Sakura understands how deeply Naruto cares for her, but the story does not show her returning those feelings romantically. Her own romantic storyline remains centered on Sasuke.
What the story says about Sakura and Naruto
The official Naruto site’s Sakura birthday column connects two events: Sakura entrusts Naruto with the responsibility of retrieving Sasuke, and Sai’s words later help her realize the depth of Naruto’s feelings toward her. It establishes that Sakura comes to understand Naruto’s devotion; it does not say that she reciprocates it romantically.
Why her romantic focus is Sasuke
Sakura’s choices and related story material keep her connection to Sasuke central. VIZ describes Naruto: Sakura’s Story—Love Riding on the Spring Breeze as a story in which Sakura works to clear Sasuke’s name and bring the perpetrator to justice. That novel description is supporting context for her continued investment in Sasuke, not a complete account of the original manga’s romance or proof of every aspect of her feelings.
Does Sakura care about Naruto?
Not returning Naruto’s romantic feelings is different from feeling nothing for him. The official recap supports the narrower conclusion that she recognizes his feelings while entrusting him with a task concerning Sasuke. It does not offer a detailed explanation of Sakura’s inner romantic reasoning, so claims about a particular psychological motive go beyond what these sources establish.

Did the creator explain why they do not end up together?
The available passage from an official Naruto site interview with Masashi Kishimoto discusses the resolution of Naruto and Sasuke’s conflict, not Sakura’s romantic motivation. It therefore cannot be used as a direct creator explanation for why Sakura does not choose Naruto.
