Zac Efron said his more prominent jaw was caused by enlarged masseter muscles—muscles used for chewing—that grew as they compensated for other facial muscles after a serious jaw injury. He connected the change noticed by viewers in 2021 to that injury and his rehabilitation, not to a reported cosmetic procedure. What Zac Efron said caused the change
In a September 2022 interview with Men’s Health, Efron explained that he had been doing facial physical therapy after injuring his jaw. While in Australia in 2021, he said, he took time away from his usual therapy. He described the facial muscles as working together, with his masseter muscles compensating for other muscles that were not fully healthy. “The masseters just grew,” he said. “They just got really, really big.”

The earlier accident
Efron dated the accident to November 2013. As summarized by Vanity Fair, he said he was running at home in socks when he slipped, hit his face on a granite fountain and lost consciousness. Entertainment Tonight later reported that his mouth had to be wired shut. These details come from Efron’s account and the outlets’ reporting.

The 2021 appearance and 2022 explanation
Viewers began commenting on Efron’s jaw after his appearance in a Bill Nye Earth Day musical video on Facebook Watch in April 2021. The Men’s Health interview, published in September 2022, connected that change in appearance to his earlier injury and his account of facial rehabilitation.
On September 14, 2022, Efron spoke with Entertainment Tonight at the Toronto International Film Festival while promoting The Greatest Beer Run Ever. He said he learned about the plastic-surgery speculation from his mother and described the accident as nearly fatal: “I almost died, but we’re good.” The remark is his description of the accident, not a medical assessment.
