Kratom is a psychoactive tree native to Southeast Asia. Its main studied compounds act on opioid receptors in the brain, and reported effects range from increased energy and alertness to sedation, nausea, and confusion. It can also be associated with dependence, withdrawal, and other serious harms. Kratom has not been shown safe and effective as a medical treatment for pain, opioid withdrawal, or other conditions.

What kratom is

Kratom is the common name for Mitragyna speciosa, a tree in the coffee family found in Thailand and neighboring countries. In parts of Southeast Asia, people have traditionally chewed its leaves or prepared them as tea, including in contexts involving fatigue, work productivity, ceremonies, and symptoms. Those historical uses do not establish that kratom is a safe or effective treatment.

Kratom contains many alkaloids. Research and regulatory attention focus chiefly on mitragynine and 7-hydroxymitragynine (7-OH). Both interact with mu-opioid receptors in the brain, but products sold as kratom are not necessarily alike: ordinary leaf preparations differ from products that contain added or concentrated 7-OH.

This distinction matters when interpreting claims about kratom. A traditional leaf preparation, a product marketed as an extract, and a product with enhanced 7-OH levels cannot simply be treated as identical. NCCIH and FDA descriptions do not establish that their effects or risks are interchangeable. Product names and marketing do not, by themselves, confirm the ingredients or concentrations in a particular item.

How kratom can affect the brain

NCCIH reports that mitragynine and 7-OH interact with opioid receptors. The FDA also describes mitragynine activity involving serotonin, dopamine, norepinephrine, and kappa opioid receptors. These interactions help explain why reported effects can vary and may resemble stimulation in some circumstances and opioid or sedative effects in others. The precise effects depend on factors such as the amount and circumstances of use; not everyone experiences the same effects. NCCIH’s kratom overview and the FDA’s information on kratom summarize the current agency descriptions.

People report stimulant-like effects such as increased energy, alertness, and a rapid heart rate. Others report relaxation or pain relief, while FDA and NCCIH also describe effects such as confusion, sedation, nausea, and constipation. These descriptions are reports of possible effects, not a predictable sequence that every person will experience. The agencies’ summaries do not provide a reliable way to forecast an individual response from a product label or a general description of kratom.

Reported effect pattern Examples What the evidence supports
Stimulant-like Increased energy or alertness; rapid heart rate These are reported effects, not guaranteed outcomes.
Opioid- or sedative-like Relaxation, pain relief, confusion, sedation, nausea or vomiting, constipation These effects are consistent with opioid-related activity described by FDA; they should not be read as proof of medical benefit.

The receptor activity is relevant to safety as well as to the effects people seek. FDA describes possible opioid-related effects that include sedation, physical dependence, withdrawal, and respiratory depression. That does not mean every user will experience these outcomes, or that they occur at the same likelihood in every circumstance. It does mean that calling kratom a botanical does not remove the need to consider its psychoactive and opioid-related effects.

Effects on the body and safety risks

Commonly reported effects and serious reactions

NCCIH lists nausea, constipation, dizziness, and drowsiness among reported mild effects. Rare but serious reports include seizures, high blood pressure, and liver problems. FDA notes possible opioid-related effects including respiratory depression, as well as nausea or vomiting, constipation, physical dependence, and withdrawal. These are potential outcomes, not a list of effects every user will experience.

The evidence also needs to be read carefully: “reported” does not mean that an effect is inevitable, and it does not tell a reader how often the effect occurs. The official summaries describe a range of mild effects and serious reports, but the sources cited here do not establish a population-wide risk rate for each outcome. A short list of effects should not be mistaken for a complete prediction of what will happen to an individual.

NCCIH says fatal overdose from kratom alone appears extremely rare, but combinations with other drugs have been associated with deaths and severe adverse effects. The nature and likelihood of interactions are not fully established, and more research is needed. Do not assume that combining kratom with a medicine or another substance is safe.

This is an important distinction: a statement that deaths from kratom alone appear extremely rare is not a finding that kratom is harmless, and it should not be generalized to combinations. NCCIH specifically notes severe outcomes linked to use with other drugs while also saying that more research is needed to understand interactions. The available evidence therefore does not support using a general claim about overdose risk to judge the safety of a particular combination.

