The “Tetris effect” is an informal name for mental carryover after long stretches of repetitive play: falling shapes or puzzle-like thoughts that show up when you rest, close your eyes, or drift toward sleep. It is a descriptive phrase, not a diagnosis. Separately, a small group of trauma studies has tested a short, structured task that uses Tetris to reduce unwanted, vivid memories of a traumatic event. That task is not the same as playing the game, and the evidence does not show that ordinary Tetris treats PTSD.
The everyday meaning of the “Tetris effect”
People use the term for a familiar experience: after a marathon session of a falling-block puzzle game, the shapes keep appearing in your mind as you try to fall asleep, or you catch yourself seeing blocks when you close your eyes. The term describes the pattern of the experience. It does not describe a condition, and it is not a clinical category.
A brief image or thought linked to a recently repeated activity is not, on its own, evidence of a mental-health disorder. The sources reviewed for this article do not establish how common the experience is, how long it lasts, or where it becomes a concern, so any figure you see for those points should be treated with caution.
The picture changes when the images are persistent, distressing, linked to a traumatic event, or interfering with sleep or daily life. In those cases, a doctor or mental-health professional is the right place to start. Established treatments for trauma-related symptoms exist, and none of the studies below replaces them.

Where Tetris enters trauma research
The clinical connection is not about bedtime imagery. It concerns trauma-related intrusive memories: involuntary, vivid recollections of a traumatic event that return without warning. Researchers have tested a procedure in which a short reminder of a traumatic memory is followed by a visuospatial task, with Tetris as the game. The proposed logic is that the visual-spatial demand competes with the visual imagery that fuels the intrusion. Three studies illustrate the evidence, each in a different population.
2017: emergency department proof-of-concept trial
A 2017 randomized proof-of-concept trial tested a trauma-memory reminder followed by about 20 minutes of Tetris, delivered in an emergency department within six hours of a motor-vehicle accident. The comparison group completed a written activity log. The published abstract reports fewer intrusive memories over the following week and a faster decline in how often they occurred. The effect was consistent on one intrusion-symptom measure at one week but was not found on other symptom clusters or at one month. The authors said a larger trial was warranted.

2024: analogue trauma-film pilot
Asselbergs and colleagues, writing in the Journal of Behavior Therapy and Experimental Psychiatry (2024), randomized 60 healthy students who watched a trauma film to one of three conditions: a Tetris dual-task, imagery rescripting, or no intervention. At one week, the Tetris group reported fewer film-related intrusions than the no-intervention group. There was no effect on the vividness or emotionality of the memories, on PTSD-like symptoms, on intrusion intensity, or on explicit memory. Because participants watched a film rather than lived through trauma, the study is an analogue test, and its small sample limits how far the results generalize.
2026: healthcare-worker trial reported by UKRI
On 25 February 2026, UK Research and Innovation (UKRI) reported a randomized trial of 99 healthcare workers who had been exposed to trauma at work during the COVID-19 pandemic. The trial tested the Imagery Competing Task Intervention (ICTI). According to UKRI, the ICTI group had ten times fewer intrusive memories than either control group four weeks after starting, and 70% of ICTI participants reported no intrusive memories at six months. UKRI also reports improvement in PTSD symptoms. These are figures from UKRI’s news report, not from a full peer-reviewed paper, and the complete outcome tables were not available when this article was written. The trial was led by Emily Holmes, Professor of Psychology at Uppsala University.

The structured protocol is not casual play
According to UKRI’s description, the ICTI involves three steps, and the game is only the last of them:
- Recall the memory briefly. The person calls up the traumatic memory without describing it in detail.
- Learn mental rotation. The person practises rotating objects in the “mind’s eye,” a visuospatial skill.
- Play Tetris in a specific, slower way. The person uses the rotation skill during a deliberately paced session, not during typical gameplay.
The proposed explanation is that the visuospatial processing competes with visual flashback imagery. That is the intervention’s proposed mechanism, not evidence that an unstructured game session produces the same effect.

Holmes described the difference directly: “It is far more than just playing Tetris, and while it is simple to use, it’s been a complicated process to refine and develop.”

What the evidence does not establish
- Prevention or treatment of PTSD in general. Each study measured intrusive memories, and in some cases a narrow set of symptoms. None shows that Tetris prevents or treats PTSD as a whole.
- Durable benefit. The longest follow-up reported here is six months, and it comes from a single news report.
- Generalization beyond these groups. The populations studied were students watching a film, people treated shortly after an accident, and healthcare workers in one trial. Other trauma types and settings remain untested in these reports.
- Replacement of care. The studies are trials of a clinician- and researcher-developed protocol, not guidance for people to run on their own.
Emily Holmes said the intervention “focuses on our mental imagery, not words, and is designed to be as gentle, brief and practical as possible to fit into people’s busy lives.” Tayla McCloud, Research Lead for Digital Mental Health at Wellcome, responded to the 2026 results with a conditional: “If we can get similarly strong results in bigger trials, this could have an enormous impact.” That conditional is the honest summary of where the evidence stands. The promising findings need replication in larger, independently reported trials before they can be treated as established.

If you are dealing with intrusive memories after a traumatic event, the practical step is to speak with a doctor or a trauma-trained clinician, who can assess your symptoms and discuss established options. Playing Tetris on your own is not a substitute for that assessment.
A physical or digital Tetris game is relevant here only as the game used in these studies. Buying one has no therapeutic value on its own.
