Parker and Knight return to the ship?

While reviewing a crime-scene photograph, Knight and Parker spot a small piece of evidence that may have been missed during the original search. They go back to the USS Goodell without the rest of the team, hoping the overlooked detail will clarify the case.

Instead, the ship becomes a trap. Parker and Knight pursue a suspect—or at least a mysterious figure—inside the vessel and are locked in the engine room. A piece of metal pins Parker’s leg, and Knight realizes that the injury may have severed or damaged an artery. She uses her belt as a tourniquet, buying him time while they search for a way out.

The danger is made worse by the ship’s condition. The passage they need to use is flooded, communications are unreliable, and Parker is losing blood. Their investigation has become a survival operation.

How does the finale use The Poseidon Adventure?

The episode’s title, “Reef Madness,” points to the artificial-reef operation, but the main action sequence also deliberately recalls the 1972 disaster film The Poseidon Adventure. Parker references Shelley Winters and the movie when Knight prepares to swim through the flooded section of the ship.

This is not a literal recreation of the film’s capsized ocean liner. The USS Goodell is not shown being overturned in the same way. Instead, the comparison is a character-driven joke that becomes increasingly uncomfortable as Knight faces a real possibility of drowning while trying to save Parker.

For viewers who want the reference’s full context, The Poseidon Adventure is the related 1972 film Parker mentions—not an NCIS tie-in or a claim that the finale directly adapts its plot.

Knight risks her life to save Parker

With Parker’s condition deteriorating, Knight decides that the only way to reach help is to swim through the flooded passage. She leaves Parker behind temporarily, knowing that he may not survive long enough for her to find assistance, and enters the submerged part of the ship.

The underwater escape is the episode’s central set piece. Knight must navigate the flooded vessel while trying to preserve enough strength and air to reach the rescuers. Her objective is not simply to escape: she has to get medical help back to Parker before his leg injury becomes fatal.

Back at NCIS, Palmer, McGee, and Torres realize that Parker and Knight are missing. Palmer insists on joining the search because he does not want the angry words he said to Knight to be the last thing she hears from him. The team tracks the agents’ vehicle and returns to the ship.

Knight eventually reaches a point where she can signal the rescuers. The team hears banging from inside the vessel and pulls her out. They then find Parker unconscious and not breathing. He is revived and later reported to be recovering in the hospital.

Who is Lily in Parker’s vision?

While trapped and badly injured, Parker sees a young girl in a yellow dress. He calls her Lily. The apparent hallucination then shifts into what looks like a childhood memory: a young Parker is playing with Lily on a ship while his mother calls his name.

The finale intentionally refuses to explain what viewers are seeing. Lily could be connected to Parker’s childhood, but the episode does not establish whether she was a real person, a memory altered by trauma, or a hallucination created by his injury. It also does not confirm that she is a ghost or any other supernatural figure.

After Parker is safe, Knight asks him about Lily. Parker says that he does not remember talking about her and does not know who she is. That response preserves the mystery rather than solving it. The creative team has indicated that Lily was intended to remain a thread for future exploration, making the vision one of the finale’s clearest continuing storylines.

What happens between Knight and Palmer?

Alongside the ship case, Knight is dealing with a major professional decision. Director Leon Vance offers her the position of chief REACT training officer at Camp Pendleton in California. Knight had previously applied for the job after her former REACT team was killed, but she now hesitates because the Washington NCIS team has become her family.

Vance wants her answer by the end of the day. The timing is especially difficult for Palmer, who is already struggling with parenthood and with the deaths of Breena, Gibbs, Ducky, and other people close to him.

When Palmer learns about the offer, he does not ask Knight to stay. Instead, he tells her to take the job and says that he is losing her on his own terms. The exchange is an emotional rupture, but the finale does not present a clean, formally confirmed, permanent breakup.

Kasie later challenges Palmer’s reaction. She points out that he made Knight’s career decision about his own fears. Palmer recognizes that his outburst was driven by anxiety and self-protection. He had been expecting Knight to leave ever since her father’s visit, and forcing the issue gave him a painful sense of relief.

That insight does not instantly repair the relationship. When Parker and Knight return to the ship, Parker tells Knight that Palmer’s words may have been an anxiety attack rather than a final statement about their future. Knight, however, believes Palmer meant what he said. The episode leaves their relationship suspended between love, hurt, and the possibility of long-distance life.

Does Knight leave NCIS?

Yes—at least in the immediate ending, Knight accepts Vance’s offer. After Parker survives and Knight reflects on the danger of allowing fear or regret to make her choices, she gives Vance an emphatic answer: “Hell yeah, sir. Let’s do it.”

The position would take her to Camp Pendleton, California, as chief REACT training officer. That could remove her from the Washington team and place significant distance between her and Palmer.

However, the finale does not establish that Katrina Law is leaving NCIS permanently or that Knight will never return. The job is designed as an ambiguity around both Knight’s professional future and her relationship with Palmer. Knight’s decision also fits the character’s ambition: she does not want to reject an important opportunity and later resent the life she chose instead.

So the accurate answer is that Knight accepts the California assignment, while her long-term status as a regular presence in Washington remains unresolved by the episode itself.

How does the murder case end?

The immediate case is resolved. The apparently disconnected victims aboard the USS Goodell are linked by the investigation, and the NCIS team identifies the people responsible for the murders and for trapping Parker and Knight aboard the ship.

The finale deliberately gives more attention to the personal and survival story than to a lengthy procedural reconstruction of every step. Its important case outcome is that the team solves the murders and reaches the trapped agents in time to save them.

Why the finale works as a season-ending cliffhanger

“Reef Madness” closes the case but leaves the characters in motion. Parker survives, yet his brush with death introduces Lily, a mystery that may reveal something important about his childhood. Knight survives the flooded ship, but accepts a career that could take her away from the team. Palmer realizes that his fear shaped the conversation, but that realization does not erase the damage.

Those threads are stronger than a simple “who was the killer?” resolution because they affect the show’s ongoing relationships and character history:

  • Parker and Lily: Who was the girl in the yellow dress, and why did she appear to him on the ship?
  • Knight’s career: Will the Camp Pendleton assignment become a permanent new role or a temporary change?
  • Knight and Palmer: Can their relationship survive the distance and the emotional rupture?
  • The Washington team: How will Knight’s possible absence change the group’s balance?