Dependence, tolerance, and withdrawal

FDA has reported cases of kratom-related substance use disorder. Possible warning signs include cravings, taking more or using longer than intended, continuing despite harmful consequences, developing tolerance, and experiencing withdrawal after stopping. These reports establish a safety concern; they do not provide a population-wide prevalence rate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

In practical terms, these warning signs describe patterns that can matter even when someone initially uses a substance for a specific reason. Craving, increasing use beyond what was intended, and continuing despite adverse consequences are not evidence that kratom is treating the original problem. Tolerance and withdrawal are additional concerns FDA associates with kratom-related substance use disorder. The available source does not quantify how common these signs are or identify a dose at which they will occur.

FDA also notes reports of neonatal abstinence syndrome after prolonged exposure during pregnancy. This is a reported signal, not a quantified estimate of how often it occurs. Anyone who is pregnant or considering pregnancy should discuss substance exposure and safer care with a qualified clinician.

Long-term use and product contamination

Long-term effects are not well understood. Reports associate long-term use of large amounts with serious liver problems in some people, but the available information does not establish an individual’s risk. Harmful contaminants, including heavy metals and disease-causing bacteria, have been identified in some kratom products.

In a laboratory analysis, the FDA tested 30 kratom products and found significant lead and nickel levels exceeding safe-exposure references for oral daily drug intake. The agency says heavy users could potentially develop heavy-metal poisoning, with possible nervous-system or kidney damage, anemia, high blood pressure, or increased risk of certain cancers. This finding applies to the products tested; it does not mean that every kratom product is contaminated or that every user will be harmed. See the FDA’s laboratory analysis of kratom products for heavy metals.

The number and scope are worth stating precisely: FDA tested 30 products, and the agency reported lead and nickel levels above safe-exposure references for oral daily drug intake. That is a sample finding, not a survey proving that all products contain those metals. It is also not a measure of how many people have been harmed. FDA describes potential health consequences for heavy users if exposure leads to heavy-metal poisoning; those potential consequences should not be recast as outcomes that have occurred for every person using kratom.

Natural kratom leaf and enhanced 7-OH products are different

The word “kratom” can refer to botanical leaf preparations as well as products marketed with added or enhanced 7-OH. FDA says 7-OH occurs naturally in kratom in trace amounts, while its warnings address products with added 7-OH or enhanced levels. According to FDA, 7-OH is substantially more potent at mu-opioid receptors than mitragynine. Products may be concentrated, enhanced, or inaccurately labeled, so a product’s marketing name does not by itself establish what it contains.

The useful comparison is not a recommendation between two kinds of products. It is a reason to distinguish ordinary botanical leaf or preparations from products with added or concentrated 7-OH, and natural trace amounts from enhanced levels. FDA’s warning focuses on the latter category. A label or familiar product name is not assurance that a product has the composition a consumer expects, particularly when FDA says products may be inaccurately labeled.

FDA recommends that consumers avoid 7-OH products and says they are not approved for medical use. The agency’s concern is not a claim that every kratom leaf product is the same as a concentrated 7-OH product; rather, it specifically distinguishes natural trace 7-OH in kratom from added or enhanced levels. FDA’s July 29, 2025 announcement said its scheduling recommendation concerned certain 7-OH products and was not focused on natural kratom leaf products. Its 2025 announcement and consumer update on products containing 7-OH explain the distinction.

Regulatory status is time-sensitive. On a page updated July 13, 2026, FDA reported that DEA announced on July 1, 2026 the start of a temporary scheduling process for 7-OH above a proposed threshold and for three synthetic derivatives not naturally occurring in kratom: mitragynine pseudoindoxyl, MGM-15, and MGM-16. FDA also reported a separate HHS request for information on the proposed threshold. This describes an announced process, not a completed final scheduling determination. Consult the agencies’ current notices for the latest status. FDA’s 7-OH overview provides the dated account.

The distinctions in that update are specific: the reported process concerned 7-OH above a proposed threshold and named synthetic derivatives that do not naturally occur in kratom. It should not be summarized as a final decision covering every kratom leaf product. Because agencies can update the process, a dated description is more accurate than treating the status as settled or permanent.

