Dr. Neil Melendez dies in the Season 3 finale of The Good Doctor after injuries from a brewery collapse cause internal bleeding and bowel ischemia. The creative reason was separate: showrunner David Shore said the writers wanted a loss that would deeply affect the characters and audience, but he did not name a single specific reason Melendez had to be the one who died.
What caused Dr. Melendez’s death in the story?
In the Season 3 finale, “I Love You,” Melendez is injured when a brewery collapses. Claire discovers that he is bleeding internally, and the medical team finds bowel ischemia. Dr. Aaron Glassman tells Dr. Lim that even aggressive treatment would leave Melendez with little quality of life. His colleagues visit to say goodbye; Melendez and Claire acknowledge their feelings before he dies. ABC’s finale recap describes this sequence.

That is the fictional medical cause presented by the episode. It should not be taken as evidence that his death was medically inevitable outside the show’s dramatic premise.
Why did the writers choose to kill him off?
Showrunner David Shore said the decision followed extensive discussion and back-and-forth among the writers. Asked if there was a specific reason they selected Melendez, Shore declined to give one. He explained that they wanted the death of a character whose loss would be felt, noting Melendez’s connections with Lim, Claire, and Shaun.

“We wanted to lose a character that we would feel something about, and feel pain about, and certainly, he’s a character that so many of them have been in touch with.”
Shore also acknowledged that this explanation would not persuade every viewer to agree with the choice. In a separate Deadline interview, he described the decision as an ensemble-impact choice, not a uniquely necessary conclusion to Melendez’s personal story.

Was his death meant to shake up the show?
Yes. Shore told TVLine that he wanted to unsettle the audience and put the characters in challenging situations rather than keep the series too stable. The death therefore served as a major emotional disruption for the ensemble. Shore’s public explanation establishes that aim, but not every private consideration in the writers’ room.

The cited interviews do not establish that actor Nicholas Gonzalez asked to leave, that there was an actor conflict, or that any one private circumstance drove the decision. Shore specifically declined to identify a more particular reason for choosing Melendez. It is more accurate to distinguish the episode’s medical explanation from the showrunner’s stated creative rationale than to present an unverified behind-the-scenes account as fact.
