Why Does RFK Jr. Sound Raspy? Spasmodic Dysphonia Explained

RFK Jr.’s raspy, tremulous voice has been publicly attributed to spasmodic dysphonia. Here’s what the condition is, how clinicians assess it, and what treatments can help.

Robert F. Kennedy Jr.’s raspy, sometimes tremulous voice has been publicly attributed to spasmodic dysphonia, a neurological voice disorder also called laryngeal dystonia. It causes involuntary spasms in muscles that control the vocal folds, disrupting speech. That reported diagnosis is not something a listener can confirm from a recording alone.

What is spasmodic dysphonia?

Sound is produced when air from the lungs makes the vocal folds vibrate. In spasmodic dysphonia, involuntary spasms affect laryngeal muscles that control those folds. The spasms interrupt speech and may make a voice sound strained, tight, breathy, or broken. Some people also have a vocal tremor. Severe spasms can affect nearly every word. The National Institute on Deafness and Other Communication Disorders (NIDCD) describes the condition as a neurological voice disorder.

“Raspy” is a listener’s description, not a diagnosis. Hoarseness or raspiness can occur with many voice problems, including infections, reflux, vocal overuse, growths, and vocal-fold paralysis. A familiar sound—or a short video clip—cannot establish which cause is responsible.

What has been reported about Kennedy’s voice?

In a 2004 interview, Kennedy described having “a tremble in my voice … particularly when I first start talking,” according to Georgia Public Broadcasting’s Joe Hernandez. Hernandez reported that Kennedy later received an official spasmodic dysphonia diagnosis. In 2005, Kennedy said he was receiving injections about every four months. These are public statements and reporting, not an independent review of his medical records. Georgia Public Broadcasting’s report recounts those comments.

The Washington Post, citing experts, reported that Kennedy has the adductor form of the disorder. That description should likewise be understood as expert-attributed reporting rather than a claim based on access to his clinical chart.   • Abductor spasmodic dysphonia: The vocal folds spasm open during speech. The voice may sound breathy or interrupted.

Symptoms vary, and the sound of a person’s voice alone does not reliably identify the type. The American Academy of Otolaryngology–Head and Neck Surgery’s ENT Health resource describes these forms and their effects on speech.

What causes it?

The exact cause of spasmodic dysphonia is unknown. Medical sources describe the neurological involvement and ongoing research, but they do not establish a specific cause for Kennedy’s individual condition. A possible trigger should not be treated as a proven explanation.

How is spasmodic dysphonia diagnosed?

Diagnosis involves a clinical assessment rather than simply matching a voice to a sound sample. An ear, nose, and throat specialist (ENT or otolaryngologist) and a speech-language pathologist may review symptoms and health history, listen to speech, and ask the person to read or repeat particular phrases. A clinician may use a scope to see vocal-fold movement. Depending on the case, assessment can also involve a speech-language pathology evaluation, laryngeal electromyography, or a neurologist.

Because hoarseness has many possible causes, persistent voice changes deserve medical assessment. NIH’s Voice Disorders guidance advises seeing a doctor for hoarseness lasting more than three weeks, especially when it is not associated with a cold or flu.

Can it be treated?

There is currently no cure identified in NIH guidance, but treatment can reduce symptoms or make communication easier. The choice depends on the person’s symptoms and circumstances; no option works best for everyone.

Option What it does Duration or trade-offs
Botulinum toxin injections Injects the affected laryngeal muscles to reduce spasms; this is the most common treatment. NIH says improvement generally lasts three to four months, after which symptoms return and repeat injections may be needed. Temporary effects can include a weak or breathy voice and occasional swallowing problems.
Voice therapy Works with speech and voice techniques to reduce symptoms or support communication. May reduce symptoms in mild cases or complement injections; it is not presented as a cure.
Surgery May be considered in selected circumstances, including when conventional measures have not helped enough. Outcomes vary. NIDCD says comparative studies do not identify one best surgical treatment.
Voice amplifier or text-to-speech tool Can help some people communicate more easily; these are assistive aids, not treatments for the disorder. Whether an aid is useful depends on the person’s communication needs.