Is kratom an established treatment for pain or opioid withdrawal?

No. People in Western countries use kratom in attempts to manage pain or opioid withdrawal, but NCCIH says it has not been shown safe and effective for those uses or for other medical purposes. FDA says it will continue to warn against kratom for medical treatment until safety and effectiveness can be evaluated. Few well-designed human studies have administered kratom, and its abuse potential is not fully understood.

Traditional use, personal accounts, and reports of symptom relief do not demonstrate that kratom treats a medical condition. Nor does opioid-receptor activity make it an established substitute for prescribed treatment. If you are seeking help for pain, opioid withdrawal, or substance use, speak with a qualified health professional about evidence-based options rather than starting, stopping, or replacing treatment on the basis of kratom claims.

In other words, a reported effect and a proven treatment outcome are different kinds of evidence. Reports that someone used kratom for pain or withdrawal do not establish that it safely or effectively treats either condition. The official sources cited here say that safety and effectiveness have not been established and note the limited number of well-designed human studies. They do not support dosing instructions or a recommendation to substitute kratom for care with demonstrated benefits.

What the evidence can—and cannot—tell you

  • Established at a broad level: Kratom is a psychoactive botanical, and its major studied alkaloids interact with opioid receptors. Reported effects include stimulant-like and opioid- or sedative-like experiences.
  • A real safety concern: FDA and NCCIH describe adverse effects, reports of dependence and withdrawal, possible interactions, and contamination concerns.
  • Not established: Kratom’s safety and effectiveness as a medical treatment, the full extent of long-term effects, and how frequently serious outcomes occur across the population.
  • Product-specific caution: Evidence about natural leaf should not be automatically applied to concentrated or enhanced 7-OH products, and findings in tested products should not be generalized to every product.

These limits are part of the answer, not a reason to assume that nothing is known. Agencies describe the main studied compounds, their receptor activity, and a range of reported effects and safety signals. At the same time, limited human study, incomplete information on interactions and long-term effects, and the absence of established medical benefit mean that those descriptions should not be turned into precise predictions or treatment claims.

Frequently Asked Questions

What does kratom do to your body?

Reported effects vary. They can include increased energy, alertness, and rapid heart rate, or relaxation, sedation, nausea, constipation, dizziness, drowsiness, and confusion. Serious reports include seizures, high blood pressure, and liver problems. Not everyone experiences the same effects, and the evidence does not predict an individual response.

Can kratom cause dependence or withdrawal?

FDA has reported cases of kratom-related substance use disorder and possible signs including cravings, using more or longer than intended, continuing despite adverse consequences, tolerance, and withdrawal after stopping. These reports indicate a safety concern, but they do not establish how common it is across the population.

Is kratom approved or proven to treat pain or opioid withdrawal?

NCCIH says kratom has not been shown safe and effective for pain, opioid withdrawal, or other medical uses. FDA says it will continue to warn against kratom for medical treatment until its safety and effectiveness can be evaluated. Traditional use and personal reports do not prove treatment benefit.

Are natural kratom leaf and 7-OH products the same?

No. FDA says 7-OH occurs naturally in kratom in trace amounts, while its warnings address products with added or enhanced 7-OH. FDA describes 7-OH as substantially more potent at mu-opioid receptors than mitragynine and recommends avoiding 7-OH products. Product labels may not reliably establish what a product contains.

Does FDA’s heavy-metal finding mean every kratom product is contaminated?

No. FDA tested 30 products and found significant lead and nickel levels above safe-exposure references for oral daily drug intake. This is a finding in the tested sample, not proof that every product is contaminated or that every user will be harmed.

The Bottom Line

Kratom is a psychoactive botanical whose major studied alkaloids interact with opioid receptors. Reported effects range from stimulant-like to opioid- or sedative-like, and safety concerns include adverse effects, dependence, withdrawal, interactions, and contamination. It has not been shown safe and effective as a medical treatment. Natural leaf and enhanced 7-OH products should not be treated as interchangeable, and agency updates on 7-OH scheduling remain time-sensitive.
