Lily is a real girl from Alden Parker’s childhood. She first appears in Parker’s delirious memory in the Season 21 episode “Reef Madness,” and the Season 22 episode “Moonlit” ties her to the scene of his mother Eleanor’s fatal car crash. Parker recognizes Lily among the bystanders in a newspaper photograph of the crash, which turns his recurring visions into a clue about his buried family history.

The title calls “Reef Madness” the Season 21 finale. CBS’s episode guide numbers it Episode 10, so the “finale” label is not confirmed by that listing.
Season 21: where the vision starts
In “Reef Madness,” Parker and Jessica Knight are trapped aboard a ship, and Parker is badly injured. While he is delirious, he sees a girl from his childhood. The image is ambiguous at this point. It looks like a fever dream, and the episode gives no name or context for the girl.

Co-showrunner Steven D. Binder told TheWrap in a May 6, 2024 interview that the backstory existed before the on-screen payoff:
“But this is not one of those times. This is one of those times where the backstory was thought of first and we had to wait for the right moment to put it in an episode.”

Binder’s comment establishes production intent that the Lily backstory was planned. It is not a full explanation of who Lily is. He also described her as a childhood friend of Parker’s.
Season 22: Parker still doesn’t know her
In “Out of Control,” Parker tells his therapist that he does not know who Lily is. He had initially assumed the image was a fever dream tied to blood loss and pain, but the visions keep coming back. That gap is what keeps the question alive into the rest of the season. CBS’s watch page for the episode describes Parker as still seeing visions of Lily.

Season 22: the photograph in “Moonlit”
“Moonlit” is the episode that answers the question. Parker has Jimmy Palmer sketch Lily, and later he looks up a newspaper report about Eleanor’s fatal crash. In a photograph from the scene, he recognizes Lily among the people there.
According to TV Insider’s recap, this is the clearest on-screen connection in the Season 22 arc. The photo links the childhood image to the crash itself. Parker’s visions stop being only a symptom of his injuries and become a path back to his mother’s death.

What the reveal establishes and what it leaves open
The photograph confirms three things. Lily was a real person. She was at the crash scene. And her presence is the reason Parker’s memory of her connects to his mother.
It does not explain everything. The Season 22 reveal does not say why Lily was at the scene, what her connection to Parker’s family was beyond childhood, or what happened to Eleanor beyond the fact of the fatal crash. Those questions are the reason the mother storyline continued.

The listing does not provide the full explanation, and this article does not treat Season 23 as settled by the Season 22 reveal.
